Ontwikkelingseffecten van blootstelling aan microPLastics in het vroege leven
Het doel van deze observationele studie is het karakteriseren en evalueren van de blootstelling aan micro- en nanoplastic (MNP) onder moeders en baby's in moeder-kind-duades, een maand postpartum, wonend in Denver en Boulder, Colorado. De belangrijkste vragen die het wil beantwoorden zijn:
- Welke MNP's zijn aanwezig in moedermelk- en bloedmonsters van de moeder en in het ontlastingsmonster van hun baby?
- Zijn er verbanden tussen de hoeveelheid MNP's van de moeder in de moedermelk en de massa MNP-deeltjes in de ontlasting van zuigelingen?
- Welke omgevings- en levensstijlfactoren zijn het meest voorspellend voor de MNP-last van de moeder?
- Is de blootstelling van baby's aan MNP's geassocieerd met geboortegewicht en postnatale groeitrajecten?
Deelnemers zullen:
- Vul verschillende vragenlijsten in die de medische geschiedenis, levensstijlfactoren, blootstelling aan het milieu, eetgedrag, enz. beoordelen.
- Verstrek biologische monsters, waaronder: bloed van de moeder, ontlasting en moedermelk; kinderkrukje
- Klinisch bezoek om antropometrische metingen te laten documenteren, waaronder de lengte en het gewicht van de moeder, het gewicht, de lengte en de dikte van de huidplooi van het kind

Gedetailleerde beschrijving
De voorgestelde pilotstudie zal een nieuw cohort van gezonde, zogende vrouwen uit het stedelijke Denver (n = 10) en Boulder (n = 10), Colorado, tot stand brengen. Door vrouwen uit deze twee regio's te rekruteren, kunnen de onderzoekers ervoor zorgen dat onze pilotstudie een divers geografisch bereik bestrijkt met verschillende niveaus van blootstelling aan luchtvervuiling en sociale determinanten van gezondheid (SDoH). Deelnemers aan de studie zullen 1 maand postpartum (+/- 2 weken) zijn en exclusieve borstvoeding rapporteren. De onderzoekers zullen ook ontlastingsmonsters van de moeder biobanken voor toekomstige financieringsaanvragen die bovendien het darmmicrobioom en het fecale metaboloom zullen onderzoeken.
Lengte, gewicht, bloeddruk en andere antropometrische metingen: De lengte en het gewicht van de moeder worden gemeten met behulp van een gekalibreerde medische weegschaal en een commerciële stadiometer. Het gewicht van het kind zal in tweevoud worden gemeten op een digitale weegschaal die nauwkeurig is tot op 10 g nauwkeurig en de lengte zal in tweevoud worden gemeten tot op de dichtstbijzijnde 0,1 cm met behulp van een infantometer en de gestandaardiseerde aanpak van de Wereldgezondheidsorganisatie (WHO). De dikte van de huidplooi bij baby's wordt op vier plaatsen gemeten met Holtain-huidplooimeters. Het gewicht van de moeder wordt tot op 0,1 kg nauwkeurig geregistreerd en de lengte tot op 0,1 cm nauwkeurig. Geautomatiseerde bloeddruk met behulp van geschikte manchetten zal worden gebruikt om drie bloeddrukmetingen te verkrijgen, waarvan de gemiddelde waarde zal worden geregistreerd, volgens de aanbevelingen van de American Heart Association.
