Et randomiseret fase III-forsøg med adjuverende terapi, der sammenligner kemoterapi alene (seks cyklusser af Docetaxel Plus cyclophosphamid eller fire cyklusser af Doxorubicin Plus Cyclophosphamid efterfulgt af ugentlig paclitaxel) med kemoterapi plus trastuzumab hos kvinder med node-positiv eller høj-Negisk HER2 Invasiv brystkræft

Dette randomiserede fase III kliniske forsøg studerer kemoterapi med eller uden trastuzumab efter operation for at se, hvor godt de virker i behandlingen af kvinder med invasiv brystkræft. Lægemidler, der bruges i kemoterapi, virker på forskellige måder for at stoppe væksten af tumorceller, enten ved at dræbe cellerne, ved at stoppe dem i at dele sig eller ved at stoppe dem i at sprede sig. PRIMÆRE MÅL:

I. SEKUNDÆRE MÅL:

OVERSIGT: Patienterne randomiseres til 1 ud af 2 behandlingsarme.

BEMÆRK: *Kemoterapiregimen er baseret på investigatorens præference.

ARM I:

GRUPPE IA: Patienter får docetaxel intravenøst (IV) over 60 minutter og cyclophosphamid IV over 30 minutter på dag 1. Behandlingen gentages hver 3. uge i 6 cyklusser i fravær af sygdomsprogression eller uacceptabel toksicitet.

GRUPPE IB: Patienterne får doxorubicinhydrochlorid IV over 15 minutter og cyclophosphamid IV over 30 minutter på dag 1. Behandlingen gentages hver 2. eller 3. uge i 4 cyklusser i fravær af sygdomsprogression eller uacceptabel toksicitet. Begyndende 2-3 uger efter sidste dosis af doxorubicinhydrochlorid og cyclophosphamid får patienterne også paclitaxel IV over 60 minutter en gang om ugen i 12 doser i fravær af sygdomsprogression eller uacceptabel toksicitet.

ARM II:

GRUPPE IIA: Patienter får docetaxel og cyclophosphamid som i gruppe IA. Patienterne får også trastuzumab IV over 30-90 minutter på dag 1. Kurser gentages hver 3. uge i 1 år i fravær af sygdomsprogression eller uacceptabel toksicitet.

GRUPPE IIB: Patienter får doxorubicinhydrochlorid, cyclophosphamid og paclitaxel som i gruppe IB. Patienterne får også trastuzumab IV over 30-90 minutter ugentligt i 12 doser og derefter hver 3. uge for efterfølgende doser. Behandlingen gentages hver 3. uge i 1 år i fravær af sygdomsprogression eller uacceptabel toksicitet.

Efter afsluttet undersøgelsesbehandling følges patienterne op hver 6. måned i 5 år og derefter hver 12. måned i 5 år.

Inklusionskriterier:

Patienter bør have en forventet levetid på mindst 10 år, eksklusive deres diagnose brystkræft; (komorbide tilstande bør tages i betragtning, men ikke diagnosen brystkræft)
Kvinder med reproduktionspotentiale skal acceptere at bruge en effektiv ikke-hormonel præventionsmetode (f.eks. kondomer, nogle intrauterine anordninger, mellemgulv, tubal ligering, vasektomiseret partner eller abstinens) under behandlingen og i mindst 6 måneder (arm 1-patienter) og i mindst 7 måneder (arm 2-patienter) efter den sidste dosis af undersøgelsesterapi (kemoterapi eller trastuzumab)
Indsendelse af tumorprøver fra brystoperationen er påkrævet for alle patienter; derfor skal den lokale patologiafdelings politik vedrørende frigivelse af tumorprøver overvejes i screeningsprocessen; Patienter, hvis tumorprøver er placeret på en patologisk afdeling, der ifølge politik ikke vil indsende prøver til forskningsformål, bør ikke kontaktes for deltagelse i B-47-forsøget

