Los avances en genética molecular, epidemiología genética, genética de poblaciones e identificación de nuevos factores de riesgo y grupos de factores de riesgo de enfermedades cardiovasculares hicieron de 1991 un momento oportuno para aprovechar la amplia información sobre enfermedades cardiovasculares, aterosclerosis preclínica y factores de riesgo en personas que habían sido examinadas en estudios epidemiológicos de última generación en curso respaldados por el NHLBI. Mediante el reclutamiento de familiares de primer grado de muestras aleatorias de tales poblaciones definidas, FHS obtuvo información sobre la agregación familiar, las contribuciones genéticas y ambientales a la variación en las variables continuas, y la frecuencia y distribución de niveles elevados de factores de riesgo y de genes principales seleccionados en la población general. .
El FHS fue iniciado por el personal y aprobado por el Comité Asesor de Prevención y Aplicaciones Clínicas en mayo de 1990. Las solicitudes de propuestas se publicaron en julio de 1991. Los contratos se adjudicaron en junio de 1992.
NARRATIVA DE DISEÑO:
Durante las Fases I y II de mayo de 1992 a mayo de 1996, se reclutaron probandos, de 45 a 69 años de edad, del Framingham Heart Study, el Utah Family Tree Study y los sitios de Carolina del Norte y Minnesota del ARIC Study, junto con sus familiares, para participación en el Estudio Familiar del Corazón. Se seleccionaron dos grupos de probandos, ya sea al azar o por un alto riesgo familiar de cardiopatía coronaria calculado a partir de los datos del estudio principal. Se recogieron datos adicionales sobre la estructura familiar y el historial de enfermedades de 3.150 probandos y 22.909 de sus familiares. El examen clínico y el seguimiento de estas familias aleatorias y de alto riesgo de cardiopatía coronaria se realizaron en un total de 1253 familias, incluidas 5975 personas, de las cuales 102 familias, incluidas 265 personas, eran afroamericanas. Los exámenes incluyeron información sobre antropometría, presión arterial, ECG, ecografía carotídea, función pulmonar y bioquímica sanguínea. Los datos del cuestionario incluyeron antecedentes médicos y reproductivos, dieta, actividad física, consumo de tabaco y alcohol, educación, ingresos y factores psicosociales, incluida la hostilidad, el apoyo social y el estrés. Las Fases I y II incluyeron cuatro centros de campo, un centro de coordinación y un laboratorio de sangre central.
En agosto de 1996, comenzó la Fase III cuando se otorgaron acuerdos de cooperación a un consorcio de siete subvenciones iniciadas por investigadores para realizar estudios de genética molecular y epidemiología genética utilizando los datos recopilados durante las Fases I y II. La Fase III finalizó en julio de 2001. El objetivo de la Fase III fue realizar estudios de genética molecular y epidemiología genética utilizando los extensos datos sobre antecedentes familiares y médicos, factores de riesgo, estilo de vida, muestras de sangre y ADN de bancos recolectados previamente por la ESF. Los estudios incluyeron genética molecular novedosa de genes candidatos y búsquedas en todo el genoma con marcadores anónimos para la detección, mapeo y caracterización de genes de enfermedades coronarias y aterosclerosis. Se realizaron análisis de epidemiología genética que aportaron nueva información sobre los aspectos familiares de la aterosclerosis y fenotipos intermedios en afroamericanos. La Fase III también incluyó cuatro centros de campo, un laboratorio central, un laboratorio de genética molecular y un centro de coordinación.
El estudio se renovó en 2001 como Family Heart Study - Subclinical Atherosclerosis Network (FHS-SCAN) para completar los análisis de datos de escaneo de todo el genoma y para genotipar marcadores prometedores. El estudio espera inscribir 401 pedigríes informativos (3027 individuos) previamente examinados y genotipados por el Estudio del Corazón Familiar del NHLBI para cuantificar el volumen de calcio aórtico y coronario con el fin de identificar genes asociados con la aterosclerosis.
