Fase III randomisert studie av Sorafenib Plus Doxorubicin versus Sorafenib hos pasienter med avansert hepatocellulært karsinom (HCC)

Denne randomiserte fase III-studien studerer sorafenibtosylat og doksorubicinhydroklorid for å se hvor godt de virker sammenlignet med sorafenibtosylat alene ved behandling av pasienter med leverkreft som har spredt seg til nærliggende vev eller lymfeknuter eller har spredt seg til andre steder i kroppen. Sorafenibtosylat kan stoppe veksten av tumorceller ved å blokkere noen av enzymene som trengs for cellevekst og ved å blokkere blodstrømmen til svulsten. PRIMÆRE MÅL:

I. Sammenlign den totale overlevelsen (OS) for pasienter behandlet med sorafenib (sorafenibtosylat) og doksorubicin (doksorubicinhydroklorid) med den for de behandlet med sorafenib.

SEKUNDÆRE MÅL:

I. Sammenlign tid til progresjon (TTP) for pasienter behandlet med sorafenib og doksorubicin med tiden til de behandlet med sorafenib.

II. Sammenlign progresjonsfri overlevelse (PFS) for pasienter behandlet med sorafenib og doksorubicin med pasienter behandlet med sorafenib.

III. Sammenlign tumorrespons ved å bruke Response Evaluation Criteria in Solid Tumors (RECIST) kriterier for pasienter behandlet med sorafenib og doksorubicin med de som behandles med sorafenib.

OVERSIKT: Pasienter er randomisert til 1 av 2 behandlingsarmer.

ARM I: Pasienter får doksorubicinhydroklorid intravenøst (IV) på dag 1 og sorafenibtosylat oralt (PO) én gang daglig (QD) eller to ganger daglig (BID) på dag 1-21. Behandlingen gjentas hver 21. dag i 6 kurer i fravær av sykdomsprogresjon eller uakseptabel toksisitet. Etter 6 kurer kan pasienter fortsette å få sorafenibtosylat PO QD eller BID i fravær av sykdomsprogresjon eller uakseptabel toksisitet.

ARM II: Pasienter får sorafenibtosylat PO QD eller BID på dag 1-21. Kurs gjentas hver 21. dag i fravær av sykdomsprogresjon eller uakseptabel toksisitet.

Etter avsluttet studiebehandling følges pasientene opp hver 3. måned i 1 år og deretter hver 6. måned i 2 år.

Inklusjonskriterier:

Pasienter må ha patologisk eller cytologisk påvist hepatocellulært karsinom; kjent blandet histologi (f.eks. hepatocellulært karsinom pluss kolangiokarsinom) eller fibrolamellær variant er ikke tillatt
Pasienter må ha lokalt avansert eller metastatisk sykdom; lokalt avansert sykdom er definert som sykdom som anses å være uopererbar eller ikke kvalifisert for transplantasjon uten fjernmetastaser
Lesjoner må være nøyaktig målbare i minst én dimensjon (lengste diameter som skal registreres) som >= 2 cm med konvensjonelle teknikker eller som >= 1 cm med spiral computertomografi (CT) skanning
Ingen tidligere adjuvans sorafenib eller andre v-RAF-1 murin leukemi viral onkogen homolog (Raf)/vaskulær endotelial vekstfaktor (VEGF)-hemmere; annen tidligere adjuvant behandling er tillatt hvis fullført > 6 måneder før registrering med dokumentert tilbakefall av hepatocellulært karsinom (HCC)
Pasienter kan ha blitt behandlet med lokale regionale terapier forutsatt at de enten har:
- En mållesjon som ikke har vært utsatt for lokal terapi eller
- Mållesjonen(e) innenfor feltet for den lokale terapien har vist en økning på >= 20 % i størrelsen siden siste behandling
  - Slik terapi må fullføres minst 4 uker før registrering; Pasienter som har fått palliativ strålebehandling til beinet trenger ikke vente 4 uker med å begynne protokollbehandling
Tidligere tillatte terapier inkluderer følgende:
- Ekte embolisering, stråling, radioaktive mikrosfærer, etc

Antiviral behandling er tillatt, men interferonbehandling må stoppes minst 4 uker før registrering
Allotransplantater er ikke tillatt: ingen tidligere historie med allograft, inkludert men ikke begrenset til lever- og benmargstransplantasjoner
Pasienter må ha gjennomført en eventuell større operasjon >= 4 uker fra registrering
Samtidig behandling med Rifampin eller johannesurt er ikke tillatt; Pasienter bør seponere disse legemidlene minst 4 uker før registrering

Betydelig historie med hjertesykdom er ikke tillatt:
- Kongestiv hjertesvikt > klasse II New York Heart Association (NYHA)
- Hjerteinfarkt innen 6 måneder før registrering
- Alvorlig myokarddysfunksjon, definert som scintigrafisk (multigated acquisition scan [MUGA], myocardial scintigram) bestemt absolutt venstre ventrikkel ejeksjonsfraksjon (LVEF) under 45 % eller en LVEF på ekkokardiogram (ECHO) under normalgrensen ved den enkelte institusjonen
Ingen historie med blødende diatese
Pasienter som får antiretroviral kombinasjonsbehandling for humant immunsviktvirus (HIV) er ekskludert fra studien
