Fáze II studie vysoké dávky tamoxifenu pro léčbu nově diagnostikovaného supratentoriálního multiformního glioblastomu (GBM)

ODŮVODNĚNÍ: Radiační terapie využívá k poškození nádorových buněk vysokoenergetické rentgenové záření. Léky, jako je tamoxifen, mohou způsobit, že nádorové buňky budou citlivější na radiační terapii.

ÚČEL: Studie fáze II studovat účinnost kombinace radiační terapie s tamoxifenem při léčbě pacientů s nově diagnostikovaným supratentoriálním multiformním glioblastomem.

CÍLE: I. Zjistit, zda vysoká dávka tamoxifenu zahájená na začátku konvenční radioterapie zlepšuje střední dobu přežití dospělých s nově diagnostikovaným supratentoriálním multiformním glioblastomem. II. Určete proveditelnost a toxicitu vysokých dávek tamoxifenu u těchto pacientů. III. Stanovte celkové přežití s ohledem na laboratorní koreláty proteinkinázy C u pacientů léčených tímto režimem.

PŘEHLED: Toto je multicentrická studie. Počínaje 5 týdny po operaci pacienti podstupují radioterapii mozku 5 dní v týdnu po dobu 6 týdnů. Pacientky dostávají vysoké dávky tamoxifenu perorálně každých 6 hodin počínaje 1. dnem radioterapie a pokračují v nepřítomnosti progrese onemocnění. Pacienti jsou sledováni každé 3 měsíce až do roku 1, každé 4 měsíce po dobu 1 roku, každých 6 měsíců po dobu 2 let a poté každý rok.

PŘEDPOKLÁDANÝ AKRUÁLNÍ AKRUÁLNÍ: Do této studie bude během 6 měsíců nashromážděno celkem 72 pacientů.

CHARAKTERISTIKA ONEMOCNĚNÍ: Histologicky prokázaný nově diagnostikovaný, supratentoriální multiformní glioblastom (s oblastmi nekrózy) Diagnóza musí být stanovena chirurgickou biopsií nebo excizí během posledních 5 týdnů Žádné recidivující maligní gliomy Žádné metastázy pod tentoriem nebo za klenbou lebeční

CHARAKTERISTIKY PACIENTA: Věk: Dospělý Výkonnostní stav: Karnofsky 70-100 % NEBO Zubrod 0-1 Předpokládaná délka života: Nejméně 8 týdnů Hematopoetický: Hemoglobin nejméně 10 g/dl (povolena transfuze) Hematokrit nejméně 30 % (transfuze povolena) Absolutní neutrofil počet alespoň 1 500/mm3 Počet krevních destiček alespoň 100 000/mm3 Játra: Bilirubin ne vyšší než 2,0 mg/dl sérová glutamát pyruvát transamináza (SGPT) nebo sérová glutamát oxaloacetát transamináza (SGOT) ne větší než 2krát normální dusík ledviny v krvi ne: vyšší než 25 mg/dl Kreatinin ne vyšší než 1,5 mg/dl Kardiovaskulární: Žádná aktivní tromboflebitida Jiné: Zotaveno z jakékoli pooperační infekce nebo jiných komplikací Žádná závažná zdravotní onemocnění nebo psychiatrická poškození, která by vylučovala účast ve studii Žádná aktivní porucha pojivové tkáně, jako je lupus nebo sklerodermie Žádná jiná malignita za posledních 5 let kromě nemelanomatózní rakoviny kůže nebo karcinomu in situ děložního čípku nebo močového měchýře Bez předchozího karcinomu endometria nebo atypické hyperplazie endometria Žádné AIDS Netěhotné nebo kojící Plodné pacientky musí používat účinnou antikoncepci
