FONDO:
El Programa de Salud del Corazón de Pawtucket surgió de la preocupación de que las necesidades de la sociedad no podían satisfacerse mediante el tratamiento paliativo de la enfermedad coronaria. En 1977, se inició un programa diseñado para facilitar la rehabilitación y, con suerte, proporcionar posibles medidas preventivas secundarias para pacientes con enfermedad coronaria sintomática. Simultáneamente, creció la convicción de que el interés nacional se beneficiaría de una investigación cuidadosa sobre si la enfermedad coronaria era o no prevenible mediante la modificación de aquellos aspectos del comportamiento individual y grupal que influían en los principales factores de riesgo cardiovascular en las poblaciones de vida libre.
El Programa de Salud del Corazón de Pawtucket fue diseñado para fomentar la apropiación comunitaria y la participación activa en un proceso de cambio cultural. Se anticipó una reducción del factor de riesgo de aterosclerosis en toda la población como resultado de la aplicación de una variable independiente basada en la teoría del aprendizaje social. El énfasis en los factores individuales, en los factores ambientales físicos y en las influencias socioculturales en el comportamiento se diseñó para producir un cambio persistente en los factores de riesgo seguido de una reducción de la tasa de morbilidad y mortalidad para la población de la ciudad.
NARRATIVA DE DISEÑO:
La selección de una comunidad de intervención y una comunidad de control se llevó a cabo al principio del diseño del Programa de salud cardíaca de Pawtucket. Los datos del censo, actualizados hasta 1975, se usaron para identificar dos comunidades con entre 40,000 y 100,000 personas y con estabilidad de inmigración y emigración necesaria para un seguimiento a largo plazo. Las dos comunidades fueron emparejadas cuidadosamente por variables sociodemográficas.
Ambas comunidades se sometieron a encuestas de muestra aleatoria de referencia que demostraron niveles similares de factores de riesgo cardiovascular en las poblaciones de cada ciudad. La intervención comunitaria efectiva comenzó en 1983. La intervención total fue de 7,5 años. Los objetivos específicos de la intervención incluyeron una reducción del seis por ciento en el colesterol total, una reducción de 6 mm Hg en la presión arterial sistólica, una reducción relativa del 30 por ciento en fumadores activos, una reducción del dos por ciento en el peso corporal y el índice de masa corporal, una reducción de 2 ml/kg /minuto de aumento en el consumo máximo de oxígeno estimado y una reducción del 15 por ciento en las tasas de eventos de enfermedades cardiovasculares fatales y no fatales. Las técnicas educativas utilizadas por el programa incluyeron: mensajes impresos, radiales y televisados; programas de cambio de comportamiento en grupos pequeños impartidos por voluntarios laicos capacitados; reducción de la presión arterial en la comunidad y en el lugar de trabajo, detección de colesterol y factores de riesgo múltiples, eventos de asesoramiento y derivación; programas de autoayuda; currículo escolar; programas de prevención del tabaquismo; concursos de cambio de comportamiento de riesgo; etiquetado de estantes en tiendas de abarrotes y etiquetado de menús en restaurantes para indicar alimentos bajos en sodio y bajos en grasa.
La eficacia del programa se evaluó mediante encuestas bienales aleatorias sobre los factores de riesgo en los hogares, un sistema de vigilancia de la morbilidad y la mortalidad y otros métodos. En la encuesta de factores de riesgo, los hogares fueron seleccionados al azar. Dentro de cada hogar muestreado, se seleccionó un solo encuestado entre adultos elegibles. Se administró una entrevista familiar y un protocolo de prueba en el hogar que incluye preguntas sobre dieta, ejercicio, tabaquismo, comportamiento y conocimiento de enfermedades cardiovasculares. Las medidas fisiológicas incluyeron altura, peso, presión arterial, colesterol total, triglicéridos, colesterol unido a lipoproteínas de alta densidad y cotinina sérica. A una submuestra se le realizó una prueba escalonada para estimar el consumo máximo de oxígeno como medida de aptitud. Un segundo subconjunto completó un cuestionario de dieta Willett. Se realizaron cinco encuestas transversales de hogares de aproximadamente 2.800 personas por encuesta. La encuesta transversal inicial se convirtió en una encuesta de cohortes para la medición nuevamente en 1986-1987 y en 1988-1989. La tercera encuesta transversal también se convirtió en una encuesta de cohortes para la medición simultánea con las muestras transversales de 1987-1988 y 1989-1990.
En ambas comunidades se examinaron las tasas de morbilidad y letalidad por enfermedad coronaria y accidente cerebrovascular. Los datos de morbilidad y mortalidad se obtuvieron de siete hospitales del área y de los Departamentos de Salud Estatales de Rhode Island y Massachusetts. Los criterios de resultado fueron desarrollados en colaboración por el Programa de Salud del Corazón de Pawtucket, el Estudio de Reducción de Riesgo Multifactorial de Cinco Ciudades de Stanford y el Programa de Salud del Corazón de Minnesota, para maximizar el valor científico de las conclusiones extraídas de los tres estudios y permitir la combinación de los datos finales. La vigilancia se completó entre 1980 y 1983 y continuó hasta 1993.
