Fase II/III-undersøgelse af cirkulerende tumor-DNA som en forudsigelig biomarkør i adjuverende kemoterapi hos patienter med stadium IIA coloncancer (COBRA)

Dette fase II/III-forsøg undersøger, hvor godt cirkulerende tumor-deoxyribonukleinsyre (ctDNA) test i blodet virker til at forudsige behandling for patienter med stadium IIA tyktarmskræft efter operation. ctDNA er cirkulerende tumorceller, som udskilles af tumorer i blodet. At finde ctDNA i blodet betyder, at der med stor sandsynlighed er nogle små mængder kræft tilbage efter operationen. Denne kræftsygdom kan dog, hvis den opdages, ikke findes på andre tests, der normalt bruges til at finde kræft, da den er for lille. Test for ctDNA-niveauer kan hjælpe med at identificere patienter med tyktarmskræft efter operation, som har gavn af, og dem, der ikke har gavn af, at modtage kemoterapi.

PRIMÆRE MÅL:

At sammenligne recidivfri overlevelse (RFS) hos "ctDNA-detekteret" patienter behandlet med eller uden adjuverende kemoterapi efter resektion af stadium IIA coloncancer. (Fase III)

SEKUNDÆRE MÅL:

I. At beskrive forekomsten af påviselig ctDNA hos patienter med stadium IIA coloncancer efter kirurgisk resektion.

II. At estimere resultater fra tid til hændelse (overordnet overlevelse [OS], tilbagefaldsfri overlevelse [RFS] og tid til tilbagefald [TTR]) ved ctDNA-markørstatus og behandling for patienter med resekeret stadium IIA tyktarmskræft.

III. At estimere graden af compliance med adjuverende kemoterapi og/eller aktiv overvågning for patienter med resekeret stadium IIA tyktarmskræft.

UNDERSØGENDE MÅL:

I. At beskrive sammenhængen mellem kvantitative ctDNA-niveauer med tid til hændelsesresultater (RFS, OS og TTR).

II. At karakterisere genomiske profiler forbundet med recidiv ved hjælp af et ctDNA-assay hos patienter med resekeret stadium IIA tyktarmskræft.

III. At modellere omkostningseffektiviteten af brugen af ctDNA i forhold til standardbehandling i denne indstilling.

IV. At evaluere ydeevnen af et ctDNA-assay efter inkorporering af patienttumor og mononukleære celler fra perifert blod.

OVERSIGT: Patienterne randomiseres til 1 ud af 2 arme.

ARM I (BLOD OPBEVARET OG TESTET FOR ctDNA SENERE): Patienter gennemgår aktiv overvågning.

ARM II (BLODTESTET FOR ctDNA VED BASELINE): Patienter tildeles 1 ud af 2 grupper.

GRUPPE I (CtDNA DETECTED): Efter investigators skøn får patienter enten oxaliplatin intravenøst (IV) over 2 timer på dag 1, leucovorin IV over 2 timer på dag 1 og fluorouracil IV bolus over 2-4 minutter på dag 1 og derefter ved kontinuerlig IV over 46-48 timer gentaget hver 14. dag i op til 12 cyklusser i fravær af sygdomsprogression eller uacceptabel toksicitet eller oxaliplatin IV over 2 timer på dag 1 og capecitabin oralt (PO) to gange dagligt (BID) på dage 1-14 gentaget hver 21. dag i op til 8 cyklusser i fravær af sygdomsprogression eller uacceptabel toksicitet.

GRUPPE II (ctDNA IKKE DETECTED): Patienter gennemgår aktiv overvågning.

Efter afsluttet undersøgelsesbehandling følges patienterne op efter 12 måneder og derefter hver 6. måned i 2 år.

Inklusionskriterier:

