FONDO:
El Estudio de salud de las enfermeras comenzó en 1976, cuando 121.700 enfermeras registradas que vivían en once estados completaron un cuestionario enviado por correo que incluía elementos sobre su historial médico, dieta y otros factores de riesgo de cáncer. El Instituto Nacional del Cáncer financió los primeros tres años del estudio y los años posteriores con datos relacionados con el cáncer. En 1980, el Instituto Nacional del Corazón, los Pulmones y la Sangre comenzó a financiar la parte cardiovascular del estudio. Posteriormente, se administraron cuestionarios de seguimiento cada dos años. Se recolectaron muestras de sangre en una submuestra de mujeres entre 1989 y 1990 y se almacenaron para el análisis del ADN de la línea germinal y los biomarcadores circulantes.
El componente cardiovascular actualmente está financiado hasta marzo de 2022.
NARRATIVA DE DISEÑO:
En 1976, 121.700 enfermeras de 30 a 55 años completaron y devolvieron el cuestionario inicial del Estudio de Salud de las Enfermeras y forman parte de la cohorte del Estudio de Salud de las Enfermeras. La cohorte ha sido seguida por medio de cuestionarios bienales enviados por correo. Un total de 109.413 participantes respondieron al cuestionario de 1978. Además, 400 mujeres que respondieron en 1976 declararon que no deseaban continuar en el estudio y 390 mujeres fallecieron. En 1980, se agregó al cuestionario la primera evaluación dietética importante. En el intervalo 1978-1980, hubo 624 muertes, 300 mujeres rechazaron participar más, aproximadamente 99 000 completaron el cuestionario completo, incluido el cuestionario dietético, y otras 4000 completaron un cuestionario abreviado, para un total de alrededor de 103 000 respuestas. En 1982, la respuesta general fue de 101.174. Los aproximadamente 19,000 que no respondieron vivos fueron seguidos por teléfono, lo que arrojó un 71 por ciento de los que no respondieron. Así, a finales de 1982, se disponía de información actual sobre 116.150 o el 95,4 por ciento de la cohorte original. Un subconjunto de 32.826 mujeres proporcionó muestras de sangre entre 1989 y 1990. El genotipado en 1186 mujeres (382 con enfermedad coronaria y 804 controles emparejados) se realizó utilizando la plataforma Affymetrix 6.0.
Las muertes en la cohorte generalmente fueron informadas por los familiares o las autoridades postales. Al finalizar cada ciclo de envío, se buscó en el Índice Nacional de Defunciones los nombres de los que no respondieron y que podrían haber muerto. Al comparar las muertes determinadas por fuentes independientes, el estudio determinó aproximadamente el 98 por ciento de las muertes. Se obtuvieron certificados de defunción de los departamentos estatales de estadísticas vitales para confirmar todas las muertes informadas. Para todos los certificados de defunción que indicaban posible enfermedad cardiovascular, se solicitó permiso a los familiares para obtener más información. La muerte por enfermedad coronaria confirmada requería información adicional más allá del certificado de defunción, como informes de autopsia, ECG y cambios enzimáticos de infarto de miocardio antes de la muerte, dolor torácico clásico inmediatamente antes de la muerte, infarto de miocardio o angina documentados previamente, o cateterismo cardíaco previo que muestre enfermedad coronaria grave. enfermedad.
Se recopilaron datos sobre la fecha de nacimiento, el peso, el tabaquismo, el estado menopáusico y el uso provisional actual de la terapia hormonal menopáusica. También se recopilaron datos sobre casos incidentes de infarto de miocardio no mortal, accidente cerebrovascular, embolia pulmonar y angina de pecho. Estos ítems aparecían en cada cuestionario. Además, los parámetros nutricionales se evaluaron mediante un cuestionario de frecuencia alimentaria semicuantitativo autoadministrado en 1986 y nuevamente en 1990.
El estudio se renovó en 1993 prestando especial atención a los antioxidantes dietéticos, otros factores nutricionales, la actividad física, la distribución regional de grasa, la terapia menopáusica con estrógenos y progestágenos y los marcadores bioquímicos, incluidos los lípidos plasmáticos y las apoproteínas. El seguimiento de los eventos de enfermedades cardiovasculares no fatales es superior al 92 por ciento para los 121 700 participantes originales. Los eventos cardiovasculares fatales se documentan mediante certificados de defunción y se confirman y clasifican mediante la revisión de registros hospitalarios, informes de autopsias y entrevistas con familiares. Las búsquedas del Índice Nacional de Muertes para todos los que no respondieron aseguran la identificación de las muertes restantes, lo que da como resultado un seguimiento de la mortalidad que está completo en más del 98 por ciento.
El estudio se renovó en el año fiscal 2002 para continuar el seguimiento hasta marzo de 2007. Un objetivo principal de esa renovación fue comparar la capacidad predictiva de varios marcadores bioquímicos y genéticos de inflamación y activación endotelial para la enfermedad coronaria frente al accidente cerebrovascular en mujeres: proteína C reactiva (PCR), E-selectina, molécula de adhesión intercelular-1, endotelina-1 y polimorfismos de los genes CRP y E-selectina. El estudio también continúa la investigación de los determinantes del estilo de vida de las enfermedades cardiovasculares, incluida la terapia hormonal para la menopausia (dosis, formulación y duración del uso) y el consumo de alcohol (dosis y tipo de bebida), en la cohorte completa, y las interacciones de estas exposiciones con las anteriores. biomarcadores y con nuevos marcadores genéticos (polimorfismos del gen de la protrombina y la alcohol deshidrogenasa-3). Los genotipos de 1186 mujeres tipificados como parte de un estudio de asociación del genoma completo de la enfermedad coronaria se utilizaron para estudiar los factores genéticos asociados con la enfermedad cardiovascular y la interacción de los genes y la dieta en el riesgo cardiovascular. Los ciclos de renovación posteriores incluyeron el estudio de las adipoquinas plasmáticas y los predictores metabolómicos, incluidos los metabolitos de la flora intestinal, como el N-óxido de trimetilamina, la colina y la L-carnitina, de la enfermedad coronaria. El ciclo de financiación actual implica la elaboración de perfiles metabolómicos completos de casos y controles de enfermedades coronarias y el desarrollo de puntuaciones de riesgo metabolómico para enfermedades coronarias, con financiación hasta marzo de 2022.
