TAVOITTEET:

  • Vertaa doksorubisiinilla, bleomysiinillä, vinblastiinilla ja dakarbatsiinilla (ABVD) hoidettujen potilaiden häiriötöntä eloonjäämistä, kun he sairastavat paikallisesti laajaa tai pitkälle edennyttä Hodgkinin lymfoomaa ja doksorubisiinia, vinblastiinia, vinkristiiniä, bleomysiiniä, mekloretamiinia, etoposidia ja prednisonia (tai ilman prednisonia). sädehoito.
  • Vertaa näiden potilaiden kokonaiseloonjäämistä ja taudin etenemistä 5 ja 10 vuoden kuluttua näiden hoito-ohjelmien saamisesta.

OUTLINE: Tämä on satunnaistettu tutkimus. Potilaat ositetaan haitallisten riskitekijöiden lukumäärän (0–2 vs. 3–7), sairauden ominaisuuksien (paikallisesti laaja vs. pitkälle edennyt) ja tuloajan mukaan (ennen lisäystä 6 vs. lisäyksen 6 jälkeen). Potilaat satunnaistetaan yhteen kahdesta hoitohaarasta.

  • Käsivarsi A (ABVD): Potilaat saavat doksorubisiinia (25 mg/m²), bleomysiiniä (10 u/m²), vinblastiinia (6 mg/m²) ja dakarbatsiinia (375 mg/m²) IV päivinä 1 ja 15. Kurssit toistetaan 28 päivän välein. Potilaat asetetaan uudelleen 4 hoitojakson jälkeen. Potilaat, jotka ovat täysin remissiossa, saavat 2 lisäkurssia. Epäonnistuneet eloonjäämisajat määritellään ajaksi satunnaistamisesta varhaisempaan etenemiseen/relapsaan tai kuolemaan. Viiden vuoden häiriötön eloonjäämisaika on todennäköisyys, että potilas on häiriötön ja elää 5 vuotta.

Eteneminen määritellään koon kasvuksi 25 % esikäsittelymittausten tulojen summasta tai uusien leesioiden ilmaantumisesta. Maksan tai pernan merkittävä suureneminen on todiste etenemisestä. Merkittävä koon kasvu määritellään > 2,0 cm:n etäisyydeksi rannikon reunan ja jommankumman elimen alareunan välillä.

Relapsi määritellään minkä tahansa Hodgkinin taudin kliinisen todisteen uusiutumiseksi potilaalla, joka on saanut täydellisen vasteen. Relapsi osittaisen vasteen saaneille määritellään eteneväksi sairaudeksi suhteessa sairauden tilaan osittaisen remission aikana.
