Water-Based Therapy Program for POTS Management Study
California State University, Dominguez Hills is conducting a groundbreaking clinical trial exploring aquatic occupational therapy as a treatment approach for adults living with Postural Orthostatic Tachycardia Syndrome (POTS). This innovative 12-week program specifically designed for POTS patients aged 18-64 represents a promising non-pharmacological intervention that could significantly improve quality of life for those struggling with this challenging condition.
The study will enroll 12 participants who will receive a comprehensive aquatic therapy program divided into three progressive phases. During the first month, therapy focuses on foundational skills including breathing techniques, proprioception, balance, and vestibular activities through gentle exercises like belly breathing, water walking, and aqua yoga poses. The second month builds strength and endurance with water marching and core exercises using specialized equipment. The final month integrates advanced strengthening with cognitive challenges, such as combining dumbbell exercises with word association games to address the brain fog and cognitive difficulties commonly experienced by POTS patients.
Each 60-minute therapy session includes 45 minutes of pool time with a gentle floating or stretching warm-up and an ai chi cooldown. The program is carefully individualized with activities graded up or down based on each participant's specific needs and tolerance levels. The aquatic environment provides unique advantages for POTS patients, as the water's buoyancy reduces orthostatic stress while still allowing for meaningful physical activity and rehabilitation.
Researchers will measure outcomes through multiple validated assessment tools including the WHO Quality of Life questionnaire, DePaul Symptom Questionnaire for post-exertional malaise, Composite Autonomic Symptom Score, and the Malmo POTS Scale. Additional measures include functional assessments through ADL and IADL questionnaires, active stand tests to monitor orthostatic responses, balance testing, heart rate monitoring both on land and in water, and perceived exertion scales. Cognitive function will be evaluated using the Stroop Test and Trail Making Test to assess the program's impact on the cognitive symptoms often associated with POTS.
The study also includes important secondary outcomes tracking enrollment rates, program completion, attendance, cost analysis, healthcare utilization changes, participant satisfaction, and exercise self-efficacy. This comprehensive approach ensures that both the clinical effectiveness and practical implementation aspects of the aquatic therapy program are thoroughly evaluated.
This research addresses a critical gap in POTS treatment options. Current management strategies often rely heavily on medications and lifestyle modifications that may provide limited relief. Aquatic therapy offers a unique approach that leverages the physical properties of water to create a therapeutic environment ideally suited for POTS patients who struggle with upright exercise and orthostatic intolerance. The buoyancy of water reduces gravitational stress on the cardiovascular system while still providing resistance for strength training and support for balance exercises.
The potential implications of this study are substantial. If successful, this program could provide a new standard of care for POTS rehabilitation, offering patients a safe and effective way to improve physical function, reduce symptoms, and enhance overall quality of life. The research may also contribute valuable insights into exercise prescription for dysautonomia patients and help establish evidence-based guidelines for aquatic therapy in autonomic disorders.
