ODŮVODNĚNÍ: Léky používané v chemoterapii používají různé způsoby, jak zastavit dělení nádorových buněk, takže přestanou růst nebo zemřou. Radiační terapie využívá vysokoenergetické rentgenové záření k poškození nádorových buněk. Kombinace chemoterapie, radiační terapie a chirurgického zákroku může zabít více nádorových buněk.

ÚČEL: Studie fáze II studovat účinnost kombinované chemoterapie, radiační terapie a chirurgického zákroku při léčbě pacientů s primárním nebo recidivujícím sarkomem.

CÍLE:

Stanovte celkové přežití pacientů s primárními nebo recidivujícími retroperitoneálními sarkomy léčených neoadjuvantním doxorubicinem a ifosfamidem, radioterapií a chirurgickou resekcí.
Posuďte lokálně-regionální kontrolu u pacientů léčených tímto režimem.
Určete přežití bez onemocnění u pacientů léčených tímto režimem.
Stanovte patologickou odpověď u pacientů léčených tímto režimem.
Určete toxické účinky tohoto režimu u těchto pacientů.
Určete komplikace u pacientů léčených tímto režimem.

Přehled: Pacienti dostávají doxorubicin IV nepřetržitě v den 1-3 a ifosfamid IV po dobu 3 hodin ve dnech 1-4. Pacienti také dostávají filgrastim (G-CSF) počínaje 5. dnem a pokračují, dokud se krevní obraz neupraví. Léčba se opakuje každých 21 dní po maximálně 4 cykly v nepřítomnosti nepřijatelné toxicity. Počínaje 2-4 týdny po chemoterapii pacienti podstupují radioterapii denně, 5 dní v týdnu, po dobu 5-6 týdnů. Počínaje 4-7 týdny po radioterapii pacienti podstupují kompletní chirurgickou resekci s intraoperační nebo pooperační radioterapií nebo brachyterapií.

Pacienti jsou sledováni každé 3 měsíce po dobu 2 let, každých 6 měsíců po dobu 3 let a poté každý rok.

PŘEDPOKLÁDANÝ AKRUÁLNÍ AKRUÁLNÍ: Do této studie bude během 24 měsíců nashromážděno celkem 48 pacientů.

CHARAKTERISTIKA ONEMOCNĚNÍ:

Histologicky potvrzený primární nebo recidivující sarkom měkkých tkání retroperitonea nebo pánve
Žádný rhabdomyosarkom, extraoseální Ewingův sarkom, primitivní neuroektodermální nádor, osteosarkom, chondrosarkom, Kaposiho sarkom nebo agresivní fibromatóza
Žádný sarkom, který neumožňuje ušetřit dvě třetiny 1 ledviny v plánovaném ozařovacím poli
Radiograficky měřitelné onemocnění větší než 5 cm (T2) pomocí CT nebo MRI břicha a pánve
Onemocnění vysokého stupně (stupeň 3/3, 3/4, 4/4) větší než 5 cm NEBO
Recidivující onemocnění středního stupně (stupeň 2/3, 2/4) větší než 10 cm
Vhodné pro hrubou celkovou resekci (R0 nebo R1)
Žádná předchozí resekce mezisoučtu (R2).
Parciální debulking NEBO subtotální resekce tumoru s reziduálním makroskopickým onemocněním
Méně než 4 nejednoznačné plicní léze, každá menší než 3 mm v průměru podle CT skenu
Žádné multifokální onemocnění svědčící pro regionální postižení uzlin
Žádné metastázy

Žádné městnavé srdeční selhání
Žádný infarkt myokardu za posledních 6 měsíců
Žádné srdeční onemocnění třídy III nebo IV podle New York Heart Association
Ejekční frakce alespoň 50 %

Jiný:

Žádná jiná malignita za posledních 5 let kromě chirurgicky léčeného karcinomu in situ děložního čípku nebo bazocelulárního nebo spinocelulárního karcinomu kůže
Žádné vážné zdravotní nebo psychiatrické onemocnění, které by bránilo vstupu do studia
Žádná zjevná obstrukce střev
Žádná přecitlivělost na produkty odvozené od E. coli
Ne těhotná nebo kojící
Negativní těhotenský test
Plodné pacientky musí používat účinnou antikoncepci
