Meziskupinová studie fáze III adjuvantní chemoradiace po resekci adenokarcinomu žaludku nebo gastroezofageálního adenokarcinomu

ODŮVODNĚNÍ: Léky používané v chemoterapii používají různé způsoby, jak zastavit dělení nádorových buněk, takže přestanou růst nebo zemřou. Radiační terapie využívá vysokoenergetické rentgenové záření k poškození nádorových buněk. Kombinace chemoterapie s radiační terapií po chirurgickém zákroku může zabít jakékoli zbývající nádorové buňky po operaci. Dosud není známo, který režim chemoterapie a radiační terapie je účinnější při léčbě rakoviny žaludku nebo jícnu.

ÚČEL: Randomizovaná studie fáze III porovnat dva různé režimy chemoterapie a radiační terapie při léčbě pacientů, kteří podstoupili operaci rakoviny žaludku nebo jícnu.

CÍLE:

  • Porovnejte celkové přežití u pacientů s resekovaným adenokarcinomem žaludku léčených epirubicinem, cisplatinou a infuzním fluorouracilem (5-FU) vs 5-FU bolus a leukovorin kalcium před a po 5-FU plus radioterapii.
  • Porovnejte přežití bez onemocnění a míru lokální a vzdálené recidivy u těchto pacientů léčených těmito režimy.
  • Korelujte expresi předpokládaných prognostických markerů (včetně TS, ERCC-1, MSI, E-cadherinu, EGFR, p27, COX-2 a c-erbB-2) s celkovým přežitím pacientů léčených těmito režimy.
  • Korelujte specifické polymorfismy zárodečné linie související s metabolismem a rezistencí chemoterapie (včetně polymorfismů UGT2B7 [epirubicin], GST [cisplatina], ERCCI [cisplatina], XRCC1 [cisplatina], TS [5-FU], DPD [5-FU] a EGFR) s toxicitou související s léčbou a celkovým přežitím těchto pacientů.
  • Korelujte sérové ​​hladiny inzulinu podobného růstového faktoru-1 (IGF-1), IGF-2 a IGF-binding protein 3 s celkovým přežitím pacientů léčených těmito režimy.

PŘEHLED: Toto je randomizovaná, multicentrická studie. Pacienti jsou stratifikováni podle hloubky penetrace nádoru (T1 nebo T2 vs. T3 vs. T4), postižení lymfatických uzlin (0 vs. 1-3) a rozsahu lymfadenektomie (D1 nebo D2 vs. D0 nebo neznámé). Pacienti jsou randomizováni do 1 ze 2 léčebných ramen.

  • Rameno I: Pacienti dostávají leukovorin kalcium IV a fluorouracil (5-FU) IV ve dnech 1-5 kúry 1, 3 a 4. Cykly se opakují každých 28 dní. Během 2. kurzu pacienti podstupují radioterapii 5 dní v týdnu a dostávají 5-FU IV nepřetržitě po dobu 5 týdnů. Mezi 2. a 3. kúrou pacienti odpočívají 28–35 dní.
  • Rameno II: Pacienti dostávají epirubicin IV po dobu 3-15 minut a cisplatinu IV po dobu 1 hodiny v den 1 a 5-FU IV nepřetržitě ve dnech 1-21 během kurzu 1. Počínaje 1 týdnem pacienti podstupují radioterapii 5 dní v týdnu a dostávají 5-FU IV nepřetržitě po dobu 5 týdnů. Před zahájením 2. cyklu chemoterapie pacienti odpočívají 28–35 dní. Pacienti pak dostávají epirubicin, cisplatinu a 5-FU jako v kúře 1. Léčba se opakuje každých 21 dní ve 2 cyklech.

Pacienti jsou sledováni každé 3 měsíce po dobu 2 let, každé 4 měsíce po dobu 2 let a poté každoročně po dobu 3 let.

  1. Požadované parametry nádoru

    1.1 Pacienti musí mít histologicky diagnostikovaný adenokarcinom žaludku nebo gastroezofageální junkce. Adenokarcinomy jícnu, které nezahrnují gastroezofageální junkci, nejsou vhodné.

    1.2 Pacienti museli podstoupit en bloc resekci všech známých nádorů. Chirurgická resekce musela být provedena s kurativním záměrem.

    1.3 Pacienti musí mít rozšíření tumoru za muscularis propria a/nebo postižení uzlin bez známek onemocnění M1.

    • Pacienti mohou mít stadia IB, pokud existuje důkaz buď o pozitivním onemocnění uzlin (N1) nebo rozšíření tumoru za muscularis propria (tj. pacienti s T1, N1, M0 jsou způsobilí, ale pacienti s T2, N0, M0 jsou povoleni pouze v případě, že prodloužení za muscularis propria).
    • Pacienti mohou mít stadia II, IIIA, IIIB nebo stadium IV s M0 (tj. T4N2M0).

    1.4 Pacienti se známým neresekovaným karcinomem, recidivujícím karcinomem, mikroskopickým průkazem tumoru na distální nebo proximální linii resekce žaludku, nesouvislou resekcí tumoru nebo M1 (metastatickým) onemocněním nejsou vhodní.

  2. Předchozí terapie

    2.1 Není povolena žádná předchozí terapie (kromě hormonální nebo biologické) jiných malignit, s výjimkou adekvátně léčeného bazaliomu nebo spinocelulárního karcinomu kůže, neinvazivního karcinomu in situ, který byl plně resekován, nebo jiného karcinomu, pro který byl pacient pět let bez onemocnění. let.

    2.2 Pacienti, kteří podstoupili jakoukoli předchozí chemoterapii nebo radioterapii, nejsou způsobilí.

  3. Charakteristika pacienta

    3.1 Pacienti musí mít výkonnostní stav ECOG (CTC) 0, 1 nebo 2.

    3.2 Od pacientů se požaduje, aby měli adekvátní celkový kalorický příjem, který jim umožní udržet si pooperační tělesnou hmotnost. Pacienti musí mít dokumentaci o stabilní váze (nebo o snížení hmotnosti nižší než 2 libry) alespoň jeden týden před registrací.

    3.3 Všichni pacienti musí být vyšetřeni radiačním onkologem (před zařazením), aby se zajistilo, že pacient je vhodným kandidátem na radiační terapii.

    3.4 Pacienti nemusí mít jednostrannou renální funkci (pouze jedna funkční ledvina), jak bylo zjištěno pomocí CT skenu s kontrastem, urogramu, renálního skenu nebo jiné studie.

    3.5 Těhotné a kojící ženy se nemohou zúčastnit. Muži a ženy s reprodukčním potenciálem se nemohou zúčastnit, pokud během této studie nesouhlasili s používáním účinné antikoncepční metody nebo praktikováním abstinence.

    • Je známo, že účinky terapeutické radioterapie jsou teratogenní.
    • Účinky epirubicinu, cisplatiny a 5-FU na vyvíjející se lidský plod při doporučené terapeutické dávce jsou méně známé.