Beoordeling van voedselinname en fysieke activiteit: Dieetgegevens zullen worden verzameld met behulp van de National Cancer Institute (NCI) National Health and Nutritional Examination Survey (NHANES) Food Frequency Questionnaire. De vragenlijst is bedoeld om informatie vast te leggen over de voedselinname van de afgelopen maand en bevat vragen over de consumptie van fruit, groenten, zuivelproducten, volkoren granen en vezels, suiker en rood en verwerkt vlees. De vragenlijst wordt via internet afgenomen, waarbij een onderzoeksassistent in de buurt aanwezig is om u te helpen met eventuele vragen. Omdat de onderzoekers geïnteresseerd zijn in de blootstelling aan microplastics uit voedsel, zullen aanvullende voedingsgegevens worden verzameld via de administratie van het Nutrition Data System for Research (NDSR) 24-uurs voedingsherinneringen, die via de telefoon zullen worden beheerd door een geregistreerde diëtist of onderzoekspersoneel onder de directe begeleiding van een geregistreerde diëtist. De onderzoekers zullen moeders vragen om drie voedingsherinneringen voor zichzelf en drie voor hun kind in te vullen, idealiter gedurende twee weekdagen en een weekend. Tijdens het klinische bezoek aan het Clinical Translational Research Center (CTRC) zal één voedingsherinnering voor zowel moeder als kind worden afgerond. Onderzoeksassistenten zullen samenwerken met de deelnemers om vast te stellen welke dagen voor hen het meest geschikt zijn om volgende terugroepacties uit te voeren. Ten slotte zullen de onderzoekers, om de hoeveelheid melk te beoordelen die zuigelingen tijdens een enkele voeding consumeren, de zuigelingen tijdens het onderzoeksbezoek wegen voordat ze worden gevoed. Vervolgens wordt moeders gevraagd hun kind borstvoeding te geven. Na voltooiing van de voeding zullen de onderzoekers de baby's opnieuw wegen om grofweg de hoeveelheid geconsumeerde melk vast te stellen.
Fecale monsters van moeder en kind: Er worden fecale verzamelkits verstrekt aan in aanmerking komende deelnemers. Om besmetting met plastic te voorkomen, bevatten de fecale opvangsets van de moeder een absorberend kussentje dat al het plastic op de ontlastingshoed, handschoenen, een metalen lepel en een glazen container bedekt. Voor de collectie kinderkrukjes krijgen moeders een luier, bijendoekje en -doekje en alle verzamelbenodigdheden die vrij zijn van plastic. De onderzoekers zullen deze verzamelbenodigdheden en instructies voorafgaand aan hun bezoek aan de deelnemers overhandigen, zodat zij deze monsters thuis kunnen verzamelen. De onderzoekers zullen hen vragen om de monsters binnen 24 uur na het bezoek te nemen om de microbiële samenstelling van de ontlastingsmonsters te behouden. In de instructies wordt gespecificeerd dat monsters tot hun bezoek in de koelkast moeten worden bewaard. De onderzoekers zullen een koeler ter beschikking stellen waarmee de deelnemers de monsters naar het CTRC kunnen vervoeren. Als de deelnemers om welke reden dan ook deze monsters niet thuis konden verkrijgen, mogen ze deze tijdens het bezoek verzamelen. Als ze tijdens het klinische bezoek worden verzameld, worden de ontlastingsmonsters van moeder en kind in 3-4 glazen injectieflacons overgebracht en bewaard. in een koelkast van -4°C gedurende 24 uur voordat het bij -80°C wordt bewaard voor MNP en darmmicrobioomsequencing. Fecale verzamelbuizen maken gebruik van een fecaal conserveermiddel, dat veranderingen in aerobe en anaerobe bacteriën voorkomt die tijdens verzending kunnen optreden. Fecale sequencing zal worden uitgevoerd aan de Emory University (Dr. Doug Walker). Alleen geanonimiseerde monsters worden naar het laboratorium van Dr. Walker gestuurd. DNA zal worden gesequenced op het Illumina HiSeq 4000-platform. Microbiële samenstellingsprofielen zullen voorspeld worden met behulp van MetaPhlan. Monsters zullen functioneel in kaart worden gebracht met behulp van HUMAnN2 om het overvloedsniveau van genfamilies uit de UniProt-referentieclusters te verkrijgen die verder in kaart zullen worden gebracht in de MetaCyc-database met metabolische routes. Genen zullen worden gegroepeerd in clusters van orthologe groepen uit EggNOG-gegevens. De overvloed aan genenfamilies zal worden gegroepeerd in bredere functionele categorieën op basis van annotatie voor de UniProt-genontologie. Voor de metabolomics-analyse zullen fecale monsters worden geanalyseerd met behulp van ultra-performance vloeistofchromatografie (UPLC) en hoge resolutie/tandem massaspectrometrie (MS/MS) van Metabolon. Verbindingen van exogene, menselijke en microbiële oorsprong zullen worden geïdentificeerd door vergelijking met bibliotheekgegevens van gezuiverde standaarden of terugkerende onbekende entiteiten. Metabolon onderhoudt een referentiebibliotheek van meer dan 4.500 bekende metabolieten en meer dan 9.000 nieuwe metabolieten. Biochemische identificaties zullen gebaseerd zijn op drie criteria: retentie-index, nauwkeurige massamatch met de bibliotheek +/- 10 ppm, en de MS/MS voorwaartse en achterwaartse scores tussen de experimentele gegevens en authentieke standaarden.
