Denne randomiserte fase III studien gir everolimus sammen med bevacizumab for å se hvor godt det virker sammenlignet med everolimus alene ved behandling av pasienter med avansert nyrekreft som utviklet seg etter førstelinjebehandling. Everolimus kan stoppe veksten av tumorceller ved å blokkere noen av enzymene som trengs for cellevekst. PRIMÆRE MÅL:

l. For å sammenligne den totale overlevelsen til pasienter som får bevacizumab pluss everolimus og everolimus alene blant pasienter med avansert nyrecellekarsinom som utvikler seg etter førstelinjebehandling av vaskulær epidermal vekstfaktorreseptor (VEGFR)-tyrosinkinasehemmer (TKI).

SEKUNDÆRE MÅL:

I. For å sammenligne den progresjonsfrie overlevelsen og andelen som opplever en objektiv respons (definert som fullstendig klinisk respons [cCR] + delvis respons [PR]) hos pasienter med avansert nyrecellekarsinom som får bevacizumab pluss everolimus og everolimus alene.

II. For å sammenligne grad 3 eller høyere toksisitet hos pasienter som får hvert behandlingsregime.

OVERSIKT: Pasienter er randomisert til 1 av 2 behandlingsarmer.

ARM I: Pasienter får everolimus oralt (PO) en gang daglig (QD) på dag 1-28.

ARM II: Pasienter får everolimus PO QD på dag 1-28 og bevacizumab intravenøst ​​(IV) over 30-90 minutter på dag 1 og 15.

I begge armer gjentas kursene hver 28. dag i fravær av sykdomsprogresjon eller uakseptabel toksisitet.

Inklusjonskriterier:

  • Nyrecellekarsinom med en eller annen komponent av klarcellet histologi
  • Metastatisk eller uoperabel sykdom
  • Må ha blitt behandlet med minst 1 tidligere behandling med VEGFR tyrosinkinasehemmer og ha utviklet seg eller vært intolerant overfor behandling
  • Ingen tidligere systemisk behandling med et bindemiddel for vaskulær endotelial vekstfaktor (VEGF) (f.eks. bevacizumab)
  • Ingen tidligere systemisk terapi med noen mekanistisk mål for rapamycin (mTOR)-hemmere (f.eks. sirolimus, temsirolimus, everolimus)
  • Tidligere cytokinbehandling er tillatt
  • Eventuell systemisk behandling må fullføres minst 4 uker før registrering
  • >= 2 uker siden tidligere stråling (inkludert palliativ)

  • Pasienter må ikke ha hatt en større kirurgisk prosedyre, åpen biopsi eller betydelig traumatisk skade innen 4 uker før studieregistrering, og må ha kommet seg helt etter en slik prosedyre

    • Følgende regnes ikke for å være store prosedyrer: thoracentese, paracentese, portplassering, laparoskopi, thorakoskopi, bronkoskopi, endoskopiske ultrasonografiske prosedyrer, mediastinoskopi, hudbiopsier, snittbiopsier og rutinemessige tannprosedyrer
  • Pasienter må ha målbar sykdom etter RECIST-kriterier; lesjoner som kan måles nøyaktig i minst én dimensjon (lengste diameter som skal registreres) som >= 2 cm med konvensjonelle teknikker eller som >= 1 cm med spiral computertomografi (CT) skanning

  • Ingen aktive hjernemetastaser: Pasienter med behandlede, stabile hjernemetastaser i minst tre måneder er kvalifisert så lenge de oppfyller følgende kriterier:

    • Behandlede hjernemetastaser er definert som å ikke ha noe pågående behov for steroider og ingen tegn på progresjon eller blødning etter behandling i minst 3 måneder, som konstatert ved klinisk undersøkelse og hjerneavbildning (magnetisk resonanstomografi [MRI] eller CT) (stabil dose av antikonvulsiva midler) er tillatt); behandling for hjernemetastaser kan omfatte strålebehandling av hele hjernen (WBRT), strålekirurgi (RS; Gamma Knife, lineær akselerator [LINAC] eller tilsvarende) eller en kombinasjon som anses hensiktsmessig av den behandlende legen; Pasienter med metastaser i sentralnervesystemet (CNS) behandlet med nevrokirurgisk reseksjon eller hjernebiopsi utført innen 3 måneder før dag 1 er ikke kvalifisert
    • Baseline hjerneavbildning (MR/CT) er nødvendig
  • Ingen alvorlige ikke-helende sår, sår eller benbrudd

  • Ingen arterielle trombotiske hendelser innen 6 måneder etter registrering:

    • Inkludert forbigående iskemisk angrep (TIA), cerebrovaskulær ulykke (CVA), perifer arteriell trombe, ustabil angina eller angina som krever kirurgisk eller medisinsk intervensjon de siste 6 månedene, eller hjerteinfarkt (MI); pasienter med klinisk signifikant perifer arteriesykdom (dvs. claudicatio på mindre enn én blokk), signifikant vaskulær sykdom (dvs. aortaaneurisme, aortadisseksjonshistorie) eller andre arterielle trombotiske hendelser er ikke kvalifiserte
  • Pasienter som har opplevd dyp venetrombose eller lungeemboli i løpet av de siste 6 månedene må ha stabil terapeutisk antikoagulasjon for å bli registrert i denne studien
  • Pasienter som får blodplatehemmende midler og profylaktisk antikoagulasjon er kvalifisert
  • Ingen utilstrekkelig kontrollert hypertensjon: (definert som et blodtrykk på >= 160 mmHg systolisk og/eller >= 90 mmHg diastolisk på medisiner), eller noen tidligere historie med hypertensiv krise eller hypertensiv encefalopati

  • Ingen kjent alvorlig svekkelse av lungefunksjonen, definert som >= grad 2 dyspné eller hoste, eller enten:

    • Behov for ekstra oksygen, eller
    • I tilfeller der lungefunksjons- eller pulsoksymetritester er oppnådd, er tvungen ekspirasjonsvolum av lungen i løpet av ett sekund (FEV1) eller tvungen vitalkapasitet (FVC) < 50 % av forutsagt, eller enkeltpustdiffunderende kapasitet i lungen for karbonmonoksid (DLCO) er < 35 % av antatt oksygenmetning i hvilerommet er mindre enn 90 %
  • Ingen aktiv eller alvorlig leversykdom (f.eks. akutt eller kronisk hepatitt, skrumplever)
  • Ingen positiv serologi for anti-hemoglobin C (HBC) eller anti-hepatitt C virus (HCV) antistoffer; hepatitt B-virus (HBV) seropositive pasienter (hepatitt B overflateantigen [HBsAg] positive) er kvalifisert hvis de overvåkes nøye for bevis på aktiv HBV-infeksjon ved HBV deoksryribonukleinsyre (DNA)-testing og samtykker i å motta suppressiv behandling med lamivudin eller annen HBV - suppressiv behandling inntil minst 4 uker etter siste dose everolimus

  • Ingen historie med abdominal fistel, gastrointestinal perforering eller intraabdominal abscess innen 6 måneder før oppstart av behandling
  • Ingen pågående immunsuppressiv behandling: inkludert kronisk systemisk behandling med kortikosteroider (>= 10 mg/dag prednisonekvivalent)
  • Arkivvev må være tilgjengelig for innsending: selv om det er valgfritt pasienter å velge å delta i de korrelative delstudiene eller ikke

  • Pasienter som er gravide eller ammer er ikke kvalifisert