Alle følgende iscenesættelseskriterier (ifølge 7. udgave af American Joint Committee on Cancer [AJCC] Cancer Staging Manual) skal være opfyldt:
- Ved patologisk evaluering skal primær tumor være pT1-3
- Ved patologisk evaluering skal ipsilaterale noder være pN0, pN1 (pN1mi, pN1a, pN1b, pN1c), pN2a, pN2b, pN3a eller pN3b
  - Hvis pN0, skal et af følgende kriterier være opfyldt:
    - pT2 og østrogenreceptor (ER) negativ og progesteronreceptor (PgR) negativ; eller
    - pT2 og ER positive (PgR status kan være positiv eller negativ) og enten grad 3 histologi eller Oncotype DX recidivscore på >= 25; eller
    - pT3 uanset hormonreceptorstatus, histologisk karakter og Oncotype DX-tilbagefaldsscore
HER2-status for den primære tumor skal evalueres før randomisering; al udført test skal indikere, at tumoren er HER2-lav som defineret nedenfor
- IHC skal udføres, og IHC-farvningsresultaterne skal angive en score på 1+ (in situ hybridisering [ISH] test er ikke påkrævet) eller 2+ (ISH skal også udføres og skal indikere, at tumoren er HER2-lav som beskrevet nedenfor )
- Hvis ISH-test udføres, skal testresultaterne være som følger, og IHC skal være 1+ eller 2+: forholdet mellem HER2 og kromosomoptællingsprobe 17 (CEP17) skal være < 2,0 eller, hvis et forhold ikke blev udført, HER2-genet kopiantal skal være < 4 pr. kerne

Patienten skal have gennemgået enten total mastektomi eller brystbevarende operation (lumpektomi); (patienter, der har fået foretaget en brystvortebesparende mastektomi er berettiget)
For patienter, der gennemgår lumpektomi, skal marginerne af den resekerede prøve være histologisk fri for invasiv tumor og ductal carcinoma in situ (DCIS) som bestemt af den lokale patolog; hvis patologisk undersøgelse viser tumor ved resektionslinjen, kan yderligere operative procedurer udføres for at opnå klare marginer; hvis tumor stadig er til stede ved den resekerede margin efter re-excision(er), skal patienten gennemgå total mastektomi for at være berettiget; (patienter med marginer positive for lobulært karcinom in situ [LCIS] er kvalificerede uden yderligere resektion)
For patienter, der gennemgår mastektomi, skal marginerne være fri for grov resterende tumor; (patienter med mikroskopiske positive marginer er berettigede, så længe postmastektomi strålebehandling [RT] af brystvæggen vil blive administreret)
Patienten skal have gennemført en af nedenstående procedurer til evaluering af patologisk nodalstatus:
- Sentinel lymfadenektomi alene:
  - Hvis patologisk nodal stadieinddeling baseret på sentinel lymfadenektomi er pN0 eller pN1b
  - Hvis patologisk nodal stadieinddeling baseret på sentinel lymfadenektomi er pN1mi eller pN1a, skal den primære tumor være T1 eller T2 ved patologisk evaluering, og den nodale involvering skal begrænses til 1 eller 2 positive noder
- Sentinel lymfadenektomi efterfulgt af fjernelse af yderligere non-sentinel lymfeknuder, hvis sentinel node (SN) er positiv; eller
- Axillær lymfadenektomi med eller uden SN-isoleringsprocedurer
Intervallet mellem den sidste operation for brystkræft (behandling eller iscenesættelse) og randomisering må ikke være mere end 84 dage

Alkalisk fosfatase skal være =< 2,5 x ULN for laboratoriet

Patienter med AST eller alkalisk fosfatase > ULN er berettiget til at blive inkluderet i undersøgelsen, hvis leverbilleddannelse (computertomografi [CT], magnetisk resonansbilleddannelse [MRI], positronemissionstomografi [PET]-CT eller PET-scanning) udført inden for 90 dage før til randomisering viser ikke metastatisk sygdom, og ovenstående krav er opfyldt

Vurdering af venstre ventrikulær ejektionsfraktion (LVEF) skal udføres inden for 90 dage før randomisering; LVEF-vurdering udført ved 2-dimensionelt (D) ekkokardiogram foretrækkes, men multi-gated acquisition (MUGA) scanning kan erstattes baseret på institutionelle præferencer
- For patienter, der vil modtage TC-kemoterapiregimet, skal LVEF være >= 50 % uanset hjertebilledbehandlingsfacilitetens nedre normalgrænse
- For patienter, der vil modtage AC-->WP kemoterapiregimen, skal LVEF være >= 55 % uanset hjertebilledbehandlingsfacilitetens nedre grænse for normal
- BEMÆRK: Da præ-entry LVEF fungerer som basislinjen for sammenligning af efterfølgende LVEF-vurderinger, er det afgørende, at denne baseline-undersøgelse er en nøjagtig vurdering; hvis baseline LVEF er > 70 %, opfordres investigator til at få bekræftet nøjagtigheden af det oprindelige LVEF resultat og gentage testen, hvis nøjagtigheden er usikker

