En randomisert fase III postoperativ studie av platinabasert kjemoterapi vs. capecitabin hos pasienter med gjenværende trippel-negativ basallignende brystkreft etter neoadjuvant kjemoterapi

Denne randomiserte fase III-studien studerer hvor godt cisplatin eller karboplatin (platinabasert kjemoterapi) virker sammenlignet med capecitabin ved behandling av pasienter med gjenværende (resterende) basallignende trippelnegativ brystkreft etter kjemoterapi etter kirurgi (neoadjuvant). Legemidler som brukes i kjemoterapi, som cisplatin, karboplatin og capecitabin, virker på forskjellige måter for å stoppe veksten av tumorceller, enten ved å drepe cellene, ved å stoppe dem fra å dele seg eller ved å hindre dem i å spre seg. PRIMÆRE MÅL:

I. Å sammenligne invasiv sykdomsfri overlevelse (IDFS) hos trippel-negativ brystkreft (TNBC) pasienter med gjenværende basallignende sykdom etter neoadjuvant kjemoterapi som er randomisert til post-preoperativ platinabasert kjemoterapi med de som er randomisert til capecitabin.

SEKUNDÆRE MÅL:

I. For å evaluere total overlevelse (OS) og responsfri overlevelse (RFS) i de to armene hos pasienter med TNBC med gjenværende basallignende sykdom etter neoadjuvant kjemoterapi.

II. For å karakterisere bivirkninger og tolerabilitet av platinamiddel samt kapecitabin hos pasienter med TNBC med gjenværende sykdom etter neoadjuvant kjemoterapi.

III. For å identifisere frekvensen av basallignende genuttrykk ved bruk av prediksjonsanalyse av mikroarray 50 (PAM50) analyse ved digital messenger ribonukleinsyre (mRNA) kvantifisering blant medikamentresistente gjenværende TNBC etter neoadjuvant kjemoterapi.

IV. For å vurdere forskjellen i helserelatert livskvalitet (HRQL) mellom den platinabaserte og capecitabin-kjemoterapiarmen.

UNDERSØKENDE MÅL:

I. Å sammenligne IDFS hos TNBC-pasienter med gjenværende ikke-basallignende sykdom etter neoadjuvant kjemoterapi som er randomisert til post-preoperativ platinabasert kjemoterapi med de som er randomisert til capecitabin.

II. For å beskrive frekvensen av nevrotoksisitet over tid i platinaarmen, frekvensen av medisinoverholdelse i capecitabin-armen og frekvensen av amenoré i begge armer.

VIII. For å utforske betydningen av CTC-nummer/fenotype og ctDNA-detekterte mutasjoner (mutasjonsbelastning, spesifikke mutasjoner) for å forutsi RFS.

IX. For å bestemme frekvensen av plasmatumorcellefritt DNA (cfDNA) positivitet ved baseline og etter fullføring av studieterapi hos pasienter med TNBC med gjenværende basal subtype sykdom etter neoadjuvant kjemoterapi.

X. For å evaluere assosiasjonene mellom plasmatumor cfDNA tumorspesifikke mutasjoner (baseline og etter terapi) med RFS.

XI. For å utforske optimal biomarkørkombinasjon for RFS-prediksjon. XII. For å bestemme effekten av tobakk, operasjonalisert som brennbar tobakk (1a), andre former for tobakk (1b), og miljøeksponering av tobakk (ETS) (1c) på leverandørrapportert toksisitet for kreftbehandling (uønskede hendelser (både kliniske og hematologiske) og doseendringer).

XIII. For å bestemme effekten av tobakk på pasientrapporterte fysiske symptomer og psykologiske symptomer.

XV. Å utforske effekten av tobakksbruk og eksponering på behandlingsvarighet, relativ doseintensitet og terapeutisk nytte.

OVERSIKT: Pasienter er randomisert til 1 av 3 behandlingsarmer.

ARM A (stengt for opptjening 16.05.2016): Pasienter gjennomgår observasjon.

ARM B: Pasienter får cisplatin intravenøst (IV) eller karboplatin IV på dag 1. Behandlingen gjentas hver 3. uke i 4 kurer i fravær av sykdomsprogresjon eller uakseptabel toksisitet.

ARM C: Pasienter får capecitabin oralt (PO) to ganger daglig (BID) på dag 1-14. Behandlingen gjentas hver 3. uke i opptil 6 kurer i fravær av sykdomsprogresjon eller uakseptabel toksisitet.

Etter avsluttet studiebehandling følges pasientene opp i inntil 10 år.

