제공자 대 환자 매개 캐스케이드 유전자 검사 새로 진단 된 대장 암 환자의 1도 친척의 유전자 검사
이 임상 시험은 새로 진단 된 결장 직장암 환자의 1도 친척에서 유전자 검사를 개선하기 위해 환자 (프로 밴드)-매개 커뮤니케이션을 제공자-매개 의사 소통과 비교합니다.
결장 직장암 사례의 30%는 유전 적 기초를 가지며, 이들 환자의 약 15%는 암 감수성 유전자에서 질병 유발 (병원성) 유전 (생식선) 변이체를 갖는 것으로 추정된다.
병원성 생식선 변이체를 운반하는 대부분의 개인은 암 위험을 알지 못하고 유전자 검사에 대한 지침을 충족하지 못할 수 있습니다.
암 환자에서 병원성 생식선 변이체 또는 유전성 암 증후군을 식별하면 암에 걸릴 위험이 높다는 것을 모르는 위험에 처한 친척에게 중요한 영향을 미칩니다.
이 위험을 1도 친척에게 전하는 부담은 종종 환자에게 해당되며, 유전자 검사 결과를 올바르게 공유하고 설명하기에 충분한 지식이 부족할 수 있습니다.
유전자 검사 결과의 제공자-매개 커뮤니케이션을받는 것은 유전자 위험을 보통 프로브-매개 의사 소통의 일반적인 관행보다 1도 친척에게 전달하는데 더 효과적 일 수있다.

I. 6 개월에 프로브-매개 (ARM A)와 비교하여 프로 밴드의 1도 친척 (FDR)을 린치 증후군과 비교하는 린치 증후군과 비교한다.
II. Proband의 FDR의 비율을 공급자-매개 암 (ARM B)에서 생식선 시험을 겪는 비-리크 치 병원성 생식선 변이체와 6 개월에 프로브 밴드-매개 암 (ARM A)을 비교합니다.
보조 목표 :
I. 공급자-매개 팔 (ARM B)과 프로브-매개 캐스케이드 테스트 암 (ARM A) 사이에서 12 개월에 질병 예방 노력을 거친 병원성 생식선 변이체 (PGV)와 FDR의 비율을 비교한다.
탐색 적 목표 :
I. Cascade 유전자 검사의 차이를 다음과 같은 프로브 하위 그룹 내에서 차이를 평가하기 위해 : 인종/민족성 (비 히스패닉 백인; 다른 인종/민족), 성 (여성; 남성), 연령 (> = 50; <50), 종양 위치 (콜론; 직장), 종양 (단계 III; (vs.) 커뮤니티 설정.
III. 새로 진단 된 CRC를 갖는 다중 사이트 암 협력 그룹 연구 집단에서의 선불 생식선 시험 결과를 분석함으로써 암 감수성 유전자에서 병원성 생식선 변이체의 유병률 및 스펙트럼을 결정한다.
IV. 인종/민족 소수 민족 및 농촌 개인을 포함한 인구 통계 학적 환자 하위 군의 발생 패턴을 평가 한 Eccrual 등록의 초기 50%와 재판이 끝날 때.
V. 직접 접촉 암 (ARM B)과 프로 밴드 지향 캐스케이드 테스트 암 (ARM A) 사이의 3 년에 프로 밴드의 질병이없는 생존 (DFS)을 비교합니다.
V. 환자 지시 된 질문으로 건강 설문지의 사회적 결정 요인의 완료율을 평가합니다.
개요:
1 단계 : 환자 (프로 밴드)는 혈액 샘플을 수집하고 연구에 대한 유전자 검사를받습니다.
2 단계 : 병원성 또는 병원성 생식선 변이체 및 이들의 FDR을 갖는 프로 밴드는 2 개의 팔 중 1 개로 무작위 화된다.
ARM A : FDR은 프로 밴드의 유전자 검사 결과에 대한 프로브-매개 커뮤니케이션을받습니다.
ARM B : FDR은 프로 밴드의 유전자 검사 결과에 대한 공급자-매개 커뮤니케이션을받습니다.
프로 밴드는 최대 3 년 동안 추적되며 FDR은 최대 1 년 동안 추적됩니다.

- 1 단계 프로 밴드 : 연령> = 18 세
1 단계 프로 밴드 : 새로 진단 된 환자 (등록 후 3 개월 이내), 1 차 대장 선암종, 1 단계 내지 IV
- 조직 학적으로 입증 된 단계 1 내지 IV 결장 또는 직장 선암종 (T 또는 N, M+). 결장에서 유래 한 것으로 간주되는 종양은 소장 (예를 들어, 회장 밸브에있는 소장)으로 연장 될 수 있습니다. 전체 종양이 결장에있는 경우 종양은 결장에서 유래 한 것으로 간주 될 것입니다. 직장 관여의 경우 암은 직장 1 차로 간주됩니다.
- 하나 이상의 1 차 결장 선암종 환자는 자격이 있습니다.
- 1 단계 Probands : 0 단계 또는 현장 결장 직장암 환자 없음
