Studie tamoxifenu a raloxifenu (STAR) pro prevenci rakoviny prsu

Odůvodnění: Estrogen může stimulovat růst buněk rakoviny prsu. Hormonální terapie pomocí raloxifenu a tamoxifenu může bojovat proti rakovině prsu blokováním vychytávání estrogenu nádorovými buňkami.

ÚČEL: Randomizovaná dvojitě zaslepená klinická studie k porovnání účinnosti raloxifenu s účinností tamoxifenu v prevenci rakoviny prsu u postmenopauzálních žen.

CÍLE:

Zjistěte, zda je raloxifen více či méně účinný než tamoxifen při významném snížení incidence invazivního karcinomu prsu u postmenopauzálních žen.
Vyhodnoťte účinky tamoxifenu a raloxifenu na výskyt intraduktálního karcinomu in situ, lobulárního karcinomu in situ, karcinomu endometria, ischemické choroby srdeční, zlomenin kyčle a páteře nebo Collesových zlomenin zápěstí u těchto účastníků.
Vyhodnoťte toxické účinky těchto režimů u těchto účastníků.
Zjistit vliv těchto režimů na kvalitu života těchto účastníků (ve vybraných centrech). (Hodnocení kvality života uzavřeno s akruálním účinkem k 31. 5. 2001.)

PŘEHLED: Toto je randomizovaná, dvojitě zaslepená studie. Účastníci jsou stratifikováni podle věku (35 až 49 vs. 50 až 59 vs. nad 59), rasy (černí vs. bílí vs. ostatní), historie lobulárního karcinomu in situ (ano vs ne), předchozí hysterektomie (ano vs ne) a odhadované absolutní riziko invazivního karcinomu prsu do 5 let (podle Gailova modelu) (méně než 2,0 vs 2,0-2,9 vs 3,0-4,9 oproti 5.0 nebo vyšší). Účastníci jsou náhodně rozděleni do 1 ze 2 ramen.

Rameno I: Účastníci dostávají perorálně tamoxifen plus placebo denně po dobu 5 let.
Rameno II: Účastníci dostávají perorálně raloxifen plus placebo denně po dobu 5 let. Kvalita života se posuzuje (ve vybraných centrech) na začátku a v 6., 12., 18., 24., 30., 36., 42., 48., 54., 60. a 72. měsíci. (Hodnocení kvality života uzavřeno s akruálním účinkem k 31. 5. 2001.)

Účastníci jsou sledováni každoročně po 5 letech.

CHARAKTERISTIKA ONEMOCNĚNÍ:

Ženy po menopauze se zvýšeným rizikem rozvoje invazivní rakoviny prsu, které splňují jedno z následujících kritérií:
- Minimálně 12 měsíců od spontánního menstruačního krvácení
- Dříve dokumentovaná hysterektomie a bilaterální salpingo-ooforektomie
- Nejméně 55 let s předchozí hysterektomií s ooforektomií nebo bez ní
- Věk 35 až 54 let s předchozí hysterektomií bez ooforektomie NEBO s neznámým stavem vaječníků s dokumentovanou hladinou folikuly stimulujícího hormonu prokazující zvýšení v postmenopauzálním rozmezí
Histologicky potvrzený lobulární karcinom in situ léčený pouze lokální excizí NEBO minimální předpokládaná 5letá pravděpodobnost invazivního karcinomu prsu alespoň 1,66 % pomocí profilu hodnocení rizika rakoviny prsu
Žádný klinický důkaz malignity na fyzickém vyšetření během posledních 180 dnů
Žádné známky podezřelého nebo maligního onemocnění na bilaterálním mamografu za poslední rok
Žádná bilaterální nebo jednostranná profylaktická mastektomie
Žádný předchozí invazivní karcinom prsu nebo intraduktální karcinom in situ
