Imaging molecolare in vivo (MRI) di lesioni aterotrombotiche
Lo scopo di questo studio è sviluppare e convalidare nuove tecniche di risonanza magnetica (MRI), contrasto dinamico potenziato (DCE)-MRI e tomografia a emissione di positroni (PET)/RM per il rilevamento e la stratificazione del rischio di pazienti con aterosclerosi.

L'aterosclerosi è responsabile della maggior parte delle disabilità e dei decessi nei paesi sviluppati.
Precedenti studi hanno dimostrato che gli eventi clinici improvvisi sono fortemente correlati alla composizione della placca e al grado di infiammazione della placca.
Questi risultati sottolineano l'importanza di sviluppare marcatori surrogati non invasivi dell'infiammazione della placca per rilevare le placche asintomatiche ad alto rischio in ambito clinico.
La risonanza magnetica (MRI) con contrasto dinamico potenziato (DCE) e la tomografia a emissione di positroni (PET) con fluorodeossiglucosio 18F (FDG) con tomografia computerizzata combinata (TC) hanno mostrato risultati promettenti nella caratterizzazione e nella quantificazione dell'attività metabolica (cioè glicolisi/infiammazione) nell'aterosclerosi, prendendo di mira la presenza di neovasi (DCE-MRI) e cellule infiammatorie come i macrofagi (18F-FDG PET) nelle placche di soggetti sia animali che umani.
Tuttavia, diverse sfide devono essere superate prima di tradurre questi approcci di imaging nella pratica clinica.
Un ostacolo significativo all'adattamento degli approcci DCE-MRI convenzionali all'aterosclerosi include la necessità di immagini con un'elevata risoluzione spaziale per catturare l'eterogeneità della placca.
Ciò può essere ottenuto con tempi di scansione più lunghi, ma è in conflitto con la necessità di un'elevata risoluzione temporale richiesta per un accurato campionamento della funzione di input arterioso e la quantificazione dell'assorbimento del mezzo di contrasto.
Nell'obiettivo 1, i ricercatori svilupperanno e convalideranno una nuova sequenza a doppia immagine per DCE-MRI dell'aterosclerosi in cui i ricercatori acquisiscono un'immagine AIF ad alta risoluzione temporale, ma bassa risoluzione spaziale e un'immagine della parete del vaso ad alta risoluzione spaziale/bassa risoluzione temporale per consentire un'accurata quantificazione dell'assorbimento del mezzo di contrasto all'interno delle placche.
Questo approccio verrà confrontato con gli approcci convenzionali sia in un modello di coniglio di aterosclerosi che in soggetti umani.
La risoluzione spaziale limitata degli scanner PET convenzionali ha un impatto sull'accuratezza della quantificazione PET 18F-FDG nelle placche aterosclerotiche a causa dell'effetto del volume parziale (PVE).
I metodi di correzione PVE a posteriori che utilizzano immagini anatomiche ad alta risoluzione acquisite con una diversa modalità di imaging possono migliorare la quantificazione, ma sono impegnativi poiché richiedono un'accurata co-registrazione tra l'altra modalità di imaging e la PET.
La RM è la scelta ideale per questa seconda modalità di imaging in quanto produce immagini anatomiche ad alta risoluzione senza l'uso di radiazioni ionizzanti.
Uno scanner RM/PET combinato può quindi essere più adatto per lo sviluppo di nuove metodologie di correzione PVE.
Come parte dell'obiettivo 2, i ricercatori svilupperanno e convalideranno l'approccio combinato di imaging MR-PET (FDG) per migliorare la quantificazione dell'attività metabolica della placca aterosclerotica.
La correzione dell'attenuazione basata su MR sarà confrontata con la correzione dell'attenuazione basata su CT.
Gli approcci per migliorare la correzione del PVE e il tempo di circolazione ottimale per l'imaging della placca saranno inoltre convalidati sia nei conigli che nell'uomo.
Infine, i parametri di imaging derivati dalla migliorata DCE-MRI e MR-PET (FDG) saranno convalidati in pazienti sottoposti a endarterectomia carotidea (CEA), con l'obiettivo primario di stabilire la relazione tra imaging e marcatori istologici dell'infiammazione della placca.
Inoltre, gli investigatori valuteranno la relazione (se presente) con i biomarcatori sierici e, come endpoint esplorativo, gli investigatori studieranno mediante PCR in tempo reale la relazione dell'imaging con l'espressione genica dei marcatori di vulnerabilità della placca.

Criterio di inclusione:
- Volontari con malattia dell'arteria carotidea che sono compatibili con MRI e CT.

- e/o storia familiare di malattia
Criteri di esclusione:
- Tasso di filtrazione glomerulare <30 mg/ml (per MRI con mezzo di contrasto)
- Soggetti che hanno impianti ferromagnetici (ad es. clip per aneurisma, ICD, pacemaker, ecc.) o una condizione che potrebbe essere controindicata per la procedura di risonanza magnetica (ad es. claustrofobia)
- Le pazienti in gravidanza saranno escluse dal presente studio.

DCE-MRI
Risonanza magnetica (MRI) con e senza agenti di contrasto approvati dalla FDA: la risonanza magnetica è una tecnica di imaging non invasiva utilizzata per visualizzare in dettaglio la struttura interna del corpo.
La macchina per la risonanza magnetica è un magnete sovradimensionato sempre acceso.
Verrà utilizzato in questo studio per fornire informazioni anatomiche e funzionali (MRI con contrasto) sulle placche aterosclerotiche.

PET/TAC e PET/RM
Tomografia a emissione di positroni (PET)/tomografia computerizzata (TC): la PET è una tecnica di imaging di medicina nucleare, che produce immagini di processi funzionali nel corpo.
Il sistema rileva coppie di raggi gamma emessi indirettamente da un radionuclide emettitore di positroni (tracciante), che viene introdotto nel corpo su una molecola biologicamente attiva.
Al giorno d'oggi l'imaging PET è più utile in combinazione con l'imaging anatomico, come gli scanner CT, quindi gli scanner PET sono ora disponibili con scanner CT multi-detettore integrati di fascia alta.
Poiché le due scansioni possono essere eseguite in sequenza immediata durante la stessa sessione e con il paziente che non cambia posizione tra le due scansioni, le aree di anomalia sulle immagini PET possono essere direttamente correlate con l'anatomia sulle immagini CT.

Per evitare la radiazione aggiuntiva derivante dalla scansione TC durante l'imaging PET/TC, oggi l'imaging PET può essere accoppiato con immagini anatomiche RM.
