En dobbeltblind fase III-studie av doksorubicin og cyklofosfamid etterfulgt av paklitaksel med bevacizumab eller placebo hos pasienter med lymfeknutepositiv og høyrisiko lymfeknute-negativ brystkreft
Denne randomiserte fase III-studien studerer doksorubicinhydroklorid, cyklofosfamid og paklitaksel for å se hvor godt de fungerer med eller uten bevacizumab i behandling av pasienter med kreft som har spredt seg til lymfeknuter (lymfeknutepositive) eller kreft som ikke har spredt seg til lymfen. noder, men har høy risiko for å komme tilbake (høyrisiko, lymfeknute-negativ brystkreft).
Legemidler som brukes i kjemoterapi, som doksorubicinhydroklorid, cyklofosfamid og paklitaksel, virker på forskjellige måter for å stoppe veksten av tumorceller, enten ved å drepe cellene, ved å stoppe dem fra å dele seg eller ved å hindre dem i å spre seg.
Monoklonale antistoffer, som bevacizumab, kan forstyrre tumorcellenes evne til å vokse og spre seg.
Bevacizumab kan også stoppe veksten av brystkreft ved å blokkere blodstrømmen til svulsten.
Å gi kjemoterapi etter operasjonen kan drepe eventuelle tumorceller som er igjen etter operasjonen og bidra til å forhindre at svulsten kommer tilbake.

PRIMÆRE MÅL:
I. For å bestemme sykdomsfri overlevelse av pasienter (definert som invasiv sykdomsfri overlevelse [IFDS]) med lymfeknutepositiv og høyrisiko lymfeknute-negativ brystkreft randomisert til behandling med enten doksorubicin (doksorubicinhydroklorid)/cyklofosfamid pluss placebo fulgte med paklitaksel (AC + placebo > T + placebo) eller samme kjemoterapiregime pluss bevacizumab.
SEKUNDÆRE MÅL:
I. For å sammenligne kortsiktig (20-24 uker) versus langvarig (50-54 uker) behandling med bevacizumab.
II. For å sammenligne den totale overlevelsen. III. For å vurdere toksisitet. IV. For å evaluere sammenhengen mellom utfall i E5103 (sykdomsfri overlevelse, total overlevelse og toksisiteter) og genotype (avledet fra kandidater av enkeltnukleotidpolymorfismer og genomomfattende evalueringer).
V. Å sammenligne livskvaliteten til brystkreftpasienter behandlet med AC/paclitaxel og bevacizumab eller placebo, når det gjelder fysiske symptomer, fysisk funksjon, psykologisk tilstand og sosial funksjon over en 18 måneders periode.
VI. For å bestemme virkningen av teoretisk biomarkørinformasjon på pasienters vilje til å akseptere toksisitetene til bevacizumab for den estimerte potensielle fordelen.
VII. X. For å fastslå om det er en sammenheng mellom sent tilbakefall og BMI ved diagnose og 5 år etter diagnose, og om BMI-assosierte inflammatoriske og/eller metabolske biomarkører er assosiert med tidlig og sent residiv.
OVERSIKT: Pasienter er randomisert til 1 av 3 behandlingsarmer.
ARM I: Pasienter får doksorubicinhydroklorid intravenøst (IV), cyklofosfamid IV over 20-30 minutter og placebo IV over 30-90 minutter på dag 1. Behandlingen gjentas hver 2. eller 3. uke i 4 kurer. Fra og med 3 uker senere får pasientene paklitaksel IV over 1 time på dag 1, 8 og 15 og placebo IV over 30-90 minutter på dag 1. Behandling med paklitaksel og placebo gjentas hver 3. uke i 4 kurer.
ARM II: Pasienter får doksorubicinhydroklorid og cyklofosfamid som i arm I og bevacizumab IV over 30-90 minutter på dag 1. Behandlingen gjentas hver 2. eller 3. uke i 4 kurer. Fra og med 3 uker senere får pasientene paklitaksel som i arm I og bevacizumab IV over 30-90 minutter på dag 1. Behandling med paklitaksel og bevacizumab gjentas hver 3. uke i 4 kurer.
ARM III: Pasienter får doksorubicinhydroklorid og cyklofosfamid som i arm I og bevacizumab som i arm II. Behandlingen gjentas hver 2. eller 3. uke i 4 kurer. Fra og med 3 uker senere får pasientene paklitaksel som i arm I og bevacizumab som i arm II. Behandling med paklitaksel og bevacizumab gjentas hver 3. uke i 4 kurer. Fra og med 2 måneder senere får pasientene bevacizumab IV over 30-90 minutter på dag 1. Behandling med bevacizumab alene gjentas hver 3. uke i 10 kurer.
I alle armer fortsetter behandlingen i fravær av sykdomsprogresjon eller uakseptabel toksisitet.
Etter avsluttet studiebehandling følges pasientene opp i 15 år.

