Denne randomiserte fase III-studien studerer hvor godt gemcitabinhydroklorid og docetaxel etterfulgt av doksorubicinhydroklorid virker sammenlignet med observasjon ved behandling av pasienter med høyrisiko uterint leiomyosarkom som tidligere er fjernet ved kirurgi. PRIMÆRE MÅL:

I. For å bestemme om total overlevelse av pasienter med uterusbegrenset høygradig leiomyosarkom er overlegen blant pasienter som er tildelt behandling med adjuvant gemcitabinhydroklorid (gemcitabin) pluss docetaksel etterfulgt av doksorubicinhydroklorid sammenlignet med pasienter som er tildelt observasjon.

SEKUNDÆRE MÅL:

I. For å bestemme om behandling med adjuvant gemcitabin pluss docetaxel etterfulgt av doksorubicin forbedrer residivfri overlevelse av pasienter med uterusbegrenset høygradig leiomyosarkom sammenlignet med observasjon.

II. For å utforske virkningen av potensielle prediktorer for tilbakefall eller død, som pasientens alder, rapporterte institusjonen tumorstørrelse, livmorhalsinvolvering (ja eller nei) og mitotisk rate.

OVERSIKT: Pasienter er randomisert til 1 av 2 behandlingsarmer.

Arm I: Pasienter får adjuvant gemcitabinhydroklorid IV over 70-90 minutter på dag 1 og 8 og docetaxel IV over 30-60 minutter på dag 8. Pasienter får også filgrastim subkutant (SC) på dag 9-15 eller pegfilgrastim SC på dag 9 eller 10. Behandlingen gjentas hver 21. dag i opptil 4 kurer i fravær av sykdomsprogresjon eller uakseptabel toksisitet. Pasientene gjennomgår deretter computertomografi (CT) og/eller magnetisk resonanstomografi (MRI). Pasienter uten tegn på sykdom får doksorubicinhydroklorid IV hver 21. dag i 4 kurer i fravær av sykdomsprogresjon eller uakseptabel toksisitet. Pasienter får også filgrastim SC på dag 2-8 eller pegfilgrastim SC på dag 2 eller 3.

Arm II: Pasienter gjennomgår klinisk observasjon.

Etter avsluttet studiebehandling følges pasienter i begge armer opp hver 4. måned i 3 år og deretter hver 6. måned i 2 år.

Inklusjonskriterier:

  • Pasienter med høyrisiko uterin leiomyosarkom (LMS), Federation of Gynecology and Obstetrics (FIGO) stadium I (begrenset til corpus +/- cervix); pasienter med kjent uterin serosa involvering er ikke kvalifisert; pasienter bør ha hatt, i det minste, en fullstendig hysterektomi (inkludert fjerning av livmorhalsen); bilateral salpingo-ooforektomi er ikke nødvendig

EKSPERIMENTELL: Arm I (kjemoterapi)
Pasienter får adjuvant gemcitabinhydroklorid IV over 70-90 minutter på dag 1 og 8 og docetaxel IV over 30-60 minutter på dag 8. Pasienter får også filgrastim SC på dag 9-15 eller pegfilgrastim SC på dag 9 eller 10. Behandlingen gjentas hver 21. dag i opptil 4 kurer i fravær av sykdomsprogresjon eller uakseptabel toksisitet. Pasientene gjennomgår deretter CT og/eller MR. Pasienter uten tegn på sykdom får doksorubicinhydroklorid IV hver 21. dag i 4 kurer i fravær av sykdomsprogresjon eller uakseptabel toksisitet.
