Sociálně genomické mechanismy zdravotních rozdílů mezi dospívajícími a mladými dospělými (AYA), kteří přežili Hodgkinův a nehodgkinský lymfom

Tato studie zkoumá dopad sociálních a genetických faktorů na výsledky u adolescentů a mladých dospělých (AYA), kteří přežili rakovinu Hodgkinova nebo non-Hodgkinského lymfomu. DRUHÉ CÍLE:

I. Stanovit asociace jednotlivých faktorů resilience na DFS a OS u dospívajících a mladých dospělých pacientů, kteří přežili rakovinu.

II. Stanovit asociace sociálně-environmentálních rizikových faktorů a faktorů individuální resilience na kvalitu života (QOL) u dospívajících a mladých dospělých pacientů, kteří přežili rakovinu.

III. Kvantifikovat, do jaké míry by změny v expresi lidských genů mohly potenciálně zprostředkovat účinky sociálně-environmentálních rizikových faktorů a faktorů individuální odolnosti na DFS a OS u dospívajících a mladých dospělých, kteří přežili rakovinu.

PRŮZKUMNÝ CÍL:

I. Určit, zda vztah mezi sociálně-environmentálními rizikovými faktory nebo individuálními faktory odolnosti a distálními výsledky může být u dospívajících a mladých dospělých pacientů, kteří přežili rakovinu, ovlivněn rasou/etnickou příslušností, pohlavím a genderovou identitou a zeměpisnou polohou.

Pacienti s léčeným Hodgkinovým lymfomem nebo non-Hodgkinovým lymfomem ve Spojených státech

Popis

Kritéria pro zařazení:

  • Pacientovi musí být v době registrace >= 18 let
  • V době první primární diagnózy rakoviny Hodgkinova lymfomu nebo non-Hodgkinského lymfomu (NHL) musel být pacient ve věku 15–39 let.
  • Pacient musí mít v době registrace dokončenou terapii (s kompletní odpovědí, podle rozhodnutí lékaře).

  • Poslední datum předchozí systémové terapie pro první primární diagnózu Hodgkinova lymfomu nebo non-Hodgkinského lymfomu musí být do jednoho roku před registrací

    • POZNÁMKA: Systémová terapie se vztahuje na veškerou protinádorovou terapii, včetně, ale bez omezení, chemoterapie, intravenózních (IV) nebo perorálních cílených léků nebo ozařování a podávaných prostřednictvím klinického hodnocení nebo standardního přístupu.
  • Pacient musí mít výkonnostní stav Eastern Cooperative Oncology Group (ECOG) 0-3

  • Aby pacient mohl v této studii vyplnit požadované formuláře QOL, musí mluvit anglicky

    • POZNÁMKA: Weby nemohou překládat související formuláře QOL
  • Pacient nesmí dostávat aktivní léčbu Hodgkinova lymfomu nebo non-Hodgkinského lymfomu
  • Pacient musí mít přístup k internetu prostřednictvím počítače, tabletu nebo chytrého telefonu
  • Pacient musí mít e-mailovou adresu
  • Pacient musí mít schopnost porozumět a ochotu podepsat písemný informovaný souhlas. Za způsobilé budou považováni i pacienti se sníženou rozhodovací schopností (IDMC), kteří mají k dispozici zákonného zástupce (LAR) nebo pečovatele a/nebo člena rodiny.

Impact of social-environmental risk factors on changes in Charlson Comorbidity Index (CCI)
Časové okno: Through Study Completion, up to 2 years.
Will fit Conway-Maxwell-Poisson mixed effects models with a log link to evaluate the impact of social-environmental risk factors on changes in CCI over time. Estimates will be reported as rate ratios (and 95% confidence intervals) on the link scale, with model-based marginal means by visit to aid interpretation on the count scale. Models will be fit by maximum likelihood using all observed visits; targeted diagnostics will assess dispersion, fit, and influence.

Použije lineární smíšené modely k modelování měření kvality života s individuální odolností. Jednotlivé faktory odolnosti budou posuzovány následovně: Sociální podpora bude hodnocena pomocí Duke-UNC Funkčního dotazníku sociální podpory (DUFSS), smysl pro účel a smysl, bude posouzen zkrácenou formou Mental Health Continuum Short Form, Behavioral Self-efficacy být hodnoceny pomocí krátkých formulářů PROMIS (SF) pro zvládání symptomů, každodenních aktivit, sociálních interakcí, emocí a léků a léčby. Ukazatele socioekonomického postavení, majetku a postavení v zaměstnání/škole budou shromažďovány prostřednictvím průzkumu. Mezi další specifické demografické ukazatele, které je třeba zkoumat, patří sexuální orientace a genderová identita. Tyto metriky budou testovány jako možné modifikátory („moderátory“) účinků sociálně-environmentálních rizikových faktorů a faktorů odolnosti na genovou expresi a morbiditu, mortalitu a koncové body HRQOL.