Inklusjonskriterier:
Pasienter må ha histologisk bekreftet adenokarsinom i brystet med betydelig risiko for fjernt residiv basert på minst ett av følgende kriterier:
For aksillær lymfeknutepositiv sykdom:
- Involvering av minst én vaktpost eller aksillær lymfeknute ved rutinemessig histologisk undersøkelse; Pasienter med negative vaktpostknuter og negative aksillære noder eller involvering kun demonstrert ved immunhistokjemi er ikke kvalifisert med mindre de oppfyller ett av de andre kvalifikasjonskriteriene nedenfor
- MERK: betrakt intramammære noder som likeverdige med aksillære noder med hensyn til kvalifisering og stratifisering
For aksillær lymfeknute negativ sykdom:
- Østrogenreseptor (ER) negativ svulst >= 1 cm
- ER+ tumor >= 5 cm uavhengig av residivskåre
- ER+ svulst >= 1 cm men < 5 cm med residivscore >= 11 (pasienter som er registrert i TAILORx-studien er kvalifisert)

- MERK: brystbevaringskirurgi inkluderer lumpektomi, delvis mastektomi og eksisjonsbiopsi
- Marginene for brystbevaringskirurgi eller mastektomi må være histologisk fri for invasiv brystkreft og duktalt karsinom in situ (DCIS); Pasienter med reseksjonsmarginer positive for lobulært karsinom in situ (LCIS) er kvalifisert

- Innen =< 8 uker før randomisering: venstre ventrikkel ejeksjonsfraksjon (LVEF) >= institusjonelle grenser for normal ved multigated acquisition scan (MUGA) eller ekkokardiogram (ECHO)
Pasienter som har gjennomgått brystbevarende operasjoner må få stråling; før randomisering må etterforskeren spesifisere den planlagte stråleteknikken:
- Helbryststråling (WBRT) etter kjemoterapi
- Akselerert partiell bryststråling (APBI) etter kjemoterapi
- Akselerert delvis bryststråling (APBI) før kjemoterapi
- MERK: hvis APBI ble fullført før studiestart, må dag 1 av protokollbehandling være minst 4 uker etter fullføring av APBI
- Postmastektomi strålebehandling (RT) er nødvendig for alle pasienter med en primærtumor på >= 5 cm eller involvering av 4 eller flere lymfeknuter; post-mastektomi RT kan administreres etter etterforskerens skjønn for alle andre mastektomipasienter
- Pasienter med human epidermal vekstfaktorreseptor (HER)2+ (3+ ved immunhistokjemi [IHC] eller fluorescerende in situ hybridisering [FISH] ratio >= 2) brystkreft er ikke kvalifisert
- Pasienter med synkron bilateral brystkreft (diagnostisert innen en måned) er kvalifisert hvis tumor-, node-, metastase- (TNM)-stadiet oppfyller kvalifikasjonskriteriene for denne studien
- Pasienter må ikke ha kliniske tegn på inflammatorisk sykdom eller faste aksillære noder ved diagnose
Pasienter må ikke tidligere ha mottatt cellegift eller hormonbehandling for denne brystkreften; tidligere behandling med antracyklin, antracendion eller taxan for enhver tilstand er ikke tillatt
- MERK: tidligere bruk av tamoxifen for kjemoprevensjon er tillatt, men må avbrytes ved studiestart; på samme måte er tidligere bruk av raloksifen tillatt, men må avbrytes ved studiestart
Pasienter skal ikke ha hatt noen større kirurgiske inngrep innen 28 dager etter planlagt behandlingsstartdato
- MERK: ikke-operativ biopsi eller plassering av en vaskulær tilgangsenhet regnes ikke som en større operasjon
- Pasienter kan ikke ha fått plassering av en vaskulær tilgangsanordning innen 24 timer etter planlagt dag 1 av behandlingen
Pasienter må ikke ha klinisk signifikant kardiovaskulær eller cerebrovaskulær sykdom, inkludert:
Enhver historie om
- Cerebrovaskulær sykdom inkludert forbigående iskemisk angrep (TIA), hjerneslag eller subaraknoidal blødning
- Iskemisk tarm

Pasienter som trenger full dose antikoagulasjon kan registrere seg forutsatt at de oppfyller følgende kriterier:
- Pasienten må ha et internasjonalt normalisert forhold (INR) (vanligvis mellom 2 og 3) på en stabil dose warfarin eller ha en stabil dose av lavmolekylært heparin (LMW)

- Pasienter må ikke ha overfølsomhet overfor paklitaksel eller legemidler som bruker vehikelen Cremophor, eggstokkcelleprodukter fra kinesisk hamster eller andre rekombinante humane antistoffer
- Kvinner må ikke være gravide eller ammende; alle kvinner i fertil alder må ha en blod- eller urinprøve innen 7 dager før randomisering for å utelukke graviditet
- Kvinner i fertil alder og seksuelt aktive menn må bruke en akseptert og effektiv prevensjonsmetode

Fra og med 2 måneder senere får pasientene bevacizumab IV over 30-90 minutter på dag 1.
Behandling med bevacizumab alene gjentas hver 3. uke i 10 kurer.