Afkolven van moedermelk: Vanwege mogelijke blootstelling aan het voeden van moedermelk uit plastic flessen, wordt aan alle moeders gevraagd om hun baby's gedurende zeven dagen vóór het verzamelen van de ontlasting alleen aan de borst te voeden. Net als bij de benodigdheden voor het verzamelen van ontlasting, zullen de onderzoekers moeders voorafgaand aan het onderzoeksbezoek voorzien van een Haakaa siliconen borstkolf, een siliconen bewaarcontainer met dop en instructies. Moeders wordt gevraagd een volledige expressie van één borst te geven. Als moeders het niet prettig vinden om een volledige uiting te geven, zullen de studiecoördinatoren met de moeders samenwerken om een bedrag te bepalen dat geschikt is voor de inzameling. De onderzoekers verzamelen minimaal ongeveer 4 gram moedermelk. Als moeders vóór het bezoek hun moedermelkmonster niet kunnen verzamelen, krijgen ze een rustige, besloten CTRC-studieruimte waar ze moedermelk kunnen afkolven met behulp van een Haakaa siliconen borstkolf in een siliconen bewaarcontainer. Na de verzameling zal een lid van ons onderzoeksteam de maatregelen in steriele glazen buisjes verdelen voor opslag. Na de monstername worden alle monsters vóór verwerking en analyse bij -80 °C bewaard.
Bloedafname: Er worden bloedafnames afgenomen (50 ml) voor analyse van microplastics. Bloed wordt afgenomen met behulp van een roestvrijstalen 21G-naald die onder vacuüm is aangesloten op een glazen container, zodat bloed dat rechtstreeks van de deelnemer wordt afgenomen, in de glazen vacutainer wordt gedeponeerd. Glazen vacutainers zijn voorzien van een rubberen afdichting voor opslag.

Uitsluitingscriteria:
- Fysieke, mentale of cognitieve handicap die deelname verhindert; huidige opsluiting
- Chronische aandoeningen bij moeder en/of kind (bijv. hartziekte, kanker, auto-immuunziekte, diabetes, darmdarmsyndroom/-ziekte), aandoeningen die voor het eerst tijdens de zwangerschap worden gediagnosticeerd (bijv. zwangerschapsdiabetes), neurologische aandoeningen bij moeder en/of kind (bijv. epilepsie, toevallen), psychologische aandoeningen bij moeder en/of kind (bijv. bipolaire stoornis), of neurologische/psychische aandoeningen gediagnosticeerd bij de moeder tijdens of onmiddellijk na de zwangerschap (bijv. postnatale depressie)
- Eerder gediagnosticeerd met een ernstige ziekte (bijvoorbeeld intra-uteriene groeivertraging, verstikking bij de geboorte, kanker) of eetstoornis of eerdere maagbandoperatie
- Antibioticagebruik tijdens de zwangerschap of binnen 1 maand na de bevalling, roken, drugsgebruik of alcoholmisbruik
- Woont buiten de onderzoeksregio tijdens de zwangerschap of postpartumperiode
