Et randomiseret fase III-forsøg med Oxaliplatin (OXAL) Plus 5-Fluorouracil (5-FU)/Leucovorin (CF) med eller uden Cetuximab (C225) efter kurativ resektion for patienter med stadium III tyktarmskræft

Dette randomiserede fase III-studie blev oprindeligt designet til at sammenligne tre forskellige kombinationskemoterapiregimer for at se, hvor godt de virker. Pr. 1. september 2004 blev undersøgelsen udvidet til i alt 6 arme (de oprindelige 3 arme (A, B, C) og 3 yderligere arme, som var de samme som de første 3 men med cetuximab) til behandling af patienter, der har er blevet opereret for stadium III tyktarmskræft. Lægemidler, der bruges i kemoterapi, såsom irinotecanhydrochlorid, fluorouracil, leucovorin calcium og oxaliplatin, virker på forskellige måder for at stoppe tumorceller i at dele sig, så de holder op med at vokse eller dør. Monoklonale antistoffer såsom cetuximab kan lokalisere tumorceller og enten dræbe dem eller levere tumordræbende stoffer til dem uden at skade normale celler. Kombination af mere end et kemoterapilægemiddel med monoklonalt antistofbehandling og giv det efter operationen kan dræbe eventuelle resterende tumorceller. Det var ikke kendt på det tidspunkt, hvor denne undersøgelse blev udviklet, hvilken kombinationskemoterapikur, der er mere effektiv efter operation til behandling af tyktarmskræft. Denne undersøgelse havde flere vigtige ændringer, baseret på resultaterne af andre fase III forsøg. Fra 6/1/2005 fik patienterne ikke længere irinotecan i denne undersøgelse, og behandlingsarme B, C, E og F blev afbrudt. Patienter på arm B og C krydsede til arm A. Patienter på arm E og F krydsede til arm D. Patienter på arm C og F, som ikke var nået til irinotecan, fortsatte på henholdsvis arm A og D. Pr. 18/8/2008 blev præ-screening for Kirsten rottesarkom (KRAS) status tilføjet med mutant KRAS (eller KRAS ikke evaluerbar) patienter sat på arm G og vildtype KRAS patienter randomiseret mellem arm A og arm D. Patienter på arm G blev behandlet efter lægens skøn og fulgt for sygdom og overlevelsesstatus. KRAS blev bestemt i et centralt laboratorium og var proces for alle patienter i denne undersøgelse. Det primære endepunkt for denne undersøgelse blev ændret den 18/8/2008 for at fokusere på patienter med vildtype KRAS-tumorer. Alle ændringer blev godkendt af Central Institution Review Board, lokale Institutional Review Boards, NCI og NCCTG Data Safety Monitoring Board.

OVERSIGT: Dette er en randomiseret, multicenter undersøgelse. Patienterne stratificeres efter positiv lymfeknudeinvolvering (1-3 vs 4 eller mere), histologi (høj [dårligt differentieret eller udifferentieret] vs lav [godt til moderat differentieret]) og klinisk T-stadium (T1 eller T2 vs. T3 vs. T4 ). Patienter randomiseres til 1 ud af 6 behandlingsarme (pr. 6/1/2005 er patienterne randomiseret til kun behandlingsarm I og IV; arme II, III, V og VI er lukket for optjening). Fra 18/8/2008 blev præscreening for KRAS-status tilføjet med mutant KRAS (eller KRAS ikke evaluerbar) patienter sat på arm G og vildtype KRAS patienter randomiseret mellem arm A og arm D.

ARM A: Patienterne får oxaliplatin intravenøst (IV) over 2 timer, leucovorin calcium IV over 2 timer og fluorouracil IV kontinuerligt over 46-48 timer på dag 1. Behandlingen gentages hver 14. dag i op til 12 kure i fravær af uacceptabel toksicitet eller tilbagevendende sygdom.

ARM B (lukket for periodisering pr. 6/1/2005 - aktuelt tilmeldte patienter kan gå over til arm I for resten af behandlingen): Patienterne får irinotecan hydrochlorid IV over 2 timer på dag 1 og leucovorin calcium og fluorouracil som i arm A Behandling gentages hver 14. dag i op til 12 kure i fravær af uacceptabel toksicitet eller tilbagevendende sygdom.

ARM C (lukket for optjening pr. 6/1/2005 - aktuelt tilmeldte patienter kan gå over til arm I for resten af behandlingen): Patienter får samme behandling som i arm A i 6 forløb efterfulgt af samme behandling som i arm B for 6 kurser (i alt 12 kurser). Behandlingen fortsætter i fravær af uacceptabel toksicitet eller tilbagevendende sygdom.

ARM D: Patienterne får cetuximab* IV over 1 time på dag 1 og 8 og oxaliplatin, leucovorin calcium og fluorouracil som i arm A. Behandlingen gentages hver 14. dag i op til 12 kure i fravær af uacceptabel toksicitet eller tilbagevendende sygdom.

ARM E (lukket for periodisering pr. 6/1/2005 - aktuelt tilmeldte patienter kan gå over til arm D for resten af behandlingen): Patienterne får cetuximab* som i arm D og irinotecan hydrochlorid, leucovorin calcium og fluorouracil som i arm B. Behandlingen gentages hver 14. dag i op til 12 kure i fravær af uacceptabel toksicitet eller tilbagevendende sygdom.

ARM F (lukket for periodisering pr. 6/1/2005 - aktuelt tilmeldte patienter kan gå over til arm D for resten af behandlingen): Patienterne får cetuximab* som i arm D og kemoterapi som i arm C.

ARM G (tilføjet fra 18.8.2008, mutant KRAS (eller KRAS ikke evaluerbar) patienter): Lokalt rettet terapi.

BEMÆRK: *Cetuximab administreres over 2 timer i en højere dosis på dag 1, selvfølgelig 1.

Livskvalitet (QOL) vurderes ved baseline, 3 måneder og ved slutningen af behandlingen. Fra 18/8/2008 blev QOL afbrudt.

Patienterne følges i maksimalt 8 år fra randomisering.
