FUNDO:
A trombose desempenha um papel importante nas fases tardias da oclusão coronária. A agregação plaquetária é um grande componente na formação de trombos arteriais. Em estudos farmacológicos, a aspirina demonstrou inibir a agregação plaquetária e, portanto, pode-se esperar que previna a oclusão coronariana. Esses efeitos são aparentes na faixa de dose de 100-1000 mg/dia e podem ser mais evidentes com 160 miligramas diários. Doses mais altas parecem não ser mais eficazes na inibição da concordância plaquetária ou no tempo de sangramento prolongado.
Embora um estudo inicial de caso-controle de Jick e Miettinen tenha mostrado um grande benefício, a maioria dos estudos observacionais mostrou um benefício cardiovascular de cerca de 20%. Dados conclusivos só poderiam resultar de um estudo randomizado com um grande tamanho de amostra.
NARRATIVA DO DESENHO:
Amostra fixa randomizada, duplo-cega. Os participantes foram randomizados em um dos quatro grupos de tratamento: um comprimido de 325 miligramas de aspirina em dias alternados com uma cápsula de 30 miligramas de beta-caroteno; uma aspirina em dias alternados com uma cápsula de placebo de beta-caroteno; um comprimido de aspirina placebo em dias alternados com uma cápsula de beta-caroteno; e um comprimido de placebo de aspirina em dias alternados com uma cápsula de placebo de beta-caroteno. Os principais endpoints para o componente cardiovascular do estudo foram mortalidade cardiovascular, mortalidade total e eventos coronários.
