Durante los últimos cuarenta años, los estudios epidemiológicos han hecho contribuciones fundamentales al conocimiento de la etiología y la historia natural de las enfermedades crónicas: enfermedades cardiovasculares, diabetes y cánceres. Los hallazgos de los estudios epidemiológicos han proporcionado fundamentos prácticos para el desarrollo de programas de prevención y control para mejorar los estilos de vida y para controlar la presión arterial alta con medicamentos, cuando estén indicados. Estos desarrollos y su impacto en los estilos de vida y los factores de riesgo probablemente contribuyeron a la marcada disminución de las tasas de mortalidad por enfermedad coronaria, accidente cerebrovascular, todas las enfermedades cardiovasculares y todas las causas a lo largo de los años setenta y ochenta.
Los cuatro estudios epidemiológicos tienen como base las principales contribuciones de los estudios epidemiológicos de 'primera generación' anteriores, incluidos el Estudio de funcionarios públicos de Albany, el Estudio de Charleston, el Estudio del corazón de Framingham, el Estudio de Tecumseh y otros. La importancia de estos estudios radica en su potencial para dilucidar cuestiones clave de 'segunda generación' en la epidemiología de las principales enfermedades crónicas cardiovasculares y no cardiovasculares.
NARRATIVA DE DISEÑO:
Proyecto de detección de la Asociación del Corazón de Chicago en la industria: Los empleados de 85 empresas en el área metropolitana de Chicago fueron examinados entre el otoño de 1967 y principios de 1973. La mano de obra total de estas empresas superaba las 75.000 personas. La tasa de voluntarios para la proyección fue del 55 por ciento. Se animó a todos los empleados a participar independientemente del tipo de trabajo o turno trabajado. En cada sitio de trabajo se recogieron datos sobre hábito tabáquico, diagnóstico médico previo, tratamientos actuales para hipertensión, cardiopatías y diabetes, altura y peso, edad, sexo, raza, frecuencia cardiaca.
Peoples Gas Company: Todos los datos sobre las dos cohortes fueron recopilados como parte del programa de exámenes médicos periódicos para todos los empleados en el Departamento Médico de la Compañía por su personal de médicos de tiempo completo. Se recopilaron datos sobre edad, raza, altura, peso, presión arterial, colesterol sérico, antecedentes de tabaquismo, frecuencia cardíaca, ECG en reposo, antecedentes médicos y familiares, consumo de alcohol y café y té, educación, servicio militar, ocupación, actividad física, dieta y niveles de glucosa y ácido úrico en suero. Se programaron exámenes repetidos anualmente para hombres de 50 a 59 años, cada dos años para hombres de 40 a 49 años y cada cuatro años para hombres de 25 a 39 años.
Western Electric Company: este estudio longitudinal de la cardiopatía coronaria se inició en 1957 en Hawthorne Works de Western Electric Company en Chicago. Los sujetos masculinos estaban libres de enfermedad coronaria al inicio del estudio. El historial médico y el examen físico se modelaron según el Framingham Heart Study. Se recopilaron datos sobre dieta, actividad física, electrocardiograma, niveles de hemoglobina y colesterol en sangre, grasa corporal, presión arterial, altura, peso, lipoproteína lipasa, nivel de glucosa y tabaquismo.
El estudio se renovó en el año fiscal 1992 y nuevamente en el año fiscal 1998. La renovación de 1998 utilizó datos y análisis de las cuatro cohortes. Se centró en seis objetivos específicos: factores que influyen en el riesgo de enfermedad coronaria-cardiovascular a largo plazo en mujeres y hombres adultos jóvenes (de 18 a 39 años al inicio del estudio); factores que influyen en el riesgo de enfermedad coronaria-cardiovascular a largo plazo en mujeres y hombres afroamericanos; factores que influyen en el riesgo de cardiopatía coronaria-enfermedad cardiovascular durante el seguimiento anterior y posterior (primeros 15 años y más allá de los 15 años de seguimiento); impacto del estado inicial de bajo riesgo en el riesgo a largo plazo de mortalidad por enfermedad coronaria, enfermedad cardiovascular, enfermedad no cardiovascular y todas las causas; relación de la ingesta habitual de múltiples grupos de alimentos con el cambio de 10 años en el colesterol sérico, la presión arterial, el peso y con los riesgos de 30 años de muerte por enfermedad coronaria, enfermedad cardiovascular, cáncer y todas las causas; las diferencias en el impacto de los factores de riesgo entre los grupos según el sexo, la raza, la edad o el estado de los factores de riesgo.
El estudio se extendió hasta julio de 2005 y utiliza datos y análisis de la cohorte del Proyecto de Detección en la Industria de la Asociación del Corazón de Chicago.
