Sosiale genomiske mekanismer for helseforskjeller blant unge og unge voksne (AYA) overlevende av Hodgkin og non-Hodgkin lymfom

Denne studien undersøker virkningen av sosiale og genetiske faktorer på utfall hos ungdom og unge voksne (AYA) kreftoverlevere av Hodgkin eller non-Hodgkin lymfom. Sammenlignet med både eldre voksne og barndomskreftpasienter, opplever AYAer med kreft ulike diagnoser og spesifikke biologiske, kliniske, psykologiske og sosiale faktorer som påvirker risikoen for sykelighet etter behandling og for tidlig død. Innsamling av prøver av blodprøver og helse- og behandlingsinformasjon fra kreftoverlevere av Hodgkin- eller non-Hodgkin-lymfom kan hjelpe leger med å identifisere tilstander som øker sannsynligheten for at AYA-er blir syke og dør etter behandling av kreft og bedre forstå hvordan de kan møte behovene til ungdom og unge voksne kreftoverlevere.

HOVEDMÅL:

I. Å etablere sammenhengen mellom sosiale og miljømessige risikofaktorer på både sykdomsfri overlevelse (DFS) og total overlevelse (OS) for ungdom og unge voksne kreftoverlevere.

SEKUNDÆRE MÅL:

I. Å etablere assosiasjoner til individuelle motstandsfaktorer på DFS og OS for ungdom og unge voksne kreftoverlevere.

II. Å etablere assosiasjoner mellom sosiale og miljømessige risikofaktorer og individuelle motstandsfaktorer på livskvalitet (QOL) for ungdom og unge voksne kreftoverlevere.

III. For å kvantifisere i hvilken grad endringer i humant genuttrykk potensielt kan formidle effekten av sosiale og miljømessige risikofaktorer og individuelle motstandsfaktorer på DFS, og OS for ungdom og unge voksne kreftoverlevere.

UNDERSØKENDE MÅL:

I. Å bestemme om forholdet mellom sosiale og miljømessige risikofaktorer eller individuelle motstandsfaktorer og distale utfall kan modereres av rase/etnisitet, kjønn og kjønnsidentitet, og geografi for ungdom og unge voksne kreftoverlevere.

Pasienter med behandlet Hodgkin lymfom eller Non-Hodgkin lymfom over hele USA

Beskrivelse

Inklusjonskriterier:

  • Pasienten må være >= 18 år på registreringstidspunktet
  • Pasienten må ha vært mellom 15 og 39 år på tidspunktet for deres første primære kreftdiagnose av Hodgkin lymfom eller non-Hodgkin lymfom (NHL)
  • Pasienten må ha fullført behandling (med fullstendig respons, per klinikerbestemmelse) ved registreringstidspunktet

  • Pasienters siste dato for tidligere systemisk terapi for første primærdiagnose for Hodgkin lymfom eller non-Hodgkin lymfom må ha vært innen ett år før registrering

    • MERK: Systemisk terapi refererer til all kreftbehandling, inkludert men ikke begrenset til kjemoterapi, intravenøse (IV) eller orale målrettede medisiner, eller stråling, og administrert via en klinisk utprøving eller standardtilnærming
  • Pasienten må ha en Eastern Cooperative Oncology Group (ECOG) ytelsesstatus på 0-3

  • Pasienten må være engelsktalende for å kunne fylle ut de nødvendige QOL-skjemaene for denne studien

Impact of social-environmental risk factors on changes in Charlson Comorbidity Index (CCI)
Tidsramme: Through Study Completion, up to 2 years.
Will fit Conway-Maxwell-Poisson mixed effects models with a log link to evaluate the impact of social-environmental risk factors on changes in CCI over time. Estimates will be reported as rate ratios (and 95% confidence intervals) on the link scale, with model-based marginal means by visit to aid interpretation on the count scale. Models will be fit by maximum likelihood using all observed visits; targeted diagnostics will assess dispersion, fit, and influence.

Livskvalitet samlet inn via PROMIS-29 v2
Tidsramme: Inntil 2 år
Vil bruke lineære blandede modeller for å modellere livskvalitetsmålet med individuell resiliens. Individuelle resiliensfaktorer vil bli vurdert som følger: Sosial støtte vil bli evaluert ved hjelp av Duke-UNC Functional Social Support Questionnaire (DUFSS), følelsen av hensikt og mening, vil bli vurdert av Mental Health Continuum Short Form, Behavioural Self-efficacy vil vurderes ved å bruke PROMIS Short Forms (SF) for å håndtere symptomer, daglige aktiviteter, sosiale interaksjoner, følelser og medisiner og behandling. Indikatorer for sosioøkonomisk status, eiendeler og sysselsetting/skolestatus vil bli samlet inn via undersøkelse. Andre spesifikke demografiske indikatorer som skal undersøkes inkluderer seksuell legning og kjønnsidentitet. Disse beregningene vil bli testet som mulige modifikatorer ("moderatorer") av effektene av sosiale-miljømessige risikofaktorer og motstandsfaktorer på genuttrykk og sykelighet, dødelighet og endepunkter for HRQOL.
