Denne randomiserte fase III-studien studerer tipifarnib i behandling av pasienter med akutt myeloid leukemi (AML) i remisjon. Tipifarnib kan stoppe veksten av kreftceller ved å blokkere enzymene som er nødvendige for deres vekst. Det er ennå ikke kjent om tipifarnib er mer effektivt enn observasjon alene for å forhindre tilbakefall av AML.

PRIMÆRE MÅL:

I. Å sammenligne R115777 (tipifarnib) vedlikeholdsbehandling med observasjon kun med hensyn til sykdomsfri overlevelse (DFS) hos pasienter med AML i andre eller påfølgende fullstendig remisjon eller i fullstendig respons (CR) etter primær induksjonssvikt.

SEKUNDÆRE MÅL:

I. For å sammenligne total overlevelse av pasienter i begge armer. II. For å evaluere den langsiktige sikkerheten og toksisiteten ved utvidet administrering av R115777 hos AML-pasienter i remisjon.

OVERSIKT: Pasienter er randomisert til 1 av 2 behandlingsarmer.

ARM A: Pasienter får tipifarnib oralt (PO) to ganger daglig (BID) på dag 1-21. Kurs gjentas hver 28. dag i fravær av sykdomsprogresjon eller uakseptabel toksisitet.

ARM B: Pasienter gjennomgår kun observasjon.

Etter avsluttet studiebehandling følges pasientene opp i 5 år.

Inklusjonskriterier:

  • Pasienter som er kvalifisert til å delta i denne studien må falle inn i en av disse kategoriene:

    • Pasienter i første remisjon etter primær induksjonssvikt
    • Pasienter må ha mottatt minst to kjemoterapi-induksjonsregimer
    • Pasienter i andre eller påfølgende remisjon
    • Pasienter > 60 år i første remisjon

  • Pasienter må være i fullstendig remisjon (CR) eller morfologisk remisjon (MR) etter blodtelling og benmargsstudier for å delta i studien

    • Bekreftende benmarg må utføres =< 2 uker før randomisering

  • Pasienter må ha morfologisk bevis (fra benmargsaspirat, utstryk eller berøringsforberedelser av margbiopsi) på at de hadde AML av en av følgende typer før oppnåelse av CR/MR

    • Akutt myeloblastisk leukemi, minimal differensiering (fransk-amerikansk-britisk [FAB] M0)
    • Akutt myeloblastisk leukemi uten differensiering (FAB M1)
    • Akutt myeloblastisk leukemi med modning (FAB M2)
    • Akutt myelomonocytisk leukemi (FAB M4)
    • Akutt monocytisk leukemi (FAB M5)
    • Akutt erytroleukemi (FAB M6)
    • Akutt megakaryocytisk leukemi (FAB M7)
    • Refraktær anemi med overflødig blaster i transformasjon (RAEB-T)
    • AML etter Verdens helseorganisasjons (WHO) kriterier
    • Akutt myeloid leukemi med multilineage dysplasi
  • Pasienter med akutt promyelocytisk leukemi (FAB M3) er ikke kvalifisert

  • Pasienter må registreres innen 60 dager etter fullført behandling for den nåværende remisjonen; Pasienter er kvalifisert hvis de oppfyller noen av kriteriene nedenfor:

    • Innen 60 dager etter remisjon (CR eller MR) ved perifere blodtellinger etter induksjonsterapi eller
    • Innen 60 dager etter utskrivning fra sykehuset etter induksjonsterapi eller

    • Innen 60 dager etter gjenoppretting av blodverdier etter siste dose kjemoterapi

  • Alle pasientene nedenfor er kvalifisert for studieoppgang:

    • Pasienter som har mottatt konsolideringsterapi
    • Pasienter som ikke har mottatt konsoliderings- eller postremisjonsbehandling
    • Pasienter som har gjennomgått en autolog stamcelletransplantasjon
  • Pasienter som har mottatt en allogen transplantasjon (benmargstransplantasjon [BMT] eller perifer stamcelletransplantasjon [PSCT]) i sin nåværende remisjon er ikke kvalifisert; Pasienter som har hatt en allogen transplantasjon i en tidligere remisjon og for øyeblikket er i remisjon etter påfølgende tilbakefall er kvalifisert
  • Pasienter med en historie med ekstramedullær sykdom er kvalifisert hvis de er i fullstendig remisjon på tidspunktet for studiestart og ikke lenger trenger behandling for sin ekstramedullære sykdom
  • Pasienter må ikke være gravide eller ammende; alle kvinner i fertil alder må ha en blodprøve eller urinstudie < 2 uker før randomisering for å utelukke graviditet
  • Kvinner i fertil alder og seksuelt aktive menn anbefales sterkt å bruke en akseptert og effektiv prevensjonsmetode
  • Pasienter må ikke være kjent for å ha en allergi mot imidazolmedisiner, slik som klotrimazolketokonazol, mikonazol, ekonazol eller terkonazol; dette inkluderer ikke flukonazol, vorikonazol eller itrakonazol
  • Eastern Cooperative Oncology Group (ECOG) ytelsesstatus på 0 - 2

  • Pasienter må ikke ha aktiv leversykdom; total eller direkte bilirubin må være < 2 mg/dl
  • Serumglutaminoksaloeddiksyretransaminase (SGOT) (aspartataminotransferase [AST]) og serumglutamatpyruvattransaminase (SGPT) (alaninaminotransferase [ALT]) må være =< 2,5 ganger øvre normalgrense
  • Absolutt nøytrofiltall (ANC) >= 1000/mm^3
  • Blodplateantall >= 50 000/mm^3
  • Pasienter må ikke ta et hepatisk enzyminduserende antikonvulsivt middel; en pasient vil ikke være kvalifisert for studien hvis pasienten for øyeblikket tar ett av disse midlene og ikke kan byttes til et ikke-hepatisk enzyminduserende antikonvulsivt middel
