Criterio di inclusione:
Pazienti i cui risultati clinici soddisfano 5 o più criteri dei seguenti 8
- Febbre ≥ 38,5 ℃ per ≥ 7 giorni
- Splenomegalia ≥ 3 FB sotto il margine sottocostale sinistro
- Citopenie che colpiscono ≥ 2 linee su 3 in PB Hb < 9 g/L Piastrine < 100 x 109 /L ANC < 1,0 x 109 /L
- Ipertrigliceridemia e/o ipofibrinogenemia (trigliceridi a digiuno ≥ 265 mg/dL, fibrinogeno ≤ 1,5 g/L)
- Emofagocitosi in BM o milza o LN
- Attività delle cellule NK bassa o assente (secondo il riferimento del laboratorio locale)
- Ferritina sierica ≥ 500 mcg/L
- CD25 solubile (recettore sIL-2) ≥ 2.400 U/ml
- 18 anni di età e oltre.
- Tutti i pazienti (o la sua famiglia quando il paziente non può firmare il modulo di consenso a causa delle sue condizioni generali) danno il consenso informato scritto secondo le linee guida del comitato dell'istituto per la ricerca umana.
Criteri di esclusione:
- HLH da tumore maligno (come linfoma, mieloma, leucemia e altri tumori solidi)
- HLH da disturbi reumatici (come LES, AOSD, sindrome da anticorpi antifosfolipidi)
- Pazienti con disturbo psichiatrico o deficienza mentale grave tale da rendere dissimile l'adesione al trattamento e rendere impossibile il consenso informato
- Donne che allattano, donne incinte, donne in età fertile che non desiderano una contraccezione adeguata
- Paziente di sesso maschile che rifiuta i metodi per evitare la gravidanza tramite metodi come l'astinenza, il metodo di barriera (preservativo, ecc.).
- Pazienti con una diagnosi di precedente tumore maligno a meno che non siano liberi da malattia da almeno 5 anni dopo la terapia con intento curativo (eccetto il cancro della pelle non melanoma trattato curativamente, il carcinoma in situ o la neoplasia intraepiteliale cervicale)
