STAMP: Chirurgicky léčený adjuvantní karcinom z Merkelových buněk s pembrolizumabem, studie fáze III

Tato studie fáze III studuje, jak dobře pembrolizumab funguje ve srovnání s pozorováním standardní péče při léčbě pacientů s rakovinou z Merkelových buněk stadia I-III, která byla zcela odstraněna chirurgicky (resekována). Imunoterapie s monoklonálními protilátkami, jako je pembrolizumab, může pomoci imunitnímu systému těla napadnout rakovinu a může narušit schopnost nádorových buněk růst a šířit se.

PRVNÍ CÍL:

I. Porovnat přežití bez recidivy (RFS) a celkové přežití (OS) jako primární cílové parametry v obou větvích.

DRUHÉ CÍLE:

I. Vyhodnotit nežádoucí účinky. II. Vyhodnotit přežití bez vzdálených metastáz (DMFS). III. Vyhodnotit vliv záření na klinické výsledky (OS, RFS, DMFS).

Přehled: Pacienti jsou randomizováni do 1 ze 2 ramen.

ARMA: Pacienti dostávají pembrolizumab intravenózně (IV) po dobu 30 minut v den 1. Léčba se opakuje každých 21 dní po dobu až 17 cyklů v nepřítomnosti progrese onemocnění nebo nepřijatelné toxicity. Pacienti mohou také podstoupit standardní radiační terapii do 14 dnů ode dne 1, cyklu 1. Pacienti podstupují pozitronovou emisní tomografii (PET) – počítačovou tomografii (CT) nebo CT vyšetření každých 6 měsíců po dobu 5 let.

ARM B: Pacienti dostávají standardní pozorování každé 3 měsíce po dobu 1 roku a poté každých 6 měsíců po dobu 5 let. Pacienti mohou také podstoupit standardní radiační terapii do 14 dnů ode dne 1, cyklu 1. Pacienti podstupují PET-CT nebo CT každých 6 měsíců po dobu 5 let.

Po ukončení studijní léčby jsou pacienti sledováni každých 6 měsíců po dobu 5 let.

Kritéria pro zařazení:

  • Pacient musí být starší 18 let
  • Pacienti musí mít výkonnostní stav Eastern Cooperative Oncology Group (ECOG): 0, 1 nebo 2 (Nicméně ti pacienti s výkonnostním stavem 3, protože jsou upoutáni na invalidní vozík v důsledku vrozených nebo traumatických událostí více než jeden rok před diagnózou karcinomu z Merkelových buněk jsou způsobilé).

  • Pacient musí mít histologicky potvrzenou diagnózu karcinomu z Merkelových buněk (MCC), patologická stadia (American Joint Committee on Cancer [AJCC] verze 8) I-IIIb.

    • Pacienti ve stadiu I s negativní biopsií sentinelové lymfatické uzliny nejsou vhodní. Vhodné jsou pacienti, kteří mají pozitivní biopsii nebo u kterých nebyla provedena žádná biopsie.
    • Pacienti se vzdáleným metastatickým onemocněním (stadium IV) nejsou vhodní.
    • Primární nádor musí mít výrazně negativní okraje. (Mikroskopicky pozitivní okraje jsou povoleny).
    • Nádory neznámého primárního typu, které mají pouze regionální onemocnění, jsou způsobilé.
    • Pro způsobilost není vyžadována úplná disekce uzlin.
  • Pacienti s makroskopickým karcinomem z Merkelových buněk (buď identifikovaným fyzikálním vyšetřením nebo zobrazením) byli kompletně resekováni chirurgicky během 16 týdnů před randomizací.
  • Všichni pacienti musí mít stav bez onemocnění doložený kompletním fyzikálním vyšetřením a konvenčními zobrazovacími studiemi do 8 týdnů před randomizací.

  • Pacient nemusí mít v anamnéze vzdálené metastatické onemocnění.

