この観察研究の目的は、コロラド州デンバーとボルダーに住む、産後1か月の母子二人組における母親と乳児の間でのマイクロ・ナノプラスチック(MNP)曝露を特徴づけ、評価することである。 回答することを目的とした主な質問は次のとおりです。

提案されたパイロット研究では、コロラド州デンバー都市部 (n = 10) とボルダー市 (n = 10) の健康な授乳中の女性からなる新しいコホートを確立します。 これら 2 つの地域から女性を採用することで、研究者らは、大気汚染への曝露や健康の社会的決定要因 (SDoH) のレベルが異なる多様な地理的範囲をパイロット研究が確実にカバーできるようになります。 研究参加者は産後 1 か月 (+/- 2 週間) で、完全母乳育児を報告します。 研究者らは、腸内微生物叢と糞便メタボロームをさらに検査する将来の資金申請に備えて、母体の便サンプルをバイオバンク化する予定だ。
身長、体重、血圧、その他の人体測定値: 母体の身長と体重は、校正済みの医療用体重計と市販のスタディオメーターを使用して測定されます。 乳児の体重は、10 g 単位の精度のデジタルスケールで 2 回測定され、身長は幼児計と世界保健機関 (WHO) の標準化されたアプローチを使用して、0.1 cm 単位で 2 回測定されます。 乳児の皮下脂肪の厚さは、ホルテインの皮下脂肪キャリパーを使用して 4 つの部位で測定されます。 母体の体重は 0.1 kg 単位で記録され、身長は 0.1 cm 単位で記録されます。 米国心臓協会の推奨に従って、適切なカフを使用した自動血圧計を使用して 3 回の血圧測定値が取得され、そこから平均値が記録されます。
食物摂取と身体活動の評価: 食事データは、国立がん研究所 (NCI) の国民健康栄養検査調査 (NHANES) の食事頻度アンケートを使用して収集されます。 このアンケートは先月の食物摂取に関する情報を収集するように設計されており、果物、野菜、乳製品、全粒穀物と繊維、砂糖、赤身肉と加工肉の摂取量を概説する質問が含まれています。 アンケートはウェブ上で実施され、近くに研究アシスタントが質問に答えます。 研究者らが食品からのマイクロプラスチックへの曝露に関心があることを考慮すると、研究用栄養データシステム(NDSR)の24時間食事リコールの管理を通じて追加の食事データが収集されます。これは、管理栄養士または研究スタッフが電話で管理します。管理栄養士の直接監修。 研究者らは母親に対し、理想的には平日と週末の 2 回にわたって、母親自身について 3 回、乳児について 3 回の食事のリコールを完了するよう依頼します。 母親と乳児の両方に対する 1 回の食事リコールは、臨床トランスレーショナル研究センター (CTRC) での臨床訪問中に完了します。 研究助手は参加者と協力して、その後のリコールを完了するのに最適な日を決定します。 最後に、1 回の授乳中に乳児が消費する牛乳の量を評価するために、研究者は調査訪問時に授乳前に乳児の体重を量ります。 その後、母親は乳児に母乳を与えるよう求められます。 授乳が完了すると、研究者は乳児の体重を再測定して、消費したミルクの量を大まかに把握します。
母子の糞便サンプル: 資格のある参加者には糞便採取キットが与えられます。 プラスチック汚染を避けるために、母親の糞便収集キットには、糞便収集帽子、手袋、金属スプーン、ガラス容器上のプラスチックを覆う吸収性パッドが含まれています。 乳児の便の収集では、母親にはおむつ、ミツバチのラップと布、およびプラスチックを含まないすべての収集用品が提供されます。 研究者は、参加者が自宅でこれらのサンプルを収集できるように、訪問前にこれらの収集用品と説明書を参加者に渡します。 研究者らは、便サンプルの微生物組成を保存するために、訪問時間から 24 時間以内にサンプルを採取するよう依頼します。 指示には、サンプルは訪問するまで冷蔵庫に保管する必要があると指定されています。 研究者は参加者がサンプルをCTRCに輸送するためのクーラーを提供します。 何らかの理由で参加者がこれらのサンプルを自宅で入手できなかった場合、訪問中に収集することが許可されます。臨床訪問時に収集された場合、母子の便サンプルは3〜4本のガラスバイアルに移されて保管されます。 -4℃の冷蔵庫で24時間保管した後、MNPおよび腸内マイクロバイオームのシーケンスのために-80℃で保管しました。 採便チューブには便防腐剤が使用されており、輸送中に起こる可能性のある好気性細菌と嫌気性細菌の変化を防ぎます。 糞便の配列決定はエモリー大学で実施されます(Dr. ダグ・ウォーカー)。 匿名化されたサンプルのみがウォーカー博士の研究室に送られます。 DNA は Illumina HiSeq 4000 プラットフォームで配列決定されます。 微生物の組成プロファイルは、MetaPhlan を使用して予測されます。 サンプルは、HUMAnN2 を使用して機能的にマッピングされ、UniProt 参照クラスターから遺伝子ファミリーの存在量レベルが取得され、さらに MetaCyc 代謝経路データベースにマッピングされます。 遺伝子は、EggNOG データからオルソロガス グループのクラスターにグループ化されます。 遺伝子ファミリーの存在量は、UniProt 遺伝子オントロジーのアノテーションに基づいて、より広範な機能カテゴリにグループ化されます。 メタボロミクス分析では、Metabolon による超高性能液体クロマトグラフィー (UPLC) および高分解能/タンデム質量分析 (MS/MS) を使用して糞便サンプルが分析されます。 外因性、ヒト、および微生物起源の化合物は、精製された標準または再発する未知の物質のライブラリーエントリーと比較することによって同定されます。 Metabolon は、4,500 を超える既知の代謝産物と 9,000 を超える新規代謝産物の参照ライブラリを維持しています。 生化学的同定は、保持指数、ライブラリーとの正確な質量の一致 +/- 10 ppm、実験データと本物の標準の間の MS/MS フォワードおよびリバース スコアの 3 つの基準に基づいて行われます。
母乳搾り:プラスチックボトルから母乳を与えることによる潜在的な暴露のため、すべての母親は、乳児の便を採取する前の7日間、乳児に乳房からのみ授乳するよう求められます。 便採取用品と同様に、研究者は母親に Haakaa シリコン搾乳器、キャップ付きシリコン保存容器、および説明書を訪問前に提供します。 母親は、片方の乳房を完全に表現するよう求められます。 母親が完全な表現を提供することに抵抗がある場合、研究コーディネーターは母親と協力して収集に適切な量を決定します。 研究者は少なくとも約 4g の母乳を採取します。 母親が訪問前に母乳サンプルを収集できない場合は、静かなプライベート CTRC 研究室が提供され、そこで Haakaa シリコン搾乳器を使用してシリコン製保存容器に母乳を搾乳します。 収集後、研究チームのメンバーが測定値をガラス滅菌チューブに小分けして保管します。 収集後、すべてのサンプルは処理と分析の前に -80 ℃で保管されます。
採血: マイクロプラスチック分析のために採血 (50 mL) が収集されます。 血液は、真空下でガラス容器に接続されたステンレス鋼 21G 針を使用して収集されるため、参加者から直接採取された血液はガラス製バキュテナーに沈着します。 ガラス製バキュテナーには保管用のゴム製シールが付いています。
