Evaluering av mammografisk brysttetthetseffekt av aspirin: En følgestudie til alliansestudie A011502

Denne fase III-studien evaluerer mammografisk brysttetthet hos deltakere med hormonreseptornegativ brystkreft registrert på studie A011502. Høy brysttetthet har vist seg å være en sterk risikofaktor for å utvikle brystkreft, og redusert brysttetthet kan redusere risikoen for brystkreft. Deltakere behandlet med aspirin kan vise redusert brysttetthet på mammografi.

PRIMÆRE MÅL I. Å sammenligne den 1-årige mammografiske brysttettheten i det kontralaterale (upåvirkede) brystet mellom aspirin- og placeboarmen hos pasienter med hormonreseptornegativ eller positiv brystkreft registrert i A011502.

SEKUNDÆRE MÅL I. Å sammenligne den 2-årige mammografiske brysttettheten i det kontralaterale (upåvirkede) brystet mellom aspirin- og placeboarmen hos pasienter med hormonreseptornegativ eller positiv brystkreft registrert i A011502.

OVERSIKT:

Deltakerne gir bilateralt screening mammografi tatt før all kreftbehandling og innen 8 uker før registrering til A011502 og et årlig bilateralt mammografi så nær som mulig 1 år etter registrering til A011502 og så nær som mulig 2 år etter registrering til A011502 . Deltakerne gjennomgår også innsamling av blodprøve og menstruasjonssyklusdata innen 2 uker etter registrering og 1 og 2 år etter registrering til A011502.

Etter fullført studie følges deltakerne opp årlig inntil 10 år fra registrering til studie A011502.

Deltakere med hormonreseptor negativ eller positiv brystkreft registrert i Alliance A011502.

Beskrivelse

Inklusjonskriterier:

  • Må enten være påmeldt samtidig eller tidligere påmeldt til Alliance A011502. Kvalifiserte pasienter kan være enten pre- eller postmenopausale.
  • Pasienter må ha enten hormonreseptor-negativ brystkreft eller være ER+-pasienter som har fullført hormonbehandling (f.eks. tamoxifen, aromatasehemmere) minst 6 måneder før registrering til A011502.

  • Pasienter må ha baseline brysttetthetsmåling som definert av ett av følgende:

    • >= 25 % brysttetthet, eller
    • Spredte områder med fibroglandulær tetthet, eller
    • Brystsammensetning kategori b, c eller d, i henhold til Breast Imaging Reporting and Data System (BI-RADS) 2013.

  • Baseline digital screening mammografi (mediolaterale [MLO] og craniocaudale [CC] visninger) tatt før registrering må være tilgjengelig for innsending.

    • For pasienter som registrerer seg samtidig med Alliance A011502: Baseline digital screening mammografi må tas innen 8 uker før registrering til A211601. Hvis en baseline mammografi innen 8 uker ikke er tilgjengelig, må en ny screening mammografi utføres før behandling på Alliance A011502
    • For pasienter som registrerer seg retrospektivt: Pasientens tidligere rutinemammogram må være innen 1 år før registrering til A011502.
  • Kontralateralt upåvirket bryst på plass (uten tidligere kreft eller stråling, ingen implantater og ingen plan for brystkirurgi på kontralateralt bryst i løpet av studien). Pasienter med tidligere biopsi på det upåvirkede brystet er kvalifisert.
  • Ikke gravid og ikke ammende.

Deltakerne gir bilateralt screening mammografi tatt før all kreftbehandling og innen 8 uker før registrering til A011502 og et årlig bilateralt mammografi så nær som mulig 1 år etter registrering til A011502 og så nær som mulig 2 år etter registrering til A011502 . Deltakerne gjennomgår også innsamling av blodprøve og menstruasjonssyklusdata innen 2 uker etter registrering og 1 og 2 år etter registrering til A011502.

Tidsramme: Ved 1 år etterregistrering til A011502
Den 1-årige mammografiske prosentdensiteten (MPD) i det kontralaterale (upåvirkede) brystet mellom aspirin- og placeboarmene vil bli sammenlignet. Analyse av kovarians (ANCOVA) justering for baseline MPD vil bli brukt for å sammenligne MPD ved 1 år mellom armene. Etter justering for baseline MPD, vil det konkluderes med at 1-års MPD er statistisk forskjellig mellom de to armene dersom den tilsvarende tosidige p-verdien er mindre enn 0,05. Hvis normaliteten til den primære variabelen er tvilsom, kan variabel transformasjon eller ikke-parametrisk Wilcoxon rangsumtest på enkle MPD-verdier betraktes som alternative tilnærminger. En påfølgende eksplorativ analyse vil inkludere alle pasienter med en MPD beregnet ved baseline (uavhengig av denne baseline-verdien) og 1 år etter baseline.
