Denne fase III-studien sammenligner effekten av kombinasjonen av ramucirumab og pembrolizumab versus standardbehandling kjemoterapi for behandling av ikke-småcellet lungekreft som er stadium IV eller som har kommet tilbake etter en periode med bedring (tilbakevendende). SEKUNDÆR MÅL:

I. For å oppsummere rapporter om alvorlige og uventede høygradige (>= grad 3) behandlingsrelaterte bivirkninger bestemt av behandlende lege innenfor hver behandlingsarm.

Inklusjonskriterier:

  • Deltakerne må ha histologisk eller cytologisk bekreftet ikke-småcellet lungekreft (NSCLC) som er stadium IV eller tilbakevendende
  • Deltakerne må ha mottatt minst én linje med anti-PD-1- eller anti-PD-L1-behandling for alle stadier av NSCLC. Anti-PD-1 eller anti-PD-L1 kan ha blitt gitt alene eller i kombinasjon med annen behandling
  • Deltakerne må ha opplevd sykdomsprogresjon (etter den behandlende legens mening) mer enn (>) 84 dager etter oppstart (syklus 1 dag 1) av deres siste anti-PD-1- eller PD-L1-behandling
  • Deltakere som mottok anti-PD-1- eller anti-PD-L1-behandling for stadium IV eller tilbakevendende sykdom, må ha hatt best respons på anti-PD-1- eller anti-PD-L1-behandling med stabil, delvis respons eller fullstendig respons ( etter den behandlende legens mening)
  • Deltakere som mottok neoadjuvant, adjuvant og/eller konsoliderende anti-PD-1- eller anti-PD-L1-behandling som sin eneste linje med anti-PD-1- eller anti-PD-L1-behandling, må ha opplevd sykdomsprogresjon innen (=<) 365 dager fra oppstart (syklus 1 dag 1) med anti-PD-1- eller PD-L1-behandling
  • Deltakerne må ha mottatt platinabasert kjemoterapi og opplevd sykdomsprogresjon (etter den behandlende legens mening) under eller etter dette regimet
  • Deltakere med en kjent sensibiliserende mutasjon som det eksisterer en Food and Drug Administration (FDA)-godkjent målrettet behandling for NSCLC (f.eks. EGFR, ALK, ROS1, BRAF, RET, NTRK, KRAS, HER2 og MET sensibiliserende mutasjoner), må tidligere ha mottatt minst én av de godkjente terapiene. Forutgående målrettet terapi for deltakere med målrettede endringer er tillatt hvis alle andre kvalifikasjonskriterier også er oppfylt
  • Deltakere må være >= 18 år
  • Deltakerne må trygt kunne motta den undersøkelsesmedisinske kombinasjonen og etterforskerens valg av standardbehandlingsregimer i henhold til gjeldende FDA-godkjente pakningsvedlegg(er), behandlende etterforskers skjønn og institusjonelle retningslinjer
  • Deltakere må ha Zubrod prestasjonsstatus på 0-2

Ekskluderingskriterier:

  • Deltakere må ikke ha mottatt mer enn én linje med anti-PD-1 eller anti-PD-L1 for stadium IV eller tilbakevendende sykdom
  • Deltakere må ikke motta eller planlegge å motta en annen undersøkelsesterapi under studiedeltakelsen

Tidsramme: Fra dato for randomisering til dato for død på grunn av en hvilken som helst årsak, vurdert opp til 3 år
Vurdert hos deltakere med stadium IV eller tilbakevendende ikke-småcellet lungekreft med ervervet resistens mot anti-PD-1 eller anti-PD-L1 terapi randomisert til ramucirumab og pembrolizumab eller standardbehandling kjemoterapi. Vil bruke en intention-to-treat-analyse og sammenligningen vil bli gjort ved hjelp av en stratifisert log-rank test. Fordelingen av total overlevelse vil bli estimert ved hjelp av metoden til Kaplan-Meier, og en Cox proporsjonale faremodell vil bli brukt for å estimere fareforhold og tilhørende 95 % konfidensintervaller for å estimere behandlingseffekter.
