ARRIÈRE-PLAN:
L'ARVD est un trouble rare mais est considéré comme une cause majeure de mort subite et d'arythmie potentiellement mortelle, en particulier chez les jeunes. La prévalence des ARVD est inconnue mais est certainement sous-estimée en raison des difficultés à obtenir un diagnostic correct. Elle semble particulièrement fréquente dans certaines zones géographiques, probablement pour un effet fondateur, comme dans le nord-est de l'Italie, où un grand nombre de cas et de familles d'ARVD ont été décrits. Une étude non contrôlée de l'Université de Padoue a rapporté une fréquence de formes familiales d'environ 30 %, indiquant l'existence d'un gène défectueux dans une grande proportion des cas. Aux États-Unis, la fréquence de la maladie est inconnue, mais le nombre de cas semble augmenter.
L'étiologie de l'ARVD était inconnue jusqu'à très récemment. L'hypothèse principale impliquait des mécanismes apoptotiques et, dans certains cas, une infection virale. Cependant, au cours des deux dernières années, deux gènes responsables de l'ARVD ont été identifiés. Le premier code pour la plakoglobine, une protéine des jonctions cardiaques aux fonctions adhésives et de signalisation. Le deuxième gène ARVD est le récepteur cardiaque de la ryanodine (RYR2), qui n'a été caractérisé que très récemment par le groupe du Dr Danieli. En fait, cette découverte est si récente que dans cette étude, RYR2 est toujours considéré comme un candidat potentiel. La découverte des premiers gènes de la maladie fournit la base d'une approche de gène candidat suivant l'hypothèse d'une "voie finale commune". Ainsi, les candidats majeurs deviennent des gènes impliqués dans l'adhésion cellule-cellule et codant pour les canaux ioniques.
NARRATIF DE CONCEPTION :
Il s'agit d'une étude multidisciplinaire, multicentrique et collaborative portant sur les aspects cardiaques, cliniques et génétiques de l'ARVD. Les objectifs spécifiques sont (1) d'établir un registre ARVD nord-américain inscrivant des patients ARVD et les membres de leur famille, sur la base de critères de test de diagnostic standardisés, dans une étude de suivi longitudinale prospective ; (2) déterminer le fond génétique de l'ARVD en identifiant les loci chromosomiques et les mutations génétiques spécifiques associées à ce trouble ; (3) déterminer l'influence du génotype sur l'évolution clinique des patients atteints d'ARVD et explorer les associations phénotype-génotype qui contribueront à améliorer le diagnostic, la stratification des risques et la thérapie ; et (4) développer des méthodes quantitatives pour évaluer la fonction ventriculaire droite afin d'améliorer la spécificité et la sensibilité du diagnostic ARVD.

Critère d'intégration:
- Hommes et femmes ayant dépassé l'âge de la puberté
- ARVD suspecté sur la base de la présence de critères majeurs ou mineurs du groupe de travail
Critère d'exclusion:
- Enfants de moins de 12 ans
- Défibrillateur automatique interne (DCI) en place depuis plus de 2 ans (pour les proposants)
- Individus atteints d'ectopie ventriculaire monomorphe de morphologie à prédominance RBBB
- Personnes présentant des anomalies cardiomyopathiques évidentes de structure ou de fonction affectant principalement le ventricule gauche
- Les personnes atteintes d'autres affections pouvant être confondues avec une dysplasie ventriculaire droite, telles qu'une cardiopathie congénitale, par exemple une communication interauriculaire, un drainage anormal des vaisseaux pulmonaires dans l'oreillette droite et la malformation d'Ebstein
- Les personnes qui refusent de subir des tests de diagnostic au centre d'inscription le plus proche
