Et fase III-forsøg med 6 versus 12 behandlinger af adjuverende FOLFOX Plus Celecoxib eller placebo til patienter med resekeret stadium III tyktarmskræft

FORMÅL: Dette randomiserede fase III-forsøg studerer at give oxaliplatin, leucovorin-calcium og fluorouracil sammen for at sammenligne, hvor godt de virker, når de gives sammen med eller uden celecoxib til behandling af patienter med stadium III tyktarmskræft tidligere behandlet med kirurgi.

RATIONALE: Lægemidler, der bruges i kemoterapi, såsom oxaliplatin, leucovorin calcium og fluorouracil, virker på forskellige måder for at stoppe væksten af tumorceller, enten ved at dræbe cellerne eller ved at forhindre dem i at dele sig. Celecoxib kan stoppe væksten af tumorceller ved at blokere nogle af de enzymer, der er nødvendige for cellevækst. OVERSIGT: Dette er en multicenterundersøgelse. Patienterne stratificeres efter antallet af positive lymfeknuder* (1-3 vs 4 eller mere) og samtidig regelmæssig lavdosis af aspirin (ja vs nej). Patienterne randomiseres til 1 ud af 4 behandlingsarme. Se venligst afsnittet "Våben" for mere information. I alle arme fortsætter behandling med celecoxib eller placebo i 3 år i fravær af sygdomsprogression eller uacceptabel toksicitet. Blod- og vævsprøver kan indsamles til biomarkøranalyse og farmakogenomiske undersøgelser. De primære og sekundære mål for forskningsstudiet er beskrevet nedenfor.

Primært mål:

1. At sammenligne sygdomsfri overlevelse af patienter med stadium III tyktarmskræft randomiseret til standard kemoterapi kun FOLFOX eller standard kemoterapi FOLFOX med 3 års celecoxib 400 mg dagligt.

At bidrage til en international prospektiv poolet analyse, der vil sammenligne den samlede overlevelse af patienter med stadium III tyktarmskræft randomiseret til 6 behandlinger af adjuverende FOLFOX kemoterapi eller 12 behandlinger af adjuverende FOLFOX kemoterapi eller 12 behandlinger med adjuverende FOLFOX kemoterapi.
At vurdere toksicitet af celecoxib som vedligeholdelsesadjuverende terapi hos patienter med stadium III tyktarmskræft.
At vurdere forskelle i kardiovaskulær-specifikke hændelser med celecoxib versus placebo i en population af overlevende af stadium III tyktarmskræft.
At evaluere forskelle i toksicitet, især kumulativ perifer neuropati, for patienter behandlet med 6 behandlinger af FOLFOX sammenlignet med dem, der blev behandlet med 12 behandlinger af FOLFOX.

Efter afslutning af studieterapien følges patienterne op hver 6. måned i op til 6 år.

