HINTERGRUND:
Vor 1991 hatte sich wenig Forschung auf Gesundheitsprobleme konzentriert, die nur bei Frauen auftreten oder häufiger auftreten. Dies galt insbesondere für Studien zu chronischen Erkrankungen und deren Prävention bei reifen Frauen. Diese Zustände (koronare Herzkrankheit, Krebs und Osteoporose) sind die Hauptursachen für die Beeinträchtigung der Lebensqualität, Morbidität und Mortalität bei postmenopausalen Frauen in den Vereinigten Staaten. Das WHI wurde im Auftrag des Kongresses 1991 von den National Institutes of Health gegründet und ist im Office of the Director (OD) angesiedelt. Das Clinical Coordinating Center für die klinische Studie/Beobachtungsstudie wurde im September 1992 finanziert und die 16 Vanguard Clinical Centers wurden im März 1993 finanziert. Das ursprüngliche Protokoll wurde gemeinsam vom Clinical Coordinating Center und dem Program Office entwickelt und am 20. April 1993 vom Investigators Committee überprüft und genehmigt. Weitere klinische Zentren wurden 1994 finanziert.
Am 1. Oktober 1997 wurde die Verwaltung des WHI an das NHLBI übertragen, wo es als Konsortium unter der Leitung des NHLBI in Zusammenarbeit mit dem National Institute of Arthritis and Musculoskeletal and Skin Diseases (NIAMS), dem National Cancer Institute (NCI ) und dem National Institute on Aging (NIA).
DESIGN-NARRATIVE:
Wie im Ziel beschrieben, besteht die WHI aus drei Hauptkomponenten: einer randomisierten kontrollierten klinischen Studie, einer Beobachtungsstudie und einer Studie über Gemeinschaftsansätze zur Entwicklung gesundheitsfördernder Verhaltensweisen. Die Rekrutierung für das WHI begann im September 1993 und endete im Dezember 1998. Sechs klinische Zentren schlossen die Rekrutierung im Januar 1997 ab. Die restlichen 34 Zentren schlossen die Rekrutierung im Dezember 1998 ab.
KLINISCHE STUDIENKOMPONENTE
Die klinische Studienkomponente besteht aus drei Teilstudien: der Hormonersatzstudie, der Ernährungsumstellungsstudie und der Kalzium-/Vitamin-D-Ergänzungsstudie. Etwa 27.500 Frauen im Alter von 50 bis 79 Jahren nehmen an der HRT teil, die testet, ob eine Langzeit-HRT koronare Herzkrankheiten und Frakturen reduziert, ohne das Brustkrebsrisiko zu erhöhen. Frauen mit einer Gebärmutter wurden randomisiert und erhielten entweder Östrogen plus Progestin oder ein Placebo. Progestin wurde hinzugefügt, um Frauen mit einer Gebärmutter vor Gebärmutterschleimhautkrebs zu schützen. Frauen, die eine Hysterektomie hatten, wurden randomisiert und erhielten entweder Östrogen allein oder ein Placebo. Die Östrogen-plus-Gestagen-Studie wurde am 8. Juli 2002 nach einer durchschnittlichen Nachbeobachtungszeit von 5,2 Jahren auf Empfehlung des Data and Safety Monitoring Board vorzeitig abgebrochen. Die Östrogen-Monotherapie-Studie wurde unverändert bis zum 2. März 2004 fortgesetzt, als das NIH die Teilnehmer anwies, die Einnahme ihrer Studienpillen einzustellen und mit der Nachbeobachtungsphase der Studie zu beginnen. . Die Teilnehmer an der Östrogen-Mono-Studie werden acht bis zwölf Jahre lang beobachtet und alle sechs Monate in die Klinik geführt, um die Sicherheit zu gewährleisten und ihre Gesundheit zu beurteilen.
Die Studienkomponente zur Ernährungsumstellung untersucht die Wirkung einer fettarmen Ernährung mit viel Obst, Gemüse und Getreide auf Brustkrebs, Darmkrebs und Herzerkrankungen bei 48.000 postmenopausalen Frauen. Die Teilnehmer werden randomisiert einer Vergleichsgruppe, die ihre gewohnten Ernährungsgewohnheiten beibehält, oder einer Ernährungsumstellungsgruppe zugeteilt. Frauen in der Gruppe mit Ernährungsumstellung reduzieren ihre Fettaufnahme auf 20 Prozent der gesamten täglichen Kalorien, erhöhen den Obst- und Gemüsekonsum auf fünf oder mehr Portionen pro Tag und erhöhen den Getreidekonsum auf sechs oder mehr Portionen pro Tag. Darüber hinaus überwachen sie ihre Nahrungsaufnahme und nehmen an Ernährungsgruppentreffen teil, um mehr über die Umstellung ihrer Ernährung im ersten Jahr zu erfahren. Danach nehmen sie an vier Sitzungen pro Jahr teil.
