Fulvestrant og/eller Anastrozol til behandling af postmenopausale patienter med stadium II-III brystkræft, der gennemgår operation

Alternative tilgange til klinisk fase II eller III østrogenreceptor positiv neoadjuverende brystkræftbehandling (ALTERNAT) hos postmenopausale kvinder: Et fase III-studie
Undersøgelsen udføres for at afgøre, om neoadjuverende endokrin behandling med fulvestrant eller kombinationen af anastrozol og fulvestrant er bedre end anastrozol, når det gives før operation for at skrumpe kræften og stoppe den i at vokse.
Anastrozol hæmmer tumorvækst ved at reducere niveauet af østrogen og er blevet godkendt af Food and Drug Administration (FDA) i USA til brug efter operation til postmenopausale kvinder med østrogenreceptorpositiv brystkræft.
Det betragtes også som en standard for pleje at give anastrozol i et par måneder før operation for at skrumpe tumoren.
Fulvestrant hæmmer tumorcellevækst ved at reducere niveauet af østrogenreceptor i tumorcellen.
Det er ikke godkendt af FDA til brug hos kvinder med tidligt stadium af brystkræft før eller efter operation, men er godkendt af FDA til patienter med fremskreden (stadie 4) østrogenreceptor positiv brystkræft, der har spredt sig til andre dele af kroppen.

Dette kliniske forsøg var designet til at undersøge de patologiske resultater hos patienter, hvis neoadjuverende behandlingsforløb bestemmes ved hjælp af en tidlig markør for endokrin resistens (nemlig Ki67 efter 4 eller 12 ugers neoadjuverende terapi) samt at vurdere det kliniske resultat af patienter, hvis sygdomsbyrde efter afsluttende neoadjuverende endokrin behandling er klassificeret som Modificeret PEPI 0.
De primære og sekundære mål for undersøgelsen er beskrevet nedenfor.
Primære mål:
- For at bestemme om fulvestrant administreret i 24 uger som neoadjuverende endokrin behandling nedsætter andelen af endokrine resistente tumorer* i forhold til patienter behandlet med anastrozol.
- For at bestemme om fulvestrant i kombination med anastrozol, administreret i 24 uger som neoadjuverende endokrin behandling, nedsætter andelen af endokrine resistente tumorer* i forhold til patienter behandlet med anastrozol.
- At vurdere, om 5-års RFS-raten blandt kvinder med et modificeret præoperativt endokrint prognostisk indeks (PEPI) score på 0 efter 24 ugers neoadjuverende anastrozolbehandling er mindst 95 %.
- At vurdere om 5-års RFS-raten blandt kvinder med en modificeret PEPI-score på 0 efter 24 ugers neoadjuverende fulvestrant eller fulvestrant i kombination med anastrozol er mindst 95 %. Bemærk, at dette mål kun vil blive testet, hvis den valgte fulvestrantarm viste sig at være overlegen i forhold til anastrozol i mål 1 eller 2.
Endokrin resistent tumor er defineret af et af følgende kriterier*:
- Ki67> 10% efter 4 uger på neoadjuverende endokrin behandling
- Ki67> 10% efter 12 uger på neoadjuverende endokrin behandling
- Progressiv sygdom er dokumenteret når som helst under neoadjuverende endokrin behandling
Kirurgiske fund 22-24 uger efter neoadjuverende endokrin behandling er sådan, at:
- pT-stadiet er 3/4
- positive lymfeknuder er til stede eller Ki67 > 2,7 % (dvs. modificeret PEPI-score, der ikke er 0)
- Seponeret neoadjuverende endokrin behandling uanset årsag
Sekundære mål:
- At vurdere, om 5-års RFS-raten blandt kvinder med et præoperativt endokrint prognostisk indeks PEPI-score på 0 efter 24 ugers neoadjuverende anastrozolbehandling er mindst 95 %.
- At undersøge forskellene i kirurgisk udfald, kliniske og radiologiske responsrater og sikkerhedsprofil mellem fulvestrant-armen og anastrozol-armen.
- At undersøge forskellene i kirurgisk udfald, kliniske og radiologiske responsrater og sikkerhedsprofil mellem patienter randomiseret til fulvestrant i kombination med anastrozol og patienter randomiseret til anastrozol.
- At undersøge hastigheden af patologisk komplet respons (pCR) af 12 ugers neoadjuverende paclitaxel hos patienter med endokrin resistent sygdom efter 4 uger eller 12 ugers neoadjuverende endokrin behandling (med enten fulvestrant eller anastrozol eller kombinationen af fulvestrant og anastrozol).
- At undersøge hastigheden af patologisk komplet respons (pCR) blandt de patienter med endokrin resistent sygdom, efter 4 uger eller 12 ugers neoadjuverende endokrin behandling (med enten fulvestrant eller anastrozol eller kombinationen af fulvestrant og anastrozol), som vælger ikke at få neoadjuverende paclitaxel, men en anden standard neoadjuverende taxan- og/eller anthracyclinholdig kur eller CMF.
- For at opsummere hyppigheden af alvorlige (NCI CTCAE grad > 3) bivirkninger ved administration af paclitaxel i neoadjuverende omgivelser.
- At vurdere RFS for patienter med endokrine resistente tumorer defineret som: 1) Ki67 > 10 % i uge 4, 2) Ki67 > 10 % i uge 12 og 3) modificeret PEPI-score på ikke-nul på neoadjuverende endokrin behandling med alle tre grupper kombineret eller adskilt.