Krav til tumorparametre
1. Histologisk dokumenteret adenokarcinom i tyktarmen. Den grove inferior (caudad) margin af den primære tumor skal ligge over den peritoneale refleksion (dvs. patienter med endetarmskræft er ikke kvalificerede). Kirurg bekræfter, at hele tumoren var over den peritoneale refleksion, er kun påkrævet i tilfælde, hvor det er vigtigt at fastslå, om tumoren er en rektal eller colon primær.
2. Tumorer skal være blevet fuldstændigt resekeret. Hos patienter med tumor adhærent til tilstødende strukturer skal en bloc R0 resektion dokumenteres i operationsrapporten eller på anden måde bekræftes af kirurgen. Nær eller positiv radial margin er ikke udelukkelser, så længe en bloc resektion blev udført. Positiv proksimal margin eller distal margin er en udelukkelse.
3. Nodepositiv sygdom (N1 eller N2) som angivet i AJCC version 7. Enten mindst én patologisk bekræftet positiv lymfeknude eller N1C (defineret som tumoraflejring(er) i subserosa, mesenterium eller ikke-peritonealiserede perikolske eller perirektale væv uden regional lymfeknude metastaser). Patienter med resekeret stadium IV sygdom er ikke kvalificerede.
4. Ingen tegn på resterende involveret lymfeknudesygdom eller metastatisk sygdom på registreringstidspunktet.
5. Patienter med synkron tyktarmskræft er berettiget, og stadieinddeling til stratificering vil være baseret på højere N-stadium af den mere avancerede primære tumor. Patienter med synkrone tyktarms- og rektale primære tumorer er dog ikke kvalificerede.
NSAID brug
Patienter er ikke berettigede, hvis de planlægger regelmæssig brug af NSAID'er i enhver dosis mere end 2 gange om ugen (i gennemsnit) eller aspirin på mere end 325 mg mindst tre gange om ugen i gennemsnit. Lavdosis aspirin, der ikke overstiger 100 mg/dag, er tilladt. Patienter, der accepterer at stoppe med almindelige NSAID'er eller højere doser aspirin, er berettigede, og ingen var ude-periode er påkrævet.
Patienthistorie
1. Ingen tidligere eller samtidig malignitet, undtagen behandlet basalcelle- eller pladecellekræft i hud, behandlet in situ livmoderhalskræft, behandlet lobulært eller duktalt karcinom in situ i det ene bryst eller enhver anden cancer, for hvilken patienten har været sygdomsfri i mindst 5 år.
2. Ingen neurosensorisk eller neuromotorisk toksicitet ≥ grad 2 på registreringstidspunktet.
3. Ingen kendt allergi over for platinforbindelser.
4. Ingen tidligere allergisk reaktion eller overfølsomhed over for sulfonamider, celecoxib eller NSAID'er.
5. Ingen historie med øvre gastrointestinal ulceration, øvre gastrointestinal blødning eller øvre gastrointestinal perforation inden for de seneste 3 år. Patienter med ulceration, blødning eller perforation i den nedre del af tarmen er ikke udelukket.
6. Ingen symptomatisk lungefibrose eller interstitiel pneumonitis ≥ grad 2.
7. Ingen hjerterisikofaktorer, herunder:
  - Ukontrolleret højt blodtryk (systolisk blodtryk > 150).
  - Ustabil angina.
  - Anamnese med dokumenteret myokardieinfarkt eller cerebrovaskulær ulykke.
  - New York Heart Association klasse III eller IV hjertesvigt.
Graviditet/sygeplejestatus
Ikke-gravid og ikke ammende. Mænd og kvinder i den fødedygtige alder skal acceptere at anvende passende prævention under kemoterapiens varighed og i op til 8 uger efter afslutningen af kemoterapien på grund af de ukendte teratogene virkninger af FOLFOX på det udviklende foster.

Aktiv komparator: Arm A - FOLFOX og placebo (12 behandlinger) Patienterne får FOLFOX hver 2. uge plus placebo hver dag i 12 behandlinger (24 uger). Hver 2-ugers periode kaldes en cyklus. FOLFOX inkluderer oxaliplatin, leucovorin og 5-FU. Derefter får patienterne placebo alene hver dag i i alt 3 år.	Medicin: 5-fluoruracil Patienterne får 400 mg/m^2 intravenøs bolus derefter 2400 mg/m^2 kontinuerlig intravenøs infusion over 46-48 timer. Andet: placebo Patienterne får placebo indgivet gennem munden én gang dagligt. Medicin: oxaliplatin Patienterne får 85 mg/m^2 intravenøst over to timer. Medicin: leukovorin Patienterne får 400 mg/m^2 intravenøst over to timer.
Eksperimentel: Arm B - FOLFOX og celecoxib (12 behandlinger) Patienterne får FOLFOX hver 2. uge plus celecoxib hver dag i 12 behandlinger (24 uger). Hver 2-ugers periode kaldes en cyklus. FOLFOX inkluderer oxaliplatin, leucovorin og 5-FU. Derefter får patienterne celecoxib alene hver dag i i alt 3 år. Sygdomsfri overlevelse Tidsramme: Ved 3 års opfølgning	Sygdomsfri overlevelse (DFS) er defineret som tidspunktet for randomisering indtil dokumenteret progression eller død af enhver årsag. Slutpunktet for dette forsøg er at sammenligne sygdomsfri overlevelse af patienter med stadium III tyktarmskræft randomiseret til kun standard kemoterapi (FOLFOX; Arm A og Arm C) eller standard kemoterapi (FOLFOX) med 3 års celecoxib 400 mg dagligt (Arm B) og arm D). Her rapporteres procentdelen af patienter, der var i live og sygdomsfri efter 3 år. En log-rank test stratificeret med stratificeringsfaktorerne blev brugt til at sammenligne sygdomsfri overlevelse (celecoxib vs placebo)	Ved 3 års opfølgning