Die subtriale Kalzium-/Vitamin-D-Ergänzung testet, ob Kalzium- und Vitamin-D-Ergänzungen das Risiko von Hüft- und anderen Frakturen und Darmkrebs bei postmenopausalen Frauen verringern. Frauen in der Hormonersatztherapie und den Studien zur Ernährungsumstellung werden ermutigt, an der Calcium-/Vitamin-D-Ergänzungsstudie teilzunehmen. Ungefähr 45.000 postmenopausale Frauen werden randomisiert einer von zwei Studiengruppen zugeteilt. Einer Gruppe wurde nach dem Zufallsprinzip täglich 1.000 mg elementares Calcium (als Calciumcarbonat) und 400 Internationale Einheiten Vitamin D₃ verabreicht. Die zweite Gruppe erhielt ein passendes Placebo.
Frauen, die bereits Kalziumpräparate einnehmen, können diese weiterhin einnehmen. Die Teilnehmer werden acht bis elf Jahre lang beobachtet und alle sechs Monate von ihrem klinischen Zentrum kontaktiert, um ihre Sicherheit zu gewährleisten und ihre Gesundheit zu beurteilen.
Die Gesamtzahl der Studienteilnehmer in allen drei Teilstudien beträgt 68.135.
BEOBACHTUNGSSTUDIE
Zu den verschiedenen Zielen der Studie gehören: zuverlässige Schätzungen des Ausmaßes zu geben, in dem bekannte Risikofaktoren Herzkrankheiten, Krebs und Knochenbrüche vorhersagen; neue Risikofaktoren für diese und andere Krankheiten bei Frauen zu identifizieren; Risikofaktoren, Krankheitsvorliegen zu Studienbeginn und Neuerkrankungen während der WHI in allen Studienkomponenten zu vergleichen; und die Schaffung einer zukünftigen Ressource zur Identifizierung biologischer Krankheitsindikatoren, insbesondere von im Blut gefundenen Substanzen und Faktoren. Die Studie umfasste 93.726 postmenopausale Frauen und wird sie durchschnittlich neun Jahre lang verfolgen. Die Teilnehmer füllen regelmäßige Gesundheitsformulare aus und besuchen die Klinik drei Jahre nach der Einschreibung. Sie nehmen keine Medikamente und ändern ihre Gesundheitsgewohnheiten nicht.
GEMEINSCHAFTLICHE PRÄVENTIONSSTUDIE
Die kommunale Präventionsstudie besteht aus 12 separaten Studien, die an acht der universitären Präventionsforschungszentren der Centers for Disease Control and Prevention (CDC) im Rahmen einer Kooperationsvereinbarung zwischen NIH und CDC durchgeführt wurden. Die 12 Studien begannen im Oktober 1995 und dauern weitere fünf Jahre. Die Zusammenarbeit unterstützt Forschungs- und Demonstrationsprojekte zur Gesundheitsförderung und Krankheitsprävention, die gemeinschaftsbasiert sind und sich auf gesunde Verhaltensweisen konzentrieren, die die Hauptursachen für Tod und Behinderung verhindern, und die Gesundheitspraktiken fördern, die zu wirksameren Interventionen im Bereich der öffentlichen Gesundheit führen. Jedes Projekt bietet die Verbreitung von Forschungsergebnissen und die Übersetzung von Erkenntnissen in Gemeinschaftsinterventionen. Zu den untersuchten Themen gehören: Einstellungen zu Hysterektomie, Ovarektomie und chirurgischer Menopause bei Afroamerikanern; Verringerung des Risikos von Herz-Kreislauf-Erkrankungen bei schwarzen Frauen; Umwelt- und politische Interventionen zur Steigerung der körperlichen Aktivität bei Frauen aus Minderheiten im Alter von 40 bis 75; Peer-Support-Intervention für das Risiko von Herz-Kreislauf-Erkrankungen bei afroamerikanischen Frauen ab 40 Jahren; Bewertung der Wirksamkeit einer kurzen Aufklärungsintervention eines medizinischen Anbieters für Osteoporose bei Frauen aus Minderheiten im Alter von 40 Jahren und älter; Verbesserung der Bereitstellung von Diabetesversorgung für Frauen in Minderheitengruppen; und Beurteilung mäßiger körperlicher Aktivität bei Frauen.