    • POZNÁMKA: Lokoregionální recidivující onemocnění je přijatelné, pokud není metastatické (předchozí operace s radiační terapií nebo bez ní je přijatelná).
  • U pacientů s počáteční prezentací karcinomu z Merkelových buněk nesmí pacient podstoupit žádnou předchozí systémovou terapii nebo radioterapii před operací karcinomu z Merkelových buněk a nemůže absolvovat adjuvantní radiační terapii pro karcinom z Merkelových buněk více než 6 týdnů před randomizací. Pacienti, kteří aktivně podstupují radiační terapii nebo dokončili adjuvantní radiační terapii do 6 týdnů od randomizace, jsou způsobilí, pokud je datum resekce do 16 týdnů od randomizace. Do studie jsou způsobilí pacienti s předchozím ozařováním u poskytovatele jiného než radiačního onkologického centra (IROC). Pokud pacient nepodstoupil ozařování a léčba u poskytovatele radiační onkologie (IROC) není možná, může pacient zahájit a dokončit ozařování před randomizací s doporučeními, aby se řídil pokyny protokolu o ozařování s předložením záznamů o léčbě.
  • Počet bílých krvinek >= 2000/ul (během 4 týdnů před randomizací).
  • Absolutní počet neutrofilů (ANC) >= 1000/ul (během 4 týdnů před randomizací).
  • Počet krevních destiček >= 75 x 10^3/ul (během 4 týdnů před randomizací).
  • Hemoglobin >= 8 g/dl (>= 80 g/l; lze podat transfuzi) (během 4 týdnů před randomizací).
  • Kreatinin = < 2,0 x institucionální horní hranice normálu (ULN) (během 4 týdnů před randomizací).
  • Aspartátaminotransferáza (AST) a alaninaminotransferáza (ALT) = < 2,5 x institucionální ULN (během 4 týdnů před randomizací).
  • Celkový bilirubin = < 2,0 x ústavní ULN (kromě pacientů s Gilbertovým syndromem, kteří musí mít celkový bilirubin nižší než 3,0 mg/dl) (během 4 týdnů před randomizací).
  • Pacienti, kteří jsou virem lidské imunodeficience (HIV)+ s nedetekovatelnou virovou náloží HIV, jsou způsobilí za předpokladu, že splňují všechna ostatní kritéria protokolu pro účast.

Kritéria vyloučení:

  • Pacientka nesmí být těhotná nebo kojit kvůli neznámým účinkům studovaného léku v tomto nastavení. Všechny pacientky ve fertilním věku musí podstoupit krevní test nebo vyšetření moči do 2 týdnů před randomizací, aby se vyloučilo těhotenství. Pacient ve fertilním věku je každý, bez ohledu na sexuální orientaci nebo na to, zda prodělal podvázání vejcovodů, kdo splňuje následující kritéria: 1) v určitém okamžiku dosáhl menarche, 2) nepodstoupil hysterektomii nebo bilaterální ooforektomii; nebo 3) nebyla přirozeně postmenopauzální (amenorea po léčbě rakoviny nevylučuje možnost otěhotnění) po dobu alespoň 24 po sobě jdoucích měsíců (tj. měla menstruaci kdykoli v předchozích 24 po sobě jdoucích měsících).
  • Pacienti na rameni A MK-3475 (Pembrolizumab) nesmějí počít nebo zplodit děti pomocí uznávané a účinné metody (metod) antikoncepce nebo se zdržet pohlavního styku od okamžiku registrace během studijní léčby a pokračovat po dobu 120 dnů poté. poslední dávka studijní léčby. U pacientek v rameni B, které dostávají pouze radiační terapii, by antikoncepce měla být používána podle ústavního standardu.

Časové okno: Od randomizace až po recidivu onemocnění nebo smrt z jakékoli příčiny; hodnoceno do 5 let
Pro srovnání celkového přežití (OS) a RFS mezi oběma rameny bude provedena analýza záměrné léčby (ITT) pomocí stratifikovaného log-rank testu.

Vliv radiační terapie na RFS
Časové okno: Až 5 let
RFS v každém rameni bude hodnocena podle stavu radiační léčby (radiace versus [vs.] žádná radiační terapie). Analýza bude plánovanou post-hoc analýzou s primárním cílem prověřit, zda je použití pooperační radiační terapie spojeno s RFS. Coxovy multivariační modely pro RFS budou vyvinuty k vyhodnocení dopadu radiační terapie při úpravě na léčbu pembrolizumabem a klinicko-patologické faktory. Léčebná pole a údaje o dávce budou také zahrnuty do Coxova modelu jako kovariáty. Sekundární analýzy pro radioterapii (RT) budou zkoumat faktory spojené s používáním pooperační RT. Budou vyvinuty mnohorozměrné logistické regresní modely (radiační terapie vs. žádná radiační terapie) k vyhodnocení souvislostí s demografickými, klinickými/patologickými faktory a faktory souvisejícími s léčbou a léčbou pembrolizumabem.

Časové okno: Od randomizace ke vzdálené metastáze, hodnoceno do 5 let
DMFS bude hodnocen léčebnými rameny. Pokud neexistují žádné vzdálené metastázy, budou případy cenzurovány v době posledního hodnocení.

Výskyt nežádoucích jevů

Měřeno podle společných terminologických kritérií pro nežádoucí účinky verze 5.0. Nežádoucí účinky z každého ramene budou shrnuty a porovnány pomocí Fisherova exaktního testu.
