Der er et stigende antal patienter, som får langvarig warfarinbehandling til forebyggelse af arteriel tromboemboli. Den nuværende tilgang til perioperativ behandling af antikoagulering (dvs. "brobehandling") med lavmolekylært heparin (LMWH) er ikke standardiseret og er ikke blevet vurderet af tilstrækkelige randomiserede undersøgelser. De fleste klinikere anbefaler dog brobehandling.
Vi har for nylig afsluttet en multicenter-enarmspilotundersøgelse af LMWH-broterapi. Denne undersøgelse i 10 centre samlede 224 patienter på 10 måneder. I pilotstudiet var frekvensen af postoperative tromboemboliske hændelser 3,1 %, og 75 % af disse forekom hos patienter, som fik stoppet antikoagulation på grund af blødning.
Design: Et prospektivt multicenter randomiseret dobbeltblindt kontrolleret forsøg. Patienter: Konsekutive kvalificerede og samtykkende patienter fra 11 undervisningshospitaler i Canada. I alt 1773 patienter med hjerteklapproteser, der fik langvarig oral antikoagulering med warfarin, eller patienter med atrieflimren/fladder og en væsentlig risikofaktor, som kræver elektiv ikke-hjertekirurgi eller invasiv procedure, hvilket nødvendiggør reversering af deres orale antikoagulerende behandling.
Behandlingsplan: Samtykke vil blive indhentet præoperativt, men randomisering vil blive udført postoperativt efter bekræftelse af berettigelse.
Præoperativ periode: Hos alle deltagere vil warfarinbehandling blive afbrudt fem dage før proceduren. Dalteparin, en LMWH, vil blive administreret ved 200 IE/kg sc tidligt om morgenen i de tre dage forud for, men ikke dagen for, proceduren, undtagen dagen før operationen, vil dosis være 100 IE/kg givet 24 timer før operation. Warfarin vil blive genoptaget samme aften som proceduren.
Postoperativ periode: Dalteparin eller placebo vil blive administreret dagligt (startende morgenen efter proceduren), forudsat at kirurgisk hæmostase er opnået, og fortsættes i mindst fire dage og indtil INR er >2,0. Patienter, der anses for at have høj risiko for en postoperativ større blødning, vil få dalteparin eller placebo i en dosis på 5.000 IE sc dagligt. Patienter, der gennemgår procedurer, der anses for lav risiko for blødningskomplikationer, vil genoptage dalteparin eller placebo ved 200 IE/Kg s.c. daglige.
Resultater: Det primære resultat vil være hyppigheden af episoder med større tromboemboli over en 90-dages opfølgningsperiode efter randomiseringstidspunktet. Sekundære resultater vil omfatte større blødninger og generel overlevelse.
Relevans: At bygge bro eller ikke at bygge bro er et almindeligt klinisk spørgsmål uden randomiseret forsøgsbevis til at vejlede klinikere. Denne RCT vil svare på, om postoperativ brodannelse reducerer risikoen for tromboemboli eller forårsager skade.

Inklusionskriterier:
- Informeret samtykke,
- Patienter >18 år
- Patienter med protese(mekanisk) hjerteklap
- Patienter med atrieflimren eller atrieflimren og en væsentlig risikofaktor (tidligere TIA eller slagtilfælde, højt blodtryk, diabetes, i alderen >75, moderat/alvorlig venstre ventrikel dysfunktion)
- Som modtager langvarig oral antikoagulering og kræver elektiv ikke-hjertekirurgi eller en invasiv procedure med tilbageførsel af deres antikoagulerende behandling.
Ekskluderingskriterier:
- Tegn på aktiv blødning inden for de sidste 30 dage før ophør med warfarin.
- Blodpladeantal <100 x 109/L.
- Spinal eller neurokirurgi.
- Forventet levetid mindre end 3 måneder.
- Beregnet kreatininclearance <30 ml/min
- Patienter med behov for hjerteoperation.
- Flere protetiske (mekaniske) ventiler eller Starr-Edwards ventil eller protese (mekanisk) ventil med en historie med slagtilfælde eller TIA
- Anamnese med heparin-induceret trombocytopeni (HIT)
