상세 설명
기본 목표:
I. R115777(티피파르닙) 유지 요법을 2차 또는 후속 완전 관해 또는 1차 유도 실패 후 완전 반응(CR) 상태인 AML 환자의 무병 생존(DFS)과 관련하여 관찰만 하는 것과 비교하는 것.
2차 목표:
I. 양 팔에서 환자의 전체 생존을 비교하기 위해. II. 차도가 있는 AML 환자에서 R115777의 장기 투여의 장기 안전성 및 독성을 평가합니다.
개요: 환자는 2개의 치료군 중 1개로 무작위 배정됩니다.
ARM A: 환자는 1-21일에 매일 2회(BID) 티피파르닙을 구두로(PO) 투여받습니다. 과정은 질병 진행 또는 허용할 수 없는 독성이 없는 경우 28일마다 반복됩니다.
ARM B: 환자는 관찰만 받습니다.

이 연구에 참가할 자격이 있는 환자는 다음 범주 중 하나에 속해야 합니다.
- 1차 유도 실패 후 첫 번째 관해 상태에 있는 환자
- 환자는 적어도 두 가지 화학 요법 유도 요법을 받아야 합니다.
- 2차 또는 그 이후의 관해 상태에 있는 환자
- 60세 이상의 환자
환자는 연구에 참여하기 위해 혈구 수 및 골수 연구에서 완전 관해(CR) 또는 형태학적 관해(MR)에 있어야 합니다.
- 확인 골수는 무작위화 2주 전에 =< 수행되어야 합니다.

- 세계보건기구(WHO) 기준에 따른 AML
- 다계통 이형성증을 동반한 급성 골수성 백혈병
- 급성 전골수성 백혈병(FAB M3) 환자는 자격이 없습니다.
환자는 현재 완화에 대한 치료 완료 후 60일 이내에 등록해야 합니다. 환자는 아래 기준 중 하나를 충족하는 경우 자격이 있습니다.
- 유도 요법 후 말초혈구수에 의한 관해(CR 또는 MR) 60일 이내 또는
- 유도 요법 후 퇴원 후 60일 이내 또는
- 차도 후 치료 후 퇴원 후 60일 이내 또는
- 마지막 화학 요법 투여 후 혈구 수가 회복된 후 60일 이내
아래의 모든 환자는 연구 참가 자격이 있습니다.
- 강화 요법을 받은 환자
- 강화 또는 관해 후 치료를 받지 않은 환자
- 자가 줄기 세포 이식을 받은 환자
- 현재 관해 상태에서 동종이계 이식(골수 이식[BMT] 또는 말초 줄기 세포 이식[PSCT])을 받은 환자는 자격이 없습니다. 이전 차도에서 동종이계 이식을 받았고 후속 재발 후 현재 차도 상태인 환자는 자격이 있습니다.
- 골수외 질환의 병력이 있는 환자는 연구 등록 시점에 완전한 관해 상태이고 골수외 질환에 대한 치료가 더 이상 필요하지 않은 경우 자격이 있습니다.
- 환자는 임신 중이거나 수유 중이어서는 안 됩니다. 임신 가능성이 있는 모든 여성은 임신을 배제하기 위해 무작위 배정 전 < 2주 전에 혈액 검사 또는 소변 검사를 받아야 합니다.
- 가임기 여성과 성적으로 활동적인 남성은 허용되고 효과적인 피임 방법을 사용하도록 강력히 권고합니다.

- 절대 호중구 수(ANC) >= 1000/mm^3
- 혈소판 수 >= 50,000/mm^3