  - 3+ ved vurdering af enhver prøve
- ISH med et forhold på HER2 til CEP17 >= 2,0 ved evaluering af enhver prøve
- ISH-resultat, der angiver HER2-genkopinummer >= 4 pr. kerne ved evaluering af enhver prøve
T4-tumorer inklusive inflammatorisk brystkræft
Endelige kliniske eller radiologiske tegn på metastatisk sygdom
- BEMÆRK: Brystbilleddannelse (obligatorisk for alle patienter) og anden billeddannelse (hvis påkrævet) skal være udført inden for 90 dage før randomisering
Synkron eller tidligere kontralateral invasiv brystkræft (patienter med synkron og/eller tidligere kontralateral DCIS eller LCIS er kvalificerede)
Enhver tidligere historie med ipsilateral invasiv brystkræft eller ipsilateral DCIS; (patienter med synkron eller tidligere ipsilateral LCIS er kvalificerede)
Anamnese med ikke-brystmaligniteter (bortset fra in situ-kræft, der kun er behandlet med lokal excision og basalcelle- og pladecellekarcinomer i huden) inden for 5 år før randomisering
Tidligere behandling med antracykliner, taxaner eller trastuzumab for enhver malignitet

- For patienter, der vil modtage TC (uanset patientens alder): ukontrolleret hypertension defineret som vedvarende systolisk blodtryk (BP) > 150 mm Hg eller diastolisk BP > 90 mm Hg; (patienter med initiale BP-forhøjelser er berettigede, hvis initiering eller justering af BP-medicin sænker trykket for at opfylde adgangskriterierne)
- For patienter < 50 år, som vil modtage AC-->WP: ukontrolleret hypertension defineret som vedvarende systolisk BP > 150 mm Hg eller diastolisk BP > 90 mm Hg; patienter med initiale BP-stigninger er kvalificerede, hvis initiering eller justering af BP-medicin sænker trykket for at opfylde adgangskriterierne
- For patienter >= 50 år, som vil modtage AC-->WP:
  - Ukontrolleret hypertension defineret som vedvarende systolisk BP > 150 mm Hg eller diastolisk BP > 90 mm Hg
  - Kontrolleret hypertension (systolisk BP =< 150 mm Hg og diastolisk BP =< 90 mmHg), hvis der er behov for antihypertensiv medicin(er)

Eksperimentel: Arm II (kemoterapi, trastuzumab) GRUPPE IIA: Patienter får docetaxel og cyclophosphamid som i gruppe IA. Patienterne får også trastuzumab IV over 30-90 minutter på dag 1. Kurser gentages hver 3. uge i 1 år i fravær af sygdomsprogression eller uacceptabel toksicitet. GRUPPE IIB: Patienter får doxorubicinhydrochlorid, cyclophosphamid og paclitaxel som i gruppe IB. Patienterne får også trastuzumab IV over 30-90 minutter ugentligt i 12 doser og derefter hver 3. uge for efterfølgende doser. Ændring i HER2 mRNA-niveau Tidsramme: Fra baseline til 24 måneder	Interaktionen mellem behandlingseffekt og HER2-mRNA-niveau vil blive evalueret i proportional hazards-modellen, som vil omfatte en indikator for behandlingsgruppe og HER2-mRNA-niveauet som en kontinuert variabel og det tilsvarende interaktionsudtryk. Log-hazard ratio plottet for interaktionsudtrykket med 95 % konfidensintervaller vil blive brugt til at bestemme cut-off af HER2 mRNA niveauet, som ville bestemme en undergruppe af de patienter, som ville have gavn af den adjuverende trastuzumab behandling. Fcgamma-receptorpolymorfi Tidsramme: Fra baseline til op til 24 måneder	Polymorfien af Fcgamma 158-receptorgenet vil blive dikotomiseret som V/V (valin) vs. andre genotyper, og polymorfien af Fcgamma 131-receptorgenet vil blive dikotomiseret som H/H (histidin) vs. andre genotyper for at evaluere en interaktion mellem behandlingsgruppe og Fcgamma-polymorfi. Kaplan-Meier-metoden105 vil blive brugt til at estimere fordelingen af de primære endepunkter stratificeret af de dikotomiserede undergrupper, og log-rang-testen106 vil blive brugt til statistisk at sammenligne tid-til-hændelse-fordelingerne.
