Studie prevence statinových polypů u pacientů s resekovanou rakovinou tlustého střeva

Odůvodnění: Rosuvastatin může zastavit růst nádorových buněk blokováním některých enzymů potřebných pro růst buněk. Podávání rosuvastatinu po operaci může zabít jakékoli nádorové buňky, které po operaci zůstanou. Může také zabránit vzniku polypů nebo návratu rakoviny tlustého střeva. Dosud není známo, zda je rosuvastatin při léčbě rakoviny tlustého střeva, která byla odstraněna chirurgicky, účinnější než placebo.

ÚČEL: Tato randomizovaná studie fáze III studuje rosuvastatin s cílem zjistit, jak dobře funguje ve srovnání s placebem při léčbě pacientů s rakovinou tlustého střeva stadia I nebo stadia II, která byla odstraněna chirurgicky.

  • Porovnat účinek rosuvastatinu vs. placebo na 5letý výskyt adenomatózních polypů tlustého střeva nebo rekta, metachronního kolorektálního karcinomu nebo recidivy karcinomu tlustého střeva (APMC+R) u pacientů s resekovaným karcinomem tlustého střeva stadia I nebo II.

Sekundární

  • Zjistit, zda je účinek rosuvastatinu oproti placebu stejně velký u pacientů užívajících aspirin (bez ohledu na dávku) ve srovnání s pacienty, kteří aspirin neužívají.
  • Chcete-li zjistit, zda užívání aspirinu (bez ohledu na dávku) vs. žádný aspirin sníží výskyt nebo APMC+R, a pokud existuje účinek, prozkoumat vztah k dávce.
  • Zjistit účinek rosuvastatinu u pacientů s familiárním kolorektálním karcinomem.
  • Stanovit účinek rosuvastatinu u pacientů s mikrosatelitně nestabilními nádory (tj. nádory vykazující ztrátu exprese MLH1 nebo MSH2 prostřednictvím IHC).
  • Stanovit vztah mezi terapií rosuvastatinem a rysy kolorektálních adenomů a také velikostí a počtem kolorektálních adenomů.
  • Porovnat dobu do APMC+R u pacientů léčených rosuvastatinem vs.
  • Porovnat přežití bez onemocnění u pacientů léčených rosuvastatinem vs.
  • Porovnat celkové přežití pacientů léčených rosuvastatinem vs.
  • Porovnat míru recidivy karcinomu tlustého střeva u pacientů léčených rosuvastatinem vs.
  • Porovnat četnost druhých nekolorektálních primárních karcinomů u pacientů léčených rosuvastatinem vs.
  • Stanovit účinek rosuvastatinu na kvalitu života související se zdravím, celkovou kvalitu života a symptomy hlášené pacienty.
  • Porovnat výskyt a závažnost nežádoucích účinků spojených s rosuvastatinem vs.
  • K posouzení relevantních nádorových a krevních markerů, které mohou ovlivnit metabolismus, aktivitu nebo účinek studovaných léků, jako je HMG-CoA reduktáza, UGT1A6, P450-2C9, PTGS2 (COX-2) a další možné markery.

PŘEHLED: Toto je multicentrická studie. Pacienti jsou stratifikováni podle rodinné anamnézy příbuzného prvního stupně s kolorektálním karcinomem (ano vs ne), zamýšlené dávky aspirinu (žádná vs 81 mg vs 325 mg) a adjuvantní terapie rakoviny tlustého střeva (ano vs ne). Pacienti jsou randomizováni do 1 ze 2 léčebných ramen.

  • Skupina 1: Pacienti dostávají perorální placebo jednou denně po dobu 5 let.
  • Skupina 2: Pacienti dostávají perorálně rosuvastatin jednou denně po dobu 5 let.

Pacienti mohou vyplnit dotazník kvality života na začátku a po 6, 12, 36, 60 a 84 měsících.

Vzorky nádorové tkáně, séra a krve mohou být pravidelně odebírány pro biomarkerové a jiné analýzy.

Po ukončení studijní léčby jsou pacienti pravidelně sledováni po dobu až 2 let.

  • Pacienti musí mít výkonnostní stav Eastern Cooperative Oncology Group (ECOG) 0 nebo 1.
