Beschreibung
Einschlusskriterien:
- Alter ≥1 bis ≤12 Monate bei der Einschreibung.
- Geboren in der 37. Schwangerschaftswoche.
- Eltern und/oder Erziehungsberechtigte können zur Zufriedenheit des Studienarztes, der den Registrierungsprozess abschließt, einen Identitätsnachweis erbringen.
- Bei gutem Allgemeinzustand und ohne klinisch bedeutsame Krankengeschichte.
- Eltern und/oder Erziehungsberechtigte können eine Einverständniserklärung abgeben.
- Bereit, Blutproben und Daten für zukünftige Forschungen aufzubewahren.
- Wohnt in oder in der Nähe von Kalifabougou, Faladje oder Torodo, Mali und ist für die Dauer der Studie verfügbar.
Ausschlusskriterien:
- Körpergewicht <3,5 kg.
- Verhaltensbedingte, kognitive oder psychiatrische Erkrankung des Elternteils und/oder Erziehungsberechtigten, die nach Ansicht des Prüfers die Fähigkeit des Elternteils und/oder Erziehungsberechtigten beeinträchtigt, das Studienprotokoll zu verstehen und einzuhalten.
- Jegliches Fieber (≥ 37,5 °C, unabhängig von der Route) oder akute Erkrankung innerhalb von 7 Tagen vor der Randomisierung.
- Klinisch signifikante angeborene Anomalie oder dokumentierte oder vermutete schwere medizinische Erkrankung (z. B. Epilepsie in der Vorgeschichte), schwere angeborene Anomalie oder unmittelbar lebensbedrohlicher Zustand des Säuglings, der nach Einschätzung des untersuchenden Arztes die Fähigkeit beeinträchtigen kann, die Studienanforderungen zu erfüllen.
- Vorgeschichte eines vermuteten oder tatsächlichen akuten lebensbedrohlichen Ereignisses.
- Erhalt von Blutprodukten, monoklonalen oder polyklonalen Antikörpern/Immunglobulinen (z. B. Hepatitis-B-Immunglobulin, intravenöses Immunglobulin) oder voraussichtliche Verwendung während der Studie.
- Alle während der Schwangerschaft bei der Mutter bekannten akuten oder chronischen Erkrankungen.
- Ergebnis der elterlichen Studie zum Verständnistest von <80 % richtig oder nach Ermessen des Prüfers.
- Hämoglobin-, Leukozyten-, absolute Neutrophilen- oder Thrombozytenzahl außerhalb der lokalen, labordefinierten Grenzwerte des Normalwerts. (Bei „nicht klinisch signifikanten“ Werten können Teilnehmer nach Ermessen des Prüfarztes einbezogen werden.)
- ALT- oder Kreatinin (Cr)-Spiegel über der lokalen, labordefinierten Obergrenze des Normalwerts. (Bei „nicht klinisch signifikanten“ Werten können Teilnehmer nach Ermessen des Prüfarztes einbezogen werden.)
- Mutter und/oder Kind sind mit HIV infiziert.
- Sichelzellenanämie durch Tests. (Hinweis: Bekannte Sichelzellanämie ist kein Ausschlusskriterium.)
- Nachweis einer klinisch signifikanten neurologischen, kardialen, pulmonalen, hepatischen, endokrinen, rheumatologischen, autoimmunen, hämatologischen, onkologischen oder renalen Erkrankung durch Anamnese, körperliche Untersuchung und/oder Laboruntersuchungen.
- Erhalt eines Prüfpräparats innerhalb der letzten 30 Tage.
- Teilnahme oder geplante Teilnahme an einer interventionellen Studie mit einem Prüfpräparat bis zum letzten erforderlichen Protokollbesuch. (Hinweis: Eine frühere, aktuelle oder geplante Teilnahme an Beobachtungsstudien ist NICHT ausschließend.)
- Vorgeschichte einer schweren allergischen Reaktion oder Anaphylaxie.
- Von einem Arzt diagnostizierte Speicheldrüsenerkrankung (z. B. Parotitis, Sialadenitis, Sialolithiasis, Speicheldrüsentumoren).
- Vorbestehende Autoimmun- oder Antikörper-vermittelte Erkrankungen, einschließlich, aber nicht beschränkt auf systemischen Lupus erythematodes oder autoimmune Thrombozytopenie.
- Bekanntes Immunschwächesyndrom.
- Bekannte Asplenie oder funktionelle Asplenie.
- Anwendung chronischer (≥ 14 Tage) oraler oder intravenöser Kortikosteroide (außer topische oder nasale Anwendung) in immunsuppressiven Dosen (d. h. Prednison > 10 mg/Tag) oder immunsuppressiven Arzneimitteln innerhalb von 30 Tagen nach Tag 0.
- Vorheriger Erhalt des R21/Matrix-MTM-Impfstoffs.
- Vorheriger Erhalt eines Prüfimpfstoffs gegen Malaria oder eines monoklonalen Antikörpers.
- Klinische Anzeichen einer Mangelernährung.
- Andere Bedingungen, die nach Ansicht des Prüfers die Sicherheit oder Rechte einer an der Studie teilnehmenden Person gefährden, die Bewertung der Studienziele beeinträchtigen oder dazu führen würden, dass der Teilnehmer das Protokoll nicht einhalten kann.

Antibody Response to R21/Matrix-MTM Vaccine

The antibody response to the R21/Matrix-M was assessed as the total anti-Asn-Ala-Asn-Pro (anti-NANP) immunoglobulin G (IgG) ((anti-NANP IgG)) antibody titers.
The total IgG anti-NANP antibody titers was measured by electrochemiluminescence immunoassay (ECLIA), using the Meso Scale Discovery LLC-based automation platform on serum samples collected 28 days (day 224) and 84 days (day 280) after the third dose of R21/Matrix-MTM vaccination.
Outcomes analyzed as concentration of antibody titers of the polyclonal antibody response in a serum sample.
Concentration is expressed in arbitrary units per milliliter (AU/mL), which are assigned based on a sample's relative binding as compared to an established serum reference standard.