Patienten skal have underskrevet og dateret en Institutional Review Board (IRB)-godkendt samtykkeformular, der er i overensstemmelse med føderale og institutionelle retningslinjer.
Eastern Cooperative Oncology Group (ECOG) præstationsstatus på 0 eller 1.
Histologisk/patologisk bekræftet stadie IIA adenokarcinom i colon (T3, N0, M0) med mindst 12 lymfeknuder undersøgt på tidspunktet for kirurgisk resektion.
Egnet til aktiv overvågning (dvs. ingen adjuverende kemoterapi) efter skøn og som dokumenteret af den evaluerende onkolog baseret på nuværende praksismønstre.
Tumorens distale udstrækning skal være >= 12 cm fra analkanten ved præ-kirurgisk endoskopi (dvs. ekskl. rektale adenokarcinomer, der berettiger behandling med kemoradiation). Hvis patienten ikke har gennemgået en prækirurgisk endoskopi, skal den distale udstrækning af tumoren være >= 12 cm fra analkanten som bestemt ved kirurgisk undersøgelse eller præoperativ billeddannelse.
Patienten skal have haft en en bloc fuldstændig grov resektion af tumor (kurativ resektion) som endelig kirurgisk cancerbehandling inden for 14 til 60 dage efter randomisering af undersøgelsen. Patienter, der har gennemgået en to-trins kirurgisk procedure for først at foretage en dekompressiv kolostomi og derefter, i en senere procedure, for at få den endelige kirurgiske resektion, er berettigede.
Tilgængelighed og tilvejebringelse af passende kirurgisk tumorvæv til molekylær diagnostik og bekræftende profilering.
Absolut neutrofiltal (ANC) skal være >= 1200/mm^3 (inden for 28 dage før randomisering).
Blodpladetal skal være >= 100.000/mm^3 (inden for 28 dage før randomisering); og
Hæmoglobin skal være >= 9 g/dL (inden for 28 dage før randomisering).
Total bilirubin skal være =< ULN (øvre grænse for normal) for laboratoriet (inden for 28 dage før randomisering), medmindre patienten har en kronisk grad 1 bilirubinstigning på grund af Gilberts sygdom eller lignende syndrom, der involverer langsom konjugering af bilirubin; og
Alkalisk fosfatase skal være < 2,5 x ULN for laboratoriet (inden for 28 dage før randomisering); og
Aspartataminotransferase (AST) og alaninaminotransferase (ALT) skal være < 1,5 x ULN for laboratoriet (inden for 28 dage før randomisering).
Serumkreatinin =< 1,5 x ULN for laboratoriet eller målt eller beregnet kreatininclearance >= 50 ml/min ved hjælp af Cockcroft-Gault-formlen for patienter med kreatininniveauer > 1,5 x ULN for laboratoriet (inden for 28 dage før randomisering).
Graviditetstest (urin eller serum i henhold til institutionel standard) udført inden for 14 dage før randomisering skal være negativ (kun for kvinder i den fødedygtige alder).
Patienter, der får et coumarin-derivat antikoagulant, skal acceptere ugentlig monitorering af international normalized ratio (INR), hvis de er randomiseret til arm 2 og får capecitabin.

Ekskluderingskriterier:

Anden tyktarmskræfthistologi end adenokarcinom (dvs. neuroendokrint karcinom, sarkom, lymfom, pladecellekarcinom osv.).
Patologiske, kliniske eller radiologiske tegn på åbenlys metastatisk sygdom. Dette omfatter isolerede, fjerntliggende eller ikke-sammenhængende intraabdominale metastaser, selv hvis der er resektioneret (herunder tilstedeværelsen af satellitknuder, der udgør N1c-sygdom i fravær af lymfeknudeinvolvering).
Tumorrelateret tarmperforation.
Anamnese med tidligere invasiv colon malignitet, uanset sygdomsfrit interval.
Historie om organtransplantation.
Enhver tidligere systemisk kemoterapi, målrettet terapi eller immunterapi; eller strålebehandling administreret som behandling for kolorektal cancer (f.eks. er primære rektale adenokarcinomer, for hvilke behandling med neoadjuverende kemoradiation er berettiget, ikke tilladt).
Anden invasiv malignitet inden for 5 år før randomisering. Undtagelser er colonpolypper, non-melanom hudkræft eller carcinoma-in-situ inklusive dem i livmoderhalsen og brystet (DCIS).
Synkron primær rektal- og/eller tyktarmskræft.
Antineoplastisk terapi (f.eks. kemoterapi, målrettet terapi eller immunterapi) inden for 5 år før randomisering. (I forbindelse med denne undersøgelse betragtes hormonbehandling ikke som kemoterapi).
Ukontrolleret hjertesygdom, efter den behandlende medicinske onkologs opfattelse, ville udelukke brugen af nogen af de lægemidler, der er inkluderet i GI005-behandlingsregimet. Dette omfatter, men er ikke begrænset til:
- Klinisk ustabil hjertesygdom, herunder ustabil atrieflimren, symptomatisk bradykardi, ustabil kongestiv hjerteinsufficiens, aktiv myokardieiskæmi eller indlagt midlertidig pacemaker.
- Ventrikulær takykardi eller supraventrikulær takykardi, der kræver behandling med klasse Ia antiarytmika (fx quinidin, procainamid, disopyramid) eller klasse III antiarytmika (fx sotalol, amiodaron, dofetilid). Brug af andre antiarytmika er tilladt.
- Anden eller tredje grads atrioventrikulær (AV) blokering, medmindre den behandles med en permanent pacemaker.
- Komplet venstre grenblok (LBBB), medmindre det behandles med en permanent pacemaker.
Sensorisk eller motorisk neuropati >= grad 2, ifølge Common Terminology Criteria for Adverse Events (CTCAE) version (v) 5.0.
Aktiv anfaldsforstyrrelse ukontrolleret af medicin.
Aktiv eller kronisk infektion, der kræver systemisk terapi.
Kendt homozygot DPD (dihydropyrimidin dehydrogenase) mangel.
Graviditet eller amning på randomiseringstidspunktet.
