2차 목표:

I. 병용 요법에 반응하는 환자의 연구 중 생검 샘플 간의 T-세포 침윤의 차이를 추정하기 위해(각 치료 부문에서 고형 종양의 반응 평가 기준[RECIST] 1.1에 따른 확인 및 미확정, 완전 및 부분 반응 포함) .

포함 기준:

  • 환자는 4기 또는 절제 불가능한 3기인 병리학적으로 확인된 흑색종이 있어야 합니다. 환자는 피부, 점막 또는 알려지지 않은 기원의 원발성을 가질 수 있습니다. 포도막(안구) 원발성 환자는 자격이 없습니다.
  • 환자는 RECIST 1.1에 따라 측정 가능한 질병이 있어야 합니다. 측정 가능한 모든 병변은 등록 전 28일 이내에 신체 검사, 컴퓨터 단층촬영(CT) 스캔 또는 자기공명영상(MRI)으로 평가해야 합니다. 측정 가능한 유일한 질병이 피부 또는 피하인 경우 병변은 최대 치수가 최소 10mm 이상이어야 하며 캘리퍼스 및 사진을 사용하여 연속적으로 기록할 수 있어야 합니다. 측정 불가능한 질병을 평가하는 데 사용되는 테스트는 등록 전 42일 이내에 수행되어야 합니다. 모든 질병은 기본 종양 평가 양식에 평가되고 문서화되어야 합니다.
  • 중추신경계(CNS) 전이가 있는 환자는 CNS 진행의 후속 증거 없이 정위 방사선 요법, 개두술, 감마나이프 요법 또는 전뇌 방사선 요법으로 모든 병변을 적절하게 치료해야 합니다. 환자는 등록 전 최소 14일 동안 스테로이드가 필요하지 않아야 합니다. 중추신경계 전이 병력이 있는 환자는 등록 전 42일 이내에 뇌 MRI를 받아야 합니다.

  • 가임 여성과 성적으로 활동적인 남성은 연구 기간 내내 그리고 프로토콜 치료 완료 후 7개월 동안 피임 사용에 동의해야 합니다. 일상적인 피임 방법 외에도 "효과적인 피임"에는 이성애 독신 생활과 임신을 방지하기 위한 수술(정관 절제술)도 포함됩니다. 이전에 독신이었던 환자가 프로토콜 치료 중 또는 완료 후 7개월 이내에 이성애를 활성화하기로 선택한 경우 효과적인 피임 조치를 시작할 책임이 있습니다.

  • Oncology Patient Enrollment Network(OPEN) 등록 프로세스의 일부로 치료 기관의 신원이 제공되어 이 연구에 대한 현재(365일 이내) 기관 검토 위원회 승인 날짜가 시스템에 입력되었는지 확인합니다.

제외 기준:

  • 환자는 치료 의사에 따라 진행되기 전에 항 PD-1 또는 항 PD-L1 제제에 대한 부분 또는 완전 반응을 달성하지 않아야 합니다.
  • 환자는 등록 전 21일 이내에 항 PD-1 또는 항 PD-L1 제제를 포함한 전신 요법을 받은 적이 없어야 합니다.
  • 환자는 등록 전 14일 이내에 사전 방사선 요법을 받은 적이 없어야 합니다.

  • 환자는 다음이 없어야 합니다.

    • 이필리무맙 또는 다른 CTLA-4 길항제를 사용한 사전 치료
    • 항 PD-1 또는 항 PD-L1 제제의 진행과 등록 사이의 전신 요법
    • 참고: 항 PD-1 또는 항 PD-L1 요법 전에 전신 요법(BRAF 표적 제제 포함)이 허용됩니다.
  • 환자는 프로토콜 치료를 받는 동안 추가 형태의 전신 항종양 요법이 필요할 계획이 없어야 합니다.
  • 환자는 등록 전에 활동성 B형 간염 바이러스(HBV) 또는 C형 간염 바이러스(HCV) 감염이 알려진 적이 없어야 합니다.
  • 환자는 등록 당시 전신 요법이 필요한 활동성 감염이 없어야 합니다.
  • 환자는 장기 동종이식을 해서는 안 됩니다.
  • 환자는 등록 전 14일 이내에 코르티코스테로이드(> 10 mg 매일 프레드니손 또는 이에 상응하는 것) 또는 기타 면역억제제로 전신 치료를 받은 적이 없어야 합니다. 흡입 또는 국소 스테로이드 및 부신 대체 용량 =< 10 mg 매일 프레드니손 또는 이에 상응하는 것은 활동성 자가면역 질환이 없는 경우 허용됩니다.