KVALIFIKASJONSKRITERIER FOR SCREENING OG MOLEKYLÆR PROFILERING (TRINN 0)

Inklusjonskriterier:

Eastern Cooperative Oncology Group (ECOG) ytelsesstatus 0 eller 1 innen 2 uker før screening
Kvinnelige og mannlige pasienter må ha histologisk bekreftet invasiv brystkreft som oppfyller følgende kriterier:
- Klinisk stadium II-III (American Joint Committee on Cancer [AJCC] 7. utgave) ved diagnose, basert på innledende evaluering ved klinisk undersøkelse og/eller brystavbildning; ingen metastatisk sykdom tillatt
- ER- og PR- bør oppfylle ett av følgende kriterier:
  - =< 10 % celler farges positive, med svak intensitetsscore (tilsvarer Allred-score =< 3)
  - =< 1 % celler farges positive, med svak eller middels intensitetsscore (tilsvarer Allred-score =< 3)
- HER2-negativ (ikke kvalifisert for anti-HER2-behandling) vil bli definert som:
  - Immunhistokjemi (IHC) 0, 1+ uten in situ hybridisering (ISH) HER2/neu kromosom 17-forhold ELLER
  - IHC 2+ og ISH HER2/neu kromosom 17 ratio ikke-amplifisert med ratio mindre enn 2,0 og hvis rapportert gjennomsnittlig HER2 kopiantall < 6 signaler/celler ELLER

  - MERK: Pasienter som har et avvik i ER/PR/HER2-status mellom opprinnelig diagnose og kirurgisk prøve (hvis ER/PR/HER2-status ble gjentatt) er ikke kvalifisert for studiedeltakelse (dvs. ER/PR/HER2 må oppfylle kriteriene ovenfor i begge scenariene)
Pasienter må ha fullført neoadjuvant taxan +/- antracyklin; Pasienter må IKKE ha fått cisplatin eller karboplatin eller capecitabin som en del av deres neoadjuvante behandlingsregime
- MERK: Pasienter som mottok preoperativ terapi som en del av en klinisk studie kan meldes på
- MERK: Pasienter som av en eller annen grunn ikke var i stand til å fullføre den planlagte neoadjuvante kjemoterapien (dvs. toksisiteter, etc.) er kvalifisert til å delta så lenge ingen ytterligere systemisk standardbehandling er planlagt av den behandlende legen
Må ha fullført definitiv reseksjon av primærtumor
- Negative marginer for både invasivt og duktalt karsinom in situ (DCIS) er ønskelig, men pasienter med positive marginer kan registreres dersom behandlingsteamet mener at ingen ytterligere operasjon er mulig og pasienten har mottatt strålebehandling; Pasienter med marginer positive for lobulært karsinom in situ (LCIS) er kvalifisert
- Enten mastektomi eller brystbevarende kirurgi (inkludert lumpektomi eller delvis mastektomi) er akseptabelt
- Sentinel node biopsi enten før eller post neoadjuvant kjemoterapi (dvs. på tidspunktet for definitiv kirurgi) er tillatt; aksillær disseksjon oppmuntres hos pasienter med lymfeknutepåvirkning, men er ikke obligatorisk
Etter neoadjuvant kjemoterapi må pasienter påvises å ha gjenværende invasiv kreft i brystet på tidspunktet for definitiv kirurgi; gjenværende kreft er definert som et sammenhengende fokus for gjenværende invasiv kreft, i brystet, som måler >= 1 cm i diameter, og med mer enn minimal cellularitet, i henhold til lokal patologbestemmelse; dette er nødvendig på grunn av begrensninger i ekstraksjon av deoksyribonukleinsyre (DNA) for PAM50-analyse
- MERK: Tilstedeværelsen av duktalt karsinom in situ (DCIS) uten invasjon kvalifiserer ikke som gjenværende invasiv sykdom i brystet
- MERK: Til tross for lymfeknuteinvolvering hvis gjenværende invasiv kreft i brystet er < 1 cm i diameter, er ikke pasienter kvalifisert for deltakelse
Strålebehandling kan gis før eller etter protokollbehandling i henhold til standard retningslinjer for omsorg; når strålebehandling er planlagt før protokollbehandlingsadministrasjon, kan pasienter registreres og screenes mens de mottar stråling
- Strålebehandling etter mastektomi er nødvendig for alle pasienter med følgende:
  - Primærsvulst >= 5 cm (før neoadjuvant kjemoterapi [klinisk] eller på tidspunktet for definitiv kirurgi) eller involvering av 4 eller flere lymfeknuter på tidspunktet for definitiv kirurgi
  - For pasienter med primære svulster < 5 cm eller med < 4 involverte lymfeknuter før neoadjuvant kjemoterapi og på tidspunktet for endelig kirurgi, er strålebehandling etter mastektomi etter den behandlende legens skjønn
  - Stråling av regionale nodalbassenger er etter den behandlende stråleonkologen
- MERK: Bryststrålebehandling (helt bryst eller delvis) er nødvendig for pasienter som gjennomgikk brystbevarende behandling, inkludert lumpektomi eller delvis mastektomi
