FONDO:
Antes de 1991, poca investigación se había centrado en problemas de salud exclusivos o más comunes para las mujeres. Este fue especialmente el caso de los estudios de enfermedades crónicas y su prevención en mujeres maduras. Estas condiciones (enfermedad coronaria, cáncer y osteoporosis) son las principales causas de deterioro de la calidad de vida, morbilidad y mortalidad en las mujeres posmenopáusicas de los Estados Unidos. El WHI, por mandato del Congreso, fue establecido en 1991 por los Institutos Nacionales de Salud y ubicado en la Oficina del Director (OD). El Centro de Coordinación Clínica para el ensayo clínico/estudio de observación se financió en septiembre de 1992 y los 16 Centros Clínicos Vanguard se financiaron en marzo de 1993. El protocolo inicial fue desarrollado conjuntamente por el Centro de Coordinación Clínica y la Oficina del Programa y fue revisado y aprobado por el Comité de Investigadores el 20 de abril de 1993. En 1994 se financiaron centros clínicos adicionales.
El 1 de octubre de 1997, la administración del WHI se transfirió al NHLBI, donde se lleva a cabo como un esfuerzo de consorcio dirigido por el NHLBI en cooperación con el Instituto Nacional de Artritis y Enfermedades Musculoesqueléticas y de la Piel (NIAMS), el Instituto Nacional del Cáncer (NCI ), y el Instituto Nacional sobre el Envejecimiento (NIA).
NARRATIVA DE DISEÑO:
Como se ha descrito en el objetivo, la WHI tiene tres componentes principales: un ensayo clínico controlado aleatorizado, un estudio observacional y un estudio de enfoques comunitarios para desarrollar comportamientos saludables. El reclutamiento para WHI comenzó en septiembre de 1993 y finalizó en diciembre de 1998. Seis centros clínicos completaron el reclutamiento en enero de 1997. Los 34 centros restantes completaron el reclutamiento en diciembre de 1998.
COMPONENTE DE ENSAYO CLÍNICO
El componente del ensayo clínico consta de tres subensayos: el ensayo de reemplazo hormonal, el ensayo de modificación de la dieta y el ensayo de suplementos de calcio/vitamina D. Aproximadamente 27.500 mujeres de 50 a 79 años participan en la TRH, que evalúa si la TRH a largo plazo reduce la enfermedad coronaria y las fracturas sin aumentar el riesgo de cáncer de mama. Las mujeres con útero fueron aleatorizadas para recibir estrógeno más progestina o un placebo. Se añadió progestina para proteger a las mujeres con útero del cáncer de endometrio. Las mujeres que se habían sometido a una histerectomía se asignaron al azar para recibir estrógeno solo o un placebo. El ensayo de estrógeno más progestina se detuvo temprano el 8 de julio de 2002 después de un seguimiento promedio de 5,2 años por recomendación de la Junta de Monitoreo de Datos y Seguridad. El estudio de estrógeno solo continuó sin cambios hasta el 2 de marzo de 2004, cuando los NIH dieron instrucciones a los participantes para que dejaran de tomar las píldoras del estudio y comenzaran la fase de seguimiento del estudio. . Los participantes en el estudio de estrógeno solo serán seguidos durante ocho a 12 años y tendrán visitas clínicas cada seis meses para garantizar la seguridad y evaluar su salud.
El componente de ensayo de modificación dietética estudia el efecto de una dieta baja en grasas y rica en frutas, verduras y granos sobre el cáncer de mama, el cáncer colorrectal y las enfermedades cardíacas en 48 000 mujeres posmenopáusicas. Los participantes se asignan al azar a un grupo de comparación que mantiene los hábitos dietéticos habituales oa un grupo de cambio dietético. Las mujeres en el grupo de cambio de dieta disminuyeron su ingesta de grasas al 20 por ciento del total de calorías diarias, aumentaron el consumo de frutas y verduras a cinco o más porciones por día y aumentaron los granos a seis o más porciones por día. Además, controlan su ingesta de alimentos y asisten a reuniones de grupos de nutrición para aprender más sobre cómo cambiar sus dietas durante el primer año. A partir de entonces asisten a cuatro reuniones por año.
El subensayo de suplementos de calcio/vitamina D evalúa si los suplementos de calcio y vitamina D reducen el riesgo de fracturas de cadera y de otro tipo y cáncer colorrectal en mujeres posmenopáusicas. Se alienta a las mujeres en los ensayos de terapia de reemplazo hormonal y modificación de la dieta a participar en el estudio de suplementos de calcio/vitamina D. Aproximadamente 45 000 mujeres posmenopáusicas se aleatorizan en uno de dos grupos de estudio. Un grupo fue asignado al azar para recibir 1000 mg de calcio elemental (como carbonato de calcio) y 400 Unidades Internacionales de vitamina D₃ diariamente. El segundo grupo recibió un placebo equivalente.
Las mujeres que ya toman suplementos de calcio pueden continuar tomándolos. Los participantes serán seguidos durante ocho a 11 años y su centro clínico se comunicará con ellos cada seis meses para garantizar su seguridad y evaluar su salud.
El número total de participantes del ensayo en los tres subensayos es 68 135.
ESTUDIO OBSERVACIONAL
Los diversos objetivos del estudio incluyen: brindar estimaciones confiables de la medida en que los factores de riesgo conocidos predicen enfermedades cardíacas, cánceres y fracturas; identificar nuevos factores de riesgo para estas y otras enfermedades en las mujeres; comparar factores de riesgo, presencia de enfermedad al inicio del estudio y nuevas ocurrencias de enfermedad durante el WHI en todos los componentes del estudio; y crear un recurso futuro para identificar indicadores biológicos de enfermedades, especialmente sustancias y factores que se encuentran en la sangre. El estudio inscribió a 93.726 mujeres posmenopáusicas y las seguirá durante un promedio de nueve años. Los participantes completan formularios de salud periódicos y visitan la clínica tres años después de la inscripción. No toman medicación y no modifican sus hábitos de salud.
ESTUDIO DE PREVENCIÓN COMUNITARIA
El estudio de prevención comunitaria consta de 12 estudios separados realizados en ocho de los Centros de Investigación de Prevención de la Universidad de los Centros para el Control y la Prevención de Enfermedades (CDC) a través de un acuerdo de cooperación entre los NIH y los CDC. Los 12 estudios comenzaron en octubre de 1995 y continúan durante cinco años más. La colaboración apoya proyectos de investigación y demostración de promoción de la salud y prevención de enfermedades que se basan en la comunidad y se centran en comportamientos saludables que previenen las principales causas de muerte y discapacidad y que promueven prácticas de salud que conducen a intervenciones de salud pública más efectivas. Cada proyecto proporciona la difusión de la investigación y la traducción de los hallazgos en intervenciones comunitarias. Los temas bajo estudio incluyen: actitudes hacia la histerectomía, la ovariectomía y la menopausia quirúrgica entre los afroamericanos; reducir el riesgo de enfermedades cardiovasculares entre las mujeres negras; intervenciones ambientales y políticas para aumentar la actividad física entre mujeres pertenecientes a minorías de 40 a 75 años; intervención de apoyo entre pares para el riesgo de enfermedades cardiovasculares entre mujeres afroamericanas de 40 años o más; evaluar la eficacia de una breve intervención educativa de un proveedor médico para la osteoporosis en mujeres pertenecientes a minorías de 40 años o más; mejorar la prestación de atención diabética a mujeres en grupos minoritarios; y evaluación de la actividad física moderada entre las mujeres.
