Tato studie fáze III studuje, jak dobře bupropion působí při snižování únavy související s rakovinou u pacientů, kteří přežili rakovinu prsu ve stádiu I-III. Bupropion je lék, který se používá k léčbě deprese a pomáhá lidem přestat kouřit. Rakovina a její léčba může způsobit únavu. PRIMÁRNÍ CÍLE:

I. Stanovit účinnost bupropion hydrochloridu s řízeným uvolňováním (bupropion) oproti placebu při snižování únavy ve dvojitě zaslepené, placebem kontrolované, randomizované klinické studii u pacientek, které přežily rakovinu prsu s únavou.

DRUHÉ CÍLE:

I. Zhodnotit účinnost bupropionu oproti placebu na depresi a kvalitu života u přeživších pacientů s rakovinou prsu s únavou.

II. Posoudit snášenlivost bupropionu u přeživších pacientů s rakovinou prsu s únavou.

PRŮZKUMNÉ CÍLE:

I. Zhodnotit účinnost bupropionu oproti placebu na symptomatologii a kognitivní funkce u přeživších pacientů s rakovinou prsu s únavou.

II. Prozkoumat účinky bupropionu na domnělé mechanismy únavy související s rakovinou.

III. Prozkoumat asociace genotypu CYP2B6 s metabolismem bupropionu a změnami únavy.

Přehled: Pacienti jsou randomizováni do 1 ze 2 ramen.

ARM I: Pacienti dostávají bupropion hydrochlorid s řízeným uvolňováním perorálně (PO) jednou denně (QD) po dobu až 13 týdnů v nepřítomnosti nepřijatelné toxicity.

ARM II: Pacienti dostávají placebo PO QD po dobu až 13 týdnů v nepřítomnosti nepřijatelné toxicity.

Kritéria pro zařazení:

  • Mít diagnózu rakoviny prsu I-III
  • Uveďte nejhorší úroveň únavy za poslední týden jako střední až těžkou (tj. skóre >= 4 na stupnici 0–10, Screeningová opatření, otázka 1)
  • Absolvovali chirurgický zákrok, ozařování a/nebo chemoterapii alespoň 2 nebo více měsíců před zařazením (účastníci mohou dostávat udržovací, cílenou nebo hormonální terapii)
  • Umět číst a mluvit anglicky
  • V současné době není těhotná ani nekojí. Ženy ve fertilním věku musí před vstupem do studie a po dobu účasti ve studii souhlasit s používáním adekvátní antikoncepce, tj. abstinence, IUD (nitroděložní tělísko), hormonální antikoncepce (antikoncepční pilulky) nebo bariérové ​​metody (kondomy).

Měřeno funkčním hodnocením podkazetu únavy chronické onemocnění (FACIT-F) únavy (FFS). Skóre měřítka se pohybuje od 0-4; Nižší skóre označují větší únavu. Bude používat analýzu kovariance se skupinou jako hlavní faktor a základní FFS jako kovariát.

Měřeno celkovým skóre terapie rakoviny funkčního hodnocení-měřítka obecné (fakt-g) (zahrnutá v FACIT-F). Fakt-G se skládá ze čtyř dílčích stupnic: fyzické pohody, funkční pohoda), emoční pohoda a sociální pohodu. Skóre na čtyřech dílčích stupnicích se sčítají tak, aby vytvořily celkové skóre v rozmezí od 0 do 108, přičemž vyšší skóre naznačuje lepší kvalitu života. To bude použito k odhadu nepřímých (mediace) a přímých účinků a k získání 95% konfindenčních intervalů založených na bootstrapu pro tyto účinky.
