프로포폴 기반 진정 대장내시경 검사 중 Fentanyl 대 Pethidine의 안전성 및 효능: 전향적 무작위 연구

진정제 대장내시경 검사 중 프로포폴 사용이 전 세계적으로 증가했습니다.
프로포폴은 미다졸람보다 발현시간이 짧다.
그러나 가장 대중적인 첨가제 진통제인 페티딘(pethidine)은 다른 약력학적 특성을 보인다.
페티딘은 프로포폴보다 발병 시간이 길고(거의 30분) 회복하는 데 더 많은 시간이 필요합니다.
이러한 이유로 펜타닐의 사용은 전 세계적으로 널리 증가하고 있습니다.
그러나 페티딘과 펜타닐의 정확한 비교는 연구되지 않았다.
따라서 본 연구에서는 두 약물을 비교하여 이 약물에 대한 만족도를 확인하고자 하였다.

- 진정제로 대장내시경 선별검사를 받는 환자
- 20세 이상 65세 미만 환자
- 본 연구 참여에 동의한 환자
제외 기준:
- 이전에 지적된 결장 폴립을 제거하기 위해 외부 병원에서 의뢰된 환자
- 장 정결 불량으로 맹장 삽관이 불가능한 경우
- 대장내시경 직전에 진정성 EGD를 시행한 환자
- 프로포폴, 페티딘, 펜타닐에 알레르기가 있는 환자
- MAO 억제제를 정기적으로 복용한 환자
- 대장내시경 전 대장절제술을 받은 환자
- 대장내시경 전 IBD 진단을 받은 환자
- 임산부
- 심각한 심혈관 질환, 신장 질환, 폐 질환으로 진정 수준을 유지할 수 없는 환자
- 본 연구 참여에 동의하지 않는 환자
- 외국인 또는 정신지체 등으로 본 연구를 이해할 수 없는 환자
- 37.5kg 이하의 저체중 환자

이 부문의 피험자는 진정제인 프로포폴과 펜타닐로 대장내시경을 받는 환자로 구성됩니다.

펜타닐 용량 50μg(37.5kg~62.5kg), 75μg(62.5kg~87.5kg), 100μg(>87.5kg),
일회용 프로포폴 용량 1.0mg/kg IV 볼루스 로딩, 20mg, IV 볼루스, 적정(최대 MOAAS 5)

따라서 피험자의 시술 만족도를 확인합니다.
한 약물에 대한 환자의 시술 만족도가 다른 약물과 비교되는 경우 대장 내시경 검사 시 진통제로 한 약물을 선택합니다.
