CAPABLE Care + Connect: Home-Based Support for Isolated Older Adults

June 1, 2026 updated by: Johns Hopkins University

The CAPABLE Care + Connect study is testing an innovative home-based program designed to help older adults who struggle with daily activities and experience social isolation. This program combines proven rehabilitation services with social connection strategies, delivered right in participants' homes by a team of healthcare professionals.

For many older adults, especially those who are homebound, maintaining independence and social connections becomes increasingly difficult. The CAPABLE (Community Aging in Place—Advancing Better Living for Elders) program has already shown success in helping people perform daily tasks more easily. Now, researchers want to see if adding specific social connection components can further improve quality of life.

The program involves three key providers:

  • An occupational therapist (6 home visits) helps adapt the home environment and teaches techniques to make daily activities easier
  • A registered nurse (4 home visits) addresses health management needs
  • A handy worker makes minor home modifications like installing grab bars or improving lighting

Researchers believe this comprehensive approach may help homebound individuals not only function better at home but also reconnect with their communities. The study specifically focuses on people who score low on social connection measures or high on loneliness scales.

This research is particularly important because:

  1. Social isolation in older adults is linked to poorer health outcomes
  2. Few evidence-based programs currently exist to address this problem
  3. Homebound individuals often face multiple barriers to accessing care
  4. The program builds on existing successful models of home-based care

The study will measure improvements in several areas including:

  • Ability to perform daily activities (like bathing, dressing, cooking)
  • Pain levels
  • Depression symptoms
  • Social connections
  • Caregiver stress (for those with caregivers)

Participants will complete surveys before starting the program and after 4 months to track changes. Some will also share their experiences through interviews, helping researchers understand what works well and what could be improved.

This research matters because maintaining independence and social connections becomes increasingly challenging as we age, particularly when health issues limit mobility. The CAPABLE Care + Connect program could provide a model for helping older adults age comfortably at home while staying engaged with their communities—reducing the negative health impacts of isolation.

For caregivers, this study may offer valuable support in helping their loved ones maintain independence while potentially reducing their own caregiving burden. The program recognizes that supporting the care recipient often means supporting the whole caregiving team.

As our population ages and more people prefer to age in place, studies like this help identify practical, scalable solutions that improve quality of life while potentially reducing healthcare costs. The home-based approach may be particularly beneficial for those who struggle to attend clinic appointments or community programs.
