En randomisert fase III, to-arm studie av paklitaksel/karboplatin/vedlikeholdsletrozol versus letrozol monoterapi hos pasienter med stadium II-IV, primært lavgradig serøst karsinom i eggstokken eller peritoneum

Denne fase III-studien studerer hvor godt letrozol med eller uten paklitaksel og karboplatin virker ved behandling av pasienter med stadium II-IV lavgradig serøst karsinom i eggstokken, egglederen eller bukhinnen. Letrozol er en enzymhemmer som reduserer mengden østrogen laget av kroppen, som igjen kan stoppe veksten av tumorceller som trenger østrogen for å vokse. Legemidler som brukes i kjemoterapi, som paklitaksel og karboplatin, virker på forskjellige måter for å stoppe veksten av tumorceller, enten ved å drepe cellene, ved å stoppe dem fra å dele seg eller ved å hindre dem i å spre seg. HOVEDMÅL:

I. For å undersøke om letrozol monoterapi/vedlikehold (L/L) ikke er dårligere enn intravenøs (IV) paklitaksel/karboplatin og vedlikeholdsletrozol (CT/L) med hensyn til progresjonsfri overlevelse (PFS) hos kvinner med stadium II- IV primært lavgradig serøst karsinom i eggstokken eller peritoneum etter primær kirurgisk cytoreduksjon.

SEKUNDÆRE MÅL:

I. For å sammenligne arten, frekvensen og maksimal grad av toksisitet som vurdert av Common Terminology Criteria for Adverse Events (CTCAE) versjon (v) 5.0 for hver behandlingsarm.

II. For å sammenligne den relative frekvensen av objektiv tumorrespons hos de med målbar sykdom etter cytoreduktiv kirurgi for hver behandlingsarm.

III. For å sammenligne total overlevelse for hver behandlingsarm. IV. For å sammenligne CT/L- og L/L-armene med hensyn til pasienters overholdelse av letrozol-behandling målt ved antall pille.

OVERSIGT: Pasienter randomiseres til 1 av 2 armer.

ARM I: Pasienter får paklitaksel IV over 3 timer og karboplatin IV på dag 1. Sykluser gjentas hver 21. dag i opptil 6 sykluser. Pasienter får deretter letrozol oralt (PO) én gang daglig (QD) i fravær av sykdomsprogresjon eller uakseptabel toksisitet.

ARM II: Pasienter får letrozol PO QD i fravær av sykdomsprogresjon eller uakseptabel toksisitet.

Etter avsluttet studiebehandling følges pasientene opp hver 3. måned i 2 år, hver 6. måned i 3 år, deretter årlig.

Inklusjonskriterier:

Pasienter må ha psykologisk evne og generell helse som tillater gjennomføring av studiekravene og nødvendig oppfølging
Alle kvinner vil per definisjon anses som overgangsalder på grunn av kirurgisk fjerning av begge eggstokkene før prøveregistrering
Pasienter må ha nydiagnostisert stadium II-IV lavgradig serøs eggstokkreft (innlevering av patologirapport(er) kreves). Eggstokkreft = kreft i eggstokkene, egglederne og primære peritoneale kreftformer. p53-immunhistokjemi (IHC) er nødvendig og må vise ikke-avvikende mønster (ikke-avvikende p53-uttrykk er i samsvar med normal/villtype TP53). Hvis avvikende p53-ekspresjon blir funnet på p53 IHC, er pasienten IKKE kvalifisert (avvikende p53-ekspresjon er i samsvar med mutant TP53 og støtter diagnose av høygradig serøs eggstokkreft). En kopi av patologirapporten som inkluderer diagnosen lavgradig serøs eggstokkreft og ikke-aberrant p53 IHC-resultat må sendes inn i RAVE
Passende stadium for studieoppføring basert på følgende diagnostiske opparbeidelse:
- Anamnese/fysisk undersøkelse innen 14 dager før registrering
- Kontrastforsterket avbildning av bryst, mage og bekken innen 28 dager før registrering
Pasienter må ha gjennomgått et forsøk på maksimal forhåndscytoreduktiv kirurgi, med enten optimal (=< 1 cm diameter gjenværende sykdom/knute) eller suboptimal gjenværende sykdom (> 1 cm diameter gjenværende sykdom/knute) status tillatt
Pasienter må ha gjennomgått en bilateral salpingo-ooforektomi

Ekskluderingskriterier:

Pasienter med tidligere eller samtidig malignitet hvis naturhistorie eller behandling ikke har potensial til å forstyrre sikkerhets- eller effektvurderingen av undersøkelsesregimet, er kvalifisert for denne studien
Pasienter kan ikke ha mottatt neoadjuvant kjemoterapi eller strålebehandling for behandling av denne sykdommen
Pasienter kan ikke ha mottatt tidligere hormonbehandling for behandling av denne sykdommen
Pasienter med kjent overfølsomhet overfor letrozol eller overfølsomhet/intoleranse overfor karboplatin/paklitakselbehandling
Pasienter med alvorlig hjertesykdom:
- Hjerteinfarkt eller ustabil angina innen 6 måneder før registrering
- New York Heart Association (NYHA) klasse II eller høyere kongestiv hjertesvikt
Pasienter med kjente metastaser i sentralnervesystemet
Pasienter med aktiv (unntatt ukomplisert urinveisinfeksjon) eller ukontrollert systemisk infeksjon
Pasienter med >= grad 2 baseline nevropati
Kjente humant immunsviktvirus (HIV)-infiserte pasienter på effektiv antiretroviral terapi med uoppdagbar virusmengde innen 6 måneder er kvalifisert for denne studien

Progresjonsfri overlevelse (PFS) Tidsramme: Tid fra randomisert behandlingsoppdrag til dokumentasjon av sykdomsprogresjon (Response Evaluation Criteria in Solid Tumors 1.1) eller død uansett årsak, avhengig av hva som kommer først, vurdert opp til 8 år	PFS-sammenligningen vil bli vurdert ved andre interims- og sluttanalyse ved hjelp av en logrank-test stratifisert etter land og gjenværende sykdomsstatus. Estimater for fareforholdet letrozol/letrozol (L/L) vs. paklitaksel/karboplatin/letrozol (CT/L) og dets konfidensintervall vil bli oppnådd ved bruk av en stratifisert Cox proporsjonal faremodell. Potensielle forvirrende faktorer, inkludert stratifiseringsfaktorene, prestasjonsstatus og selverklært rasebetegnelse vil bli vurdert i en endelig utforskende modell. Overholdelse av letrozol vedlikeholdsbehandling Tidsramme: Ved syklus 1, 6 og 12	Kvantifiseres som gjennomsnittlig overholdelsesgrad (MRA), beregnet som andelen av produktet som ikke er returnert delt på antall dager siden forrige besøk hvor studieproduktene ble utlevert. For tiden leveres letrozol i en 2,5 mg pille, og den anbefalte dosen er 2,5 mg per dag. Dette utfallet vil bli beregnet for hver behandlingssyklus (vanligvis 21 dager). Gjennomsnittlig MRA vil bli sammenlignet mellom de randomiserte behandlingsgruppene ved bruk av lineære blandede modellmetoder.
