EVOLV-Rx VA Study: Reducing Unnecessary Medications for Older Veterans in Community Living Centers

For older Veterans living in Community Living Centers, medication safety is a critical concern. More than half of Veterans aged 65 and older in these facilities receive what experts call low-value prescribing - medications that may cause more harm than benefit. This places our honored Veterans at increased risk of adverse drug reactions, hospitalizations, and other serious health complications. The EVOLV-Rx VA study addresses this important issue by testing a new clinical support tool designed to help healthcare providers identify and safely reduce unnecessary medications.

The EVOLV-Rx system represents a significant advancement in medication safety for older adults. Developed through expert consensus and incorporating feedback from patients, caregivers, and clinicians, this innovative tool automatically detects 18 specific types of potentially inappropriate prescribing practices. What makes EVOLV-Rx particularly valuable is its focus on medications that can be safely reduced or discontinued while considering patient and family preferences. The tool provides pharmacists with detailed recommendations they can share with prescribing clinicians, helping prioritize medications that might be safely deprescribed.

This research is especially important because Community Living Centers provide an ideal environment to carefully monitor medication changes while ensuring patient safety. The study will be conducted across multiple VA facilities in the VISN 4 region using a thoughtful stepped-wedge design where each center serves as its own control. This approach allows researchers to carefully evaluate the intervention's effectiveness while gradually implementing it across all participating facilities.

For participating Veterans and their families, the study offers several potential benefits. By reducing unnecessary medications, Veterans may experience fewer side effects, lower risk of harmful drug interactions, and potentially improved quality of life. The study includes Veterans aged 65 and older who are admitted to participating Community Living Centers, though those receiving hospice care are excluded to ensure comfort-focused care remains undisturbed. Participants will continue to receive their standard medical care while benefiting from additional medication review through the EVOLV-Rx system. Pharmacists will use the dashboard as part of their regular medication review process, providing an extra layer of medication safety oversight.

Beyond the immediate benefits to participating Veterans, this research has broader implications for medication safety across the entire VA healthcare system. If successful, the EVOLV-Rx approach could be expanded nationally, helping ensure that all older Veterans receive medications that provide clear benefits while minimizing unnecessary risks.
