Hodnocení mamografického účinku aspirinu na hustotu prsu: Doprovodná studie k alianční studii A011502

Tato studie fáze III hodnotí mamografickou hustotu prsu u účastnic s rakovinou prsu s negativním hormonálním receptorem zařazeným do studie A011502. Ukázalo se, že vysoká hustota prsů je silným rizikovým faktorem pro rozvoj rakoviny prsu a snížení hustoty prsu může snížit riziko rakoviny prsu. Účastníci léčení aspirinem mohou na mamografu vykazovat sníženou hustotu prsou.

PRIMÁRNÍ CÍLE I. Porovnat 1letou mamografickou denzitu prsu v kontralaterálním (nepostiženém) prsu mezi rameny s aspirinem a placebem u pacientek s karcinomem prsu s negativním nebo pozitivním hormonálním receptorem zařazeným do A011502.

DRUHÉ CÍLE I. Porovnat 2letou mamografickou denzitu prsu v kontralaterálním (nepostiženém) prsu mezi rameny s aspirinem a placebem u pacientek s karcinomem prsu s negativním nebo pozitivním hormonálním receptorem zařazeným do A011502.

OBRYS:

Účastníci poskytují bilaterální screeningový mamogram pořízený před veškerou léčbou rakoviny a do 8 týdnů před registrací na A011502 a roční bilaterální mamograf co nejblíže 1 roku po registraci na A011502 a co nejblíže 2 roky po registraci na A011502 . Účastníci také podstoupí odběr vzorků krve a údajů o menstruačním cyklu do 2 týdnů po registraci a 1 a 2 roky po registraci do A011502.

Po dokončení studia jsou účastníci každoročně sledováni po dobu 10 let od registrace do studie A011502.

Studijní populace

Účastníci s negativní nebo pozitivní rakovinou prsu na hormonální receptor se zapsali do Alliance A011502.

Popis

Kritéria pro zařazení:

  • Musíte být buď souběžně nebo dříve zapsáni do Alliance A011502. Vhodní pacienti mohou být buď před menopauzou nebo po menopauze.
  • Pacientky musí mít buď karcinom prsu s negativním hormonálním receptorem, nebo pacientky s ER+, které dokončily hormonální terapii (např. tamoxifen, inhibitory aromatázy) alespoň 6 měsíců před registrací do A011502.

  • Pacientky musí mít základní měření hustoty prsů, jak je definováno jedním z následujících:

    • >= 25% hustota prsou, popř
    • Rozptýlené oblasti fibroglandulární hustoty, popř
    • Kategorie složení prsou b, c nebo d podle systému hlášení a dat zobrazení prsou (BI-RADS) 2013.

  • Základní digitální screeningový mamogram (mediolaterální [MLO] a kraniokaudální [CC] pohledy) pořízený před registrací musí být k dispozici pro předložení.

    • Pro pacientky, které se zapisují současně s Alliance A011502: Základní digitální screeningový mamogram musí být proveden do 8 týdnů před registrací na A211601. Pokud není k dispozici základní mamogram do 8 týdnů, musí být před léčbou na Alliance A011502 proveden nový screeningový mamogram
    • Pro pacienty, které se zapisují zpětně: Předchozí rutinní mamografické vyšetření pacientky v evidenci musí být do 1 roku před registrací do A011502.
  • Kontralaterální nepostižený prs na místě (bez předchozí rakoviny nebo ozařování, bez implantátů a bez plánu operace prsu na kontralaterálním prsu v průběhu studie). Vhodné jsou pacientky s předchozí biopsií na nepostiženém prsu.
  • Netěhotná a nekojící.

Účastníci poskytují bilaterální screeningový mamogram pořízený před veškerou léčbou rakoviny a do 8 týdnů před registrací na A011502 a roční bilaterální mamograf co nejblíže 1 roku po registraci na A011502 a co nejblíže 2 roky po registraci na A011502 . Účastníci také podstoupí odběr vzorků krve a údajů o menstruačním cyklu do 2 týdnů po registraci a 1 a 2 roky po registraci do A011502.

Mamografická procentuální hustota (MPD) v kontralaterálním (nepostiženém) prsu mezi ramenem s aspirinem a placebem
Časové okno: 1 rok po registraci na A011502
Bude porovnána 1letá mamografická procentuální hustota (MPD) v kontralaterálním (nepostiženém) prsu mezi rameny s aspirinem a placebem. Analýza kovariance (ANCOVA) úprava pro výchozí MPD bude použita k porovnání MPD po 1 roce mezi rameny. Po úpravě o výchozí MPD se dojde k závěru, že 1letá MPD je mezi oběma rameny statisticky odlišná, pokud je odpovídající oboustranná p-hodnota menší než 0,05. Je-li normalita primární proměnné sporná, pak lze jako alternativní přístupy zvážit transformaci proměnné nebo neparametrický Wilcoxonův rank-sum test na hodnotách MPD s jednoduchou změnou. Následná explorativní analýza bude zahrnovat všechny pacienty s MPD vypočtenou na začátku (bez ohledu na výchozí hodnotu) a 1 rok po výchozím stavu.

Bude porovnána 2letá mamografická denzita prsu v kontralaterálním (nepostiženém) prsu mezi dvěma rameny u pacientek s výchozí mamografickou procentní denzitou (MPD) >= 25 % a 2 roky po výchozím MPD. ANCOVA bude použita k porovnání MPD mezi rameny. Odpovídající test rozdílu mezi rameny v MPD za 2 roky bude také proveden na hladině významnosti 0,05.
