气道炎症在许多气道疾病中起着关键作用。 研究这些炎症过程和监测气道疾病的非侵入性方法的需求量很大。 刺激兴趣的是呼吸分析,例如呼出气冷凝物 (EBC) 的分析,这是一种对肺上皮衬里液 (ELF) 进行采样的技术,作为评估气道炎症的更简单方法 (1,2)。 EBC 的吸引力在于它能够无创地从呼吸道收集范围广泛的非挥发性分子,事实上它可以在很短的间隔内频繁重复而不会发生不良事件,而且收集设备可以用于广泛的范围设置包括重症监护病房 (3,4,5,6)。
EBC 的分析不能仅限于患者监测和了解肺部疾病的机制。 它还可以成为监测和筛查健康个体可能出现的早期肺组织损伤的有用工具 (6,7)。 特别需要更多关于受试者内部和日常变化的数据,这对于决定生物标志物是否可以作为研究工具甚至是否具有在临床实践中监测疾病的潜力至关重要 (7) .
人们已经认识到机械通气可能会导致肺损伤和炎症 (8,9,10)。 最近的实验和临床数据表明,在健康肺部,在没有呼气末正压的情况下,潮气量在 7 至 12 ml/kg 之间的机械通气可能会导致内皮细胞、细胞外基质和外周气道损伤,而不会产生严重的炎症反应,这随着潮气量的增加进一步恶化 (15,34)。 有几种机制可以解释机械通气引起的肺结构损伤:局部过度扩张、与潮气道关闭相关的“低肺容量”和表面活性剂失活 (15)。
高潮气量通气已被证明会导致死亡率增加,而低潮气量通气被认为是 ALI、ARDS 的肺保护策略 (11,12,13,35)。
相比之下,在其他随机研究 (31,32) 中,包括一组异质的主要胸腹部外科手术,保护性机械通气与肺内和全身炎症的减少无关。 此外,没有证据表明保护性通气可预防心脏手术中的肺部不良反应或降低全身细胞因子水平 (33)。
根据这些观察结果,并考虑到肺部机械应力增加的实际参数仍有待证明,研究可能会解决 EBC 生物标志物的分析是否与呼吸机引起的低潮气量或高潮气量肺损伤相关的问题。
材料和方法:
本研究是一项前瞻性、随机、对照试验,将在色萨利拉里萨大学医院的 ICU 进行。 已获得我院科学委员会和伦理委员会的授权。
患者:
因中风、蛛网膜下腔和/或脑出血而需要机械通气且呼吸系统健康的 ICU 患者(使用 LISS - Murray 肺损伤严重程度评分标准进行评估)(14)。
干预措施:
根据 ATS/ERS 工作组 2005 (7),将通过气管插管对机械通气患者进行 EBC 收集。 患者的血流动力学和呼吸必须稳定。
所有患者都将处于镇静状态,并通过容量控制接受通气。 呼吸频率将根据设定的潮气量进行调整,以将 pH 值维持在正常范围内 (7.35-7.45)。 SaO2 将保持等于或高于 95%。
EBC 将通过插入特殊导管(FILT,lung and chest diagnostics Ltd. 德国柏林)将呼吸冷凝水收集装置(Ecoscreen,Jaeger,德国)放入呼吸机管路的呼气肢。 EBC 的收集时间为 30 分钟。 采集期间不使用加湿器。
使用惰性气体氩脱气前后 EBC 的酸度 (pH),350 毫升/分钟,持续 10 分钟,(气体标准化)(17) 可以使用 pH 计 Jenway Model 3510 轻松测量。
所有样品将储存在 -80 ο C 以用于后续的介体测量。 在 EBC 收集之前和期间,将记录通气变量(频率、PEEP、FIO2、Vt)、肺力学、动脉压、心率、动脉血饱和度、ICP 和气血样本检查。 还将记录观察期间的肺损伤指标(PiO2/FiO2、LISS)、全身炎症指标(温度、血样中的白细胞和中性粒细胞计数)。 疾病严重程度指数(SOFA、SAPS、APACHE II)将在初始评估期间登记。
EBC分析:
收集的 EBC 将分析 pH、8-异前列烷、H2O2、亚硝酸盐/硝酸盐和细胞因子。 如前所述,将使用标准化程序进行生物标志物的测量。
pH 测量:pH 将如前所述进行测量 (16,17)。 H2O2 测量:H2O2 浓度将通过使用辣根过氧化物酶(Sigma Chemicals,St. Louis,MO)的酶法测定来确定,如前所述 (17,18,19,20)。
8-异前列烷测量:8-异前列烷将通过竞争性酶免疫测定试剂盒(Cayman Chemical, Ann Arbor, MI)测定,如前所述 (17,18,21,29)。 该测定的检测限为 4 pg/ml。
氮氧化物、亚硝酸盐/硝酸盐 (NO2/NO3) 和相关产物的测量:将如前所述进行 (17,22)。 简而言之,将通过使用分光光度测定法(Griess 反应)来确定 (23,24,25,26,27,28)。

协议详情:
初步评估后的患者将随机接受机械通气,理想体重为 6 或 12 ml/Kg,计算公式如下:
男士 [(身高 (cm)-154) x 0.9] +50 女士 [(身高 (cm)-154) x 0.9] +45.5。 观察期至少为 10 天(如果可能),EBC 收集将在入院后的前 24 小时内(第 0 天)和第 1、2、4、6、8、10 天进行。
第 0 天的 EBC 收集将在两种通气方式下进行,目的是调查从同一患者以不同潮气量通气时收集的 EBC 的数量和组成。 对于接下来的测量,每组的 EBC 收集将在预设的通风条件下进行。
将记录观察期间的并发症,例如 VAP、ARDS 或败血症。
统计分析:
将使用 SPSS for Windows v. 16.0 进行分析。 将使用 Kolmogorov-Smirnov 检验检查分布的正态性。 正态分布的数据将以平均值±标准偏差 (SD) 的形式呈现,而偏斜的数据将呈现为中位数(四分位间距)。 两组之间的比较将通过非配对 t 检验来评估正态分布和 Mann-Whitney 检验来评估偏斜数据。 多于两组的比较将通过方差分析 (ANOVA) 和适当的事后检验进行
