Yksityiskohtainen kuvaus

Ensisijainen tavoite: Suorita satunnaistettu kontrolloitu pilottikoe AYA:iden kanssa määrittääksesi AYA:n prioriteettien perusteella suoritettavien PRO:iden toteutettavuuden ja hyväksyttävyyden. AYA:t saavat luvan ja satunnaistetaan valitsemaan viisi HRQOL-aluetta kojelaudalta (Choice PRO) kullakin arviointijaksolla tai viiteen standardiin HRQOL-alueeseen (kiinteä PRO; fyysinen toiminta, kipu, kognitiivinen toiminta, sosiaalinen tuki, talous) kullakin arviointijaksolla. AYA:t suorittavat perustason, 1, 3, 6 ja 12 kuukauden arvioinnit, jotka sisältävät yhdistelmän CAT:itä ja staattisia lyhyitä lomakkeita. Toteutettavuus toteutetaan siten, että ≥ 75 % valmiit PRO:t ja ≥ 75 % hyväksyttävyys. Toteutettavuutta ja hyväksyttävyyttä verrataan Choice PRO:han satunnaistettujen AYA:iden ja kiinteän PRO:n välillä. Kiinnittymisen tehostajia (muistutuspuhelut, tekstiviestit) käytetään puuttuvien tietojen minimoimiseen ja niitä arvioidaan moderaattoreina.

Tutkiva tavoite: Pyydä AYA:n asetuksia sen suhteen, kuinka heidän PRO-datansa tulisi jakaa heidän ja/tai heidän tarjoajiensa kanssa. Tulevaisuuden tutkimuksia varten AYA:ilta kysytään, haluavatko osallistujat ja miten heidän PRO-tietojaan käyttää heidän lääkintätiiminsä sekä haluavatko osallistujat PRO-tietonsa jaettavan heidän ja heidän perheidensä kanssa ja miten.

Tutkijat odottavat, että PROMIS CAT:t lisäävät PRO-tiedonkeruukapasiteettia. AYA:t satunnaistetaan Choice PRO:han, joille annetaan mahdollisuus valita, mitkä PRO-alueet täytetään, ja niillä on vähemmän puuttuvaa dataa. Tutkijat odottavat, että suurin osa AYA:sta haluaa PRO-palautetta itselleen, perheilleen ja palveluntarjoajilleen. Löydökset edistävät ja tukevat PRO:iden sisällyttämistä kliinisiin ja tukihoitotutkimuksiin, tarjoamalla AYA:n äänen ja tiedottaen tulevasta potilaskeskeisestä hoidosta, joka on räätälöity AYA:n erilaisiin tarpeisiin. Tarjoamalla AYA:lle mahdollisuuden valita, mitkä PRO-alueet täytetään heidän terveyteen liittyvän elämänlaadun kannalta merkityksellisyyden perusteella, toisin kuin tutkimusryhmän ennalta määrittämien alueiden suorittaminen, tutkijat nostavat tutkijoiden arvioinnin potilaskeskeisyyttä.

Sisällyttämiskriteerit:

Potilaan tulee olla ≥ 18-vuotias ja ≤ 39-vuotias rekisteröintihetkellä. Potilaalla on oltava histologisesti vahvistettu diagnoosi minkä tahansa vaiheen primaarisesta syövästä 12 viikon (84 päivän) kuluessa ilmoittautumisesta.

Fisher's exact method will be used to calculate the p value. Potential moderators will include sociodemographic information (i.e., Protocol for Responding to & Assessing Patients' Assets, Risks & Experiences [PRAPARE] form, geocoding, etc.), health literacy, adherence boosters (reminder calls, text messages), cancer type, treatment plan, and location of care (inpatient hospital or outpatient clinic setting). Their effects will be evaluated using the logistic regression method with feasibility modeled as the binary response.

Aikaikkuna: Up to 12 months
Descriptive statistics (e.g., frequencies, mean, etc.) will be performed to evaluate the importance of the domains at each time point. The changes over time may be modeled using the generalize linear mixed models (e.g., zero-inflated negative binomial regression to model the ranks with excessive zeros). To examine changes in Patient Reported Outcomes Measurement Information System Computer Adaptive Tests over time, will fit linear (or generalized linear) mixed models to model the trajectory of the outcomes by group and time accounting for the repeated measures on a subject, using linear contrasts to estimate differences in means by group and by time. Will plan to perform subgroup analyses by sex, study arm, and minority status.
