Optimalisering av endokrin terapi gjennom motiverende intervjuer og tekstintervensjoner

Denne fase III-studien sammenligner et ekstra støtteprogram (påminnelser om tekstmeldinger og/eller telefonbasert rådgivning) med vanlig forsiktighet for å sikre at brystkreftpasienter tar sin endokrine terapimedisin som foreskrevet (medikamentoverholdelse). Medikamentoverholdelse er hvor godt pasienter tar medisinene som foreskrevet av legene, og god medisinsk etterlevelse er når pasienter tar medisiner riktig. Dårlig medisinoverholdelse har vist seg å være en alvorlig barriere for effektiv behandling for hormonreseptorpositive brystkreftpasienter. Ved å legge til påminnelser om tekstmeldinger og/eller telefonbasert rådgivning til vanlig behandling, kan det øke antall dager pasienter tar sin endokrine terapimedisin som foreskrevet.

PRIMÆRE MÅL:

I. Sammenlign etterlevelse av endokrin terapi (ET) etter 12 måneder hos forskjellige kvinner utsatt for tekstmeldingspåminnelser (TMR)-bare, telefonbasert motiverende intervjurådgivning (MI)-kun eller begge deler (TMR+MI), versus vanlig omsorg.

SEKUNDÆRE MÅL:

l. Sammenlign etterlevelse av endokrin terapi (ET) etter 24 måneder hos forskjellige kvinner utsatt for tekstmeldingspåminnelser (TMR)-bare, telefonbasert motiverende intervjurådgivning (MI)-kun eller begge deler (TMR+MI), versus vanlig omsorg.

OVERSIKT: Pasienter randomiseres til 1 av 4 armer.

ARM I (TMR): Pasienter mottar online pedagogisk informasjon om ET ved starten av ET-medisineringen. Pasienter mottar også daglige SMS-påminnelser om å ta ET-medisinen og månedlige tekstmeldinger om hvordan de har det med å ta ET-medisinen. Disse tekstmeldingene fortsetter i 9 måneder.

ARM II (MI): Pasienter mottar online pedagogisk informasjon om ET ved starten av ET-medisineringen. Pasientene mottar også totalt 5 motiverende samtaleveiledningssamtaler via telefon over 30-90 minutter i inntil 9 måneder. Disse øktene er utformet for å støtte pasienter mens de tar ET-medisiner, utvikle helsemål og holde seg på rett spor i å nå disse målene.

ARM III (TMR + MI): Pasienter mottar online pedagogisk informasjon om ET ved starten av ET-medisineringen. Pasienter mottar også tekstmeldinger som i arm I og veiledningssamtaler med motiverende intervjuer som i arm II.

ARM IV (ENHANCED USUAL CARE): Pasienter går på vanlige klinikkbesøk hver 3.-6. måned og mottar online pedagogisk informasjon om ET ved starten av ET-medisineringen. Pasienter får også valgfri informasjon på nett om å leve et sunt liv etter brystkreft.

Etter avsluttet studiedeltakelse følges pasientene opp i inntil 24 måneder.

  • Kvinner med en første patologisk bekreftet diagnose av stadium I-III, hormonreseptorpositiv, HER2-neu negativ, invasiv brystkreft innen 18 måneder før innmelding

    • Kvinner som har gjennomgått neo-adjuvant kjemoterapi som ikke har gjenværende invasiv sykdom etter operasjonen er kvalifisert basert på en første patologisk bekreftet diagnose
    • Hormonreseptorpositiv er definert som østrogenreseptor (ER) og/eller progesteronreseptor (PR) på > 1 %
    • HER2-neu negativ er definert som 0-1+ ved immunhistokjemisk (IHC) analyse, eller ikke-amplifisert ved fluorescens in situ hybridisering (FISH) analyse
  • Pasienter må ha mottatt kreftrettet kirurgi og/eller fullført all annen adjuvant behandling, unntatt rekonstruksjon
  • Pasienter må ha påbegynt et hormonbehandlingslegemiddel innen 6 måneder før registrering, ELLER ha mottatt resept med uttalt intensjon om å starte innen 6 uker etter registrering

  • Ingen historie med tidligere kreft som følger:

    • Invasiv eller ikke-invasiv brystkreft når som helst
    • Ikke-brystkreft, innen de siste 5 årene, unntatt ikke-melanom hudkreft

  • Pasienter må være villige til å bruke smarttelefon til studieaktiviteter

    • Pasienten skal IKKE anses som uegnet under rekrutteringsprosessen hvis de ikke har en smarttelefon
    • Hvis en deltaker ikke eier en smarttelefon eller har begrensede data- eller tekstfunksjoner eller smarttelefonen deres ikke kan støtte Alliance elektroniske pasientrapporterte utfall (ePRO) undersøkelsesapplikasjonen (appen), kan en smarttelefon og tjeneste tilbys deltakeren uten at kostnad gjennom Ohio State University (OSU) partnerskap med Verizon Wireless for varigheten av studieaktivitetene
    • CRP er BARE for å diskutere dette alternativet med de pasientene som selv identifiserer en telefonrelatert barriere for deltakelse, inkludert: mangel på smarttelefon, utilstrekkelig telefonplan (minutter/tekst/data) eller en smarttelefon som er uforenlig med alliansen ePRO app
    • For OSU-leverte telefoner vil gebyrer bli betalt av tilskuddet gjennom intervensjonsperioden. Ved utløpet av 12-måneders intervensjonsperiode vil pasientene være ansvarlige for å betale månedlige avgifter, dersom fortsatt tjeneste ønskes. Pasienter mottar online pedagogisk informasjon om ET ved starten av ET-medisineringen. Pasienter mottar også daglige SMS-påminnelser om å ta ET-medisinen og månedlige tekstmeldinger om hvordan de har det med å ta ET-medisinen. Disse tekstmeldingene fortsetter i 9 måneder.

Pasienter mottar online pedagogisk informasjon om ET ved starten av ET-medisineringen. Pasienter mottar også totalt 5 motiverende samtaleveiledningssamtaler via telefon over 30-90 minutter i inntil 9 måneder. Disse øktene er utformet for å støtte pasienter mens de tar ET-medisiner, utvikle helsemål og holde seg på rett spor i å nå disse målene.