Inklusionskriterier:
- Kvinde ≥18 år
- Eastern Cooperative Oncology Group (ECOG) præstationsstatus 0-2
Postmenopausal, verificeret af:
- post bilateral kirurgisk ooforektomi eller
- ingen spontan menstruation ≥ 1 år eller
- ingen menstruation i < 1 år med follikelstimulerende hormon (FSH) og østradiolniveauer i postmenopausalt område ifølge institutionelle standarder
- Patologisk bekræftelse af invasiv brystkræft diagnosticeret ved kernenålebiopsi
Klinisk T2-T4c, enhver N, M0 invasiv brystkræft, af AJCC 7. udgave klinisk stadieinddeling, med målet at være operation for at fuldføre udskæring af tumoren i brystet og lymfeknuden. Primær tumor skal være:
- håndgribelig
- dens største diametre er mindst 2,0 cm ved fysisk undersøgelse eller ved radiologisk vurdering
Bemærk:
- Patienter med kontralateralt duktalt carcinom in situ og/eller invasiv brystkræft er ikke kvalificerede.
- Patienter med multicentrisk brystkræft (defineret som mere end én læsion er invasiv brystkræft i det samme bryst adskilt af ≥ 2 cm normalt brystvæv er ikke kvalificerede.
- Invasiv brystkræft er østrogenreceptorpositiv med en Allred-score på 6, 7 eller 8 ifølge lokal institutions standardprotokol. Hvis en Allred Score ikke er rapporteret i den diagnostiske patologirapport, er ER-positivitet i > 66 % celler berettiget. Hvis ER-positivitet er ≤ 66 %, er farvningsintensiteten (svag, middel, stærk) nødvendig for at beregne Allred-score for at bestemme egnethed.
Invasiv brystkræft er Human Epidermal Growth Factor Receptor 2 (HER2) - En patient anses for at have HER2 negativ brystkræft, hvis et af følgende gælder:
- 0 eller 1+ ved immunhistokemi (IHC) og ISH ikke udført
- 0 eller 1+ ved IHC og ISH-forhold (HER2-genkopi/kromosom 17) < 2
- 2+ ved IHC og ISH-forhold (HER2-genkopi/kromosom 17) < 2
- Dokumentation af mammografi og ultralyd (herunder ductal carcinoma in situ (DCIS) og invasiv cancer) af det syge bryst udført inden for 56 dage før registrering. Mammogram for det upåvirkede kontralaterale bryst er påkrævet inden for 12 måneder før registrering.
Laboratorieværdier (≤ 14 dage før registrering):
- Absolut neutrofiltal (ANC) > 1000/mm^3
- Blodpladeantal > 100.000/mm^3
- Total bilirubin < 1,5 x øvre normalgrænse (ULN)
- Kreatinin < 1,5 x ULN
- Serum alanin transaminase (ALT) < 2,5 x ULN
- Vævserhvervelse: Patienten skal acceptere at levere de nødvendige forskningsbiopsier ved baseline, uge 4 og ved operationen til integrerede og integrerede biomarkører og korrelative undersøgelser.
Ekskluderingskriterier:
- Præmenopausal status
- Inflammatorisk brystkræft defineret som klinisk signifikant erytem i brystet og/eller dokumenteret dermal lymfatisk invasion (ikke direkte hudinvasion af tumor eller peau d' orange uden erytem).
- En excisionsbiopsi af denne brystkræft.
- Hormonerstatningsterapi af enhver type, megestrolacetat eller raloxifen inden for en uge før registrering.
- Tumor østrogenreceptor (ER) Allred score mellem 0-5 eller HER2 positiv ved IHC (3+) eller amplificeret af FISH > 2,0.

- Klinisk eller radiografisk tegn på metastatisk sygdom. Metastatisk oparbejdning er ikke påkrævet, men anbefales til patienter med klinisk stadium III sygdom. Bemærk: Isoleret ipsilateral supraclavikulær knudepåvirkning er tilladt.
- Brystimplantater er kun kontraindiceret, hvis implantatet udelukker de nødvendige forskningsbiopsier eller interfererer med at palpere brystlæsionen.
- Behandling for denne kræftsygdom, herunder kirurgi, strålebehandling, kemoterapi, bioterapi, hormonbehandling eller forsøgsmiddel før studiestart.
