Detailní popis

Primární cíl: Provést pilotní randomizovanou kontrolovanou studii s AYA s cílem určit proveditelnost a přijatelnost dokončení PRO na základě priorit AYA. AYA budou schváleny a náhodně vybrány k výběru pěti domén HRQOL z řídicího panelu (Choice PRO) v každém období hodnocení nebo pěti standardních domén HRQOL (Fixed PRO; fyzická funkce, bolest, kognitivní funkce, sociální podpora, finance) v každém období hodnocení. AYA dokončí základní, 1-, 3-, 6- a 12měsíční hodnocení, která budou zahrnovat kombinaci CATs a statické krátké formy. Proveditelnost bude zprovozněna jako ≥ 75 % dokončených PRO a ≥ 75 % přijatelnosti. Proveditelnost a přijatelnost bude porovnána mezi AYA randomizovanými do Choice PRO vs. Fixed PRO. Pro minimalizaci chybějících dat budou použity posilovače adherence (připomínkové hovory, textové zprávy) a vyhodnoceny jako moderátoři.

Průzkumný cíl: Vyžádejte si AYA preference ohledně toho, jak by měla být jejich PRO data sdílena s nimi a/nebo jejich poskytovateli. Za účelem informování o budoucím výzkumu budou AYA dotázáni, zda a jak chtějí účastníci, aby jejich data PRO využívaly jejich lékařské týmy, a také zda a jak chtějí účastníci, aby jejich data PRO byla sdílena s nimi a jejich rodinami.

Vyšetřovatelé očekávají, že PROMIS CAT zvýší kapacitu pro sběr dat PRO, AYA randomizované do Choice PRO, kteří dostanou příležitost vybrat si, které domény PRO dokončit, budou mít méně chybějících dat. Vyšetřovatelé očekávají, že většina AYA bude chtít zpětnou vazbu PRO pro sebe, své rodiny a své poskytovatele. Zjištění podpoří a udrží začlenění PRO do klinických studií a studií podpůrné péče, poskytnou AYA hlas a informují budoucí péči zaměřenou na pacienta přizpůsobenou různorodým potřebám AYA. Tím, že AYAs nabízí možnost vybrat si, které domény PRO dokončit na základě relevance pro kvalitu jejich života související se zdravím, na rozdíl od dokončení domén předem určených výzkumným týmem, vyšetřovatelé zvýšili přístup k hodnocení vyšetřovatelů zaměřený na pacienta.

Kritéria pro zařazení:

Pacientovi musí být v době registrace ≥ 18 let a ≤ 39 let. Pacient musí mít histologicky potvrzenou diagnózu primárního karcinomu jakéhokoli stadia do 12 týdnů (84 dnů) při registraci.

Pacient musí podstoupit, v současné době dostává nebo plánuje léčbu rakoviny, včetně chirurgického zákroku a/nebo chemoterapie a/nebo radiační terapie.

Pacient musí mít výkonnostní stav ECOG 0-3. Pacient musí mít očekávanou délku života > 24 měsíců. Pacient musí být schopen vyplňovat dotazníky v angličtině. Pacient musí mít přístup k internetu prostřednictvím počítače, tabletu nebo chytrého telefonu. Pacient musí mít e-mailovou adresu. Pacient musí mít mobilní telefon s možností zasílání textových zpráv. Pacient musí být schopen přesně poskytnout vlastní údaje (např. podle klinického úsudku je kognitivní funkce intaktní).

Pacient musí být schopen poskytnout informovaný souhlas.

Kritéria vyloučení:

Pacient nesmí mít recidivu nebo druhý primární karcinom. Pacienti nesmí mít bazocelulární karcinom kůže.

Will be defined as >= 75% completed PROs at one-month. Feasibility will be compared between two arms using the two-sample proportion tests based on two binomial distributions. Fisher's exact method will be used to calculate the p value. Potential moderators will include sociodemographic information (i.e., Protocol for Responding to & Assessing Patients' Assets, Risks & Experiences [PRAPARE] form, geocoding, etc.), health literacy, adherence boosters (reminder calls, text messages), cancer type, treatment plan, and location of care (inpatient hospital or outpatient clinic setting). Their effects will be evaluated using the logistic regression method with feasibility modeled as the binary response.

Adolescent and young adults (AYAs') health-related quality of life (HRQOL) priorities and HRQOL PRO trajectories
Časové okno: Up to 12 months
Descriptive statistics (e.g., frequencies, mean, etc.) will be performed to evaluate the importance of the domains at each time point. The changes over time may be modeled using the generalize linear mixed models (e.g., zero-inflated negative binomial regression to model the ranks with excessive zeros). To examine changes in Patient Reported Outcomes Measurement Information System Computer Adaptive Tests over time, will fit linear (or generalized linear) mixed models to model the trajectory of the outcomes by group and time accounting for the repeated measures on a subject, using linear contrasts to estimate differences in means by group and by time. Will plan to perform subgroup analyses by sex, study arm, and minority status.
